INTRODUÇÃO: Do início do século XX até a década de 70, quando a distonia foi amplamente aceita como distúrbio orgânico, intenso debate sobre sua etiologia a fez ocupar uma área cinzenta entre neurologia e psiquiatria. Desde então, avanços significativos no conhecimento relacionado à sua fisiopatologia foram alcançados, em decorrência principalmente de descobertas no campo da genética. No entanto, existem escassos estudos genéticos entre pacientes brasileiros com distonia. OBJETIVO: O principal objetivo deste estudo foi descrever as manifestações clínicas e os achados genéticos em pacientes com distonia de etiologia desconhecida, isolada ou combinada a parkinsonismo ou mioclonia. MÉTODOS: Trata-se de um estudo retrospectivo no qual foi analisada a base de dados criada pela Rede Brasileira para o Estudo das Distonias, que incluiu pacientes com distonia idiopática, isolada ou combinada a parkinsonismo ou mioclonia, com ou sem história familiar, sem evidência de etiologia degenerativa ou secundária. Essa base de dados contém informação epidemiológica, clínica (exame neurológico, questionário de sintomas motores e não motores e escore de gravidade dos sintomas motores de acordo com a escala de avaliação de distonia de Fahn-Marsden) e genética de pacientes recrutados no ambulatório de distúrbios do movimento de quatro hospitais do Estado de São Paulo. Em uma primeira fase, foram pesquisadas variantes nos genes TOR1A, GCH1, THAP1, SGCE e ATP1A3 por sequenciamento direto. Para pacientes com resultado negativo, assim como para novos participantes incluídos na segunda fase do estudo, foram realizadas análises por sequenciamento de nova geração, utilizando um painel de 12 genes envolvidos na distonia, incluindo TOR1A, TUBB4A, GCH1, TH, THAP1, SGCE, ATP1A3, PRKRA, CIZ1, ANO3, GNAL e KCTD17. Somente dados de paciente recrutados no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo foram utilizados na presente tese. RESULTADOS: Foram incluídos 87 pacientes (53 do gênero feminino), a maioria com início dos sintomas até 20 anos de idade (63,2%) e 44,8% com distonia generalizada. Três pacientes apresentavam parkinsonismo e dois mioclonia. Sintomas não motores foram relatados por 89,6% dos pacientes, sendo dor a queixa mais comum (51,7%). Variantes patogênicas ou provavelmente patogênicas foram identificadas em 29 pacientes, sendo mais frequentes em THAP1 (11 pacientes) e GCH1 (9), seguidos por TOR1A (4), PRKRA (3), SGCE (1) e TUBB4A (1). Sete variantes não haviam sido previamente relatadas na literatura: uma em TUBB4A (c.883G > A), duas em GCH1 (c.del41_44G e c.439C > T), três em THAP1 (c.62C > T, c.107_110insG e c.348delT) e uma em PRKRA (c.795C > A). Início precoce da distonia e história familiar foram preditores independentes da presença de variante patogênica ou provavelmente patogênica. CONCLUSÃO: Diferentes formas genéticas de distonia foram observadas em pacientes brasileiros e novas variantes foram identificadas, o que pode contribuir para melhorar o conhecimento atual da fisiopatologia dessa condição. O desenvolvimento de tratamentos clínicos e cirúrgicos baseados no diagnóstico etiológico pode trazer significativo benefício para os nossos pacientes

BACKGROUND: From the begining of the 20th century until the seventies, when dystonia was broadly accepted as an organic disorder, intense debate about its etiology has placed it into a grey zone between neurology and psychiatry. Since then, important advances in the knowledge related to its pathophysiology were achieved, mostly due to discoveries in the genetics field. However, there are scarce genetic studies among Brazilian patients with dystonia. OBJECTIVE: The main objective of this study was to describe the clinical manifestations and genetic findings in patients with dystonia of unkown etiology, isolated or combined with parkinsonism or myoclonus. METHODS: This is a retrospective study in which we analyzed the database created by the Brazilian Network for the Study of Dystonia, including patients with idiopathic dystonia, isolated or combined with parkinsonism or myoclonus, with or without family history, without evidence of degenerative or secondary etiology. This database includes epidemiological, clinical (neurological examination, motor and non-motor symptoms questionnaires and score of motor severity according to the FahnMarsden Dystonia Rating Scale) and genetic information of patients recruited from the movement disorders outpatient units of four hospitals in São Paulo. In a first phase, variants in the genes TOR1A, GCH1, THAP1, SGCE e ATP1A3 were searched for by direct sequencing. For patients with negative results, as well as for those enrolled in the second phase of the study, analyses through nextgeneration sequencing were performed, using a painel of 12 genes involved in dystonia, including TOR1A, TUBB4A, GCH1, TH, THAP1, SGCE, ATP1A3, PRKRA, CIZ1, ANO3, GNAL e KCTD17. Only patients recruited in the Clinical Hospital of the University of São Paulo Medical School were included in the present thesis. RESULTS: Eighty-seven patients (53 women) were included, the majority of them with disease onset until 20 years (63.2%) and 44.8% with generalized dystonia. Three patients had parkinsonism and two had myoclonus. Non-motor symptoms were reported by 89.6% of patients, with pain being the most common complaint (51.7%). Pathogenic or likely pathogenic variants were identified in 29 patients, being more frequent in THAP1 (11 patients) and GCH1 (9), followed by TOR1A (4), PRKRA (3), SGCE (1) and TUBB4A (1). Seven variants had not been previously described in the literature: one in TUBB4A (c.883G > A), two in GCH1 (c.del41_44G e c.439C > T), three in THAP1 (c.62C > T, c.107_110insG e c.348delT) and one in PRKRA (c.795C > A). Early-onset dystonia and family history were independent predictors of the presence of pathogenic or likely pathogenic variants. CONCLUSION: Different genetic forms of dystonia were observed among Brazilian patients and new variants were identified, which may contribute to improving the current knowledge of the pathophysiology of this condition. The development of clinical and surgical treatments based on etiological diagnosis can bring relevant benefit to our patients
