Introdução: A distrofia muscular por deficiência de merosina (DM-LAMA2) é uma doença genética de herança autossômica recessiva causada por mutações no gene LAMA2. Pode apresentar-se de forma variável que vai desde uma forma congênita grave até uma forma mais leve de início tardio. Os pacientes apresentam, além da fraqueza muscular, alterações osteoesqueléticas e distúrbio ventilatório. A maior parte dos pacientes não adquire a capacidade de andar. Os pacientes também apresentam manifestações no SNC caracterizadas por alteração na substância branca cerebral e, mais raramente, malformações corticais na RNM, déficit cognitivo e epilepsia. As alterações genéticas que levam às formas mais graves ou mais leves da doença, assim como presença ou ausência de manifestações do SNC ainda não estão completamente estabelecidas. Também faltam estudos relevantes de história natural que permitam compreender aspectos essenciais da evolução da doença. Objetivo: Avaliar os aspectos clínicos, moleculares e manifestações do SNC de pacientes com DM-LAMA2. Métodos: Foram incluídos pacientes com diagnóstico genético ou histológico de DM-LAMA2. Os pacientes foram avaliados segundo protocolo clínico/neurológico geral, além de escalas motoras, avaliação respiratória, goniometria, RNM de encéfalo e biópsia muscular. O gene LAMA2 foi avaliado através de sequenciamento de nova geração. Resultados: Foram incluídos 63 pacientes com DM-LAMA2 que se distribuíram em dois grupos distintos. O primeiro composto de 52 pacientes (82.5%) que nunca adquiriram a marcha e o segundo, de 11 (17.5%) pacientes que foram capazes de andar. Cinquenta e dois pacientes realizaram RNM de encéfalo, e em todos havia algum grau de alteração da substância branca cerebral; 10 (19.2%) apresentaram malformações corticais cerebrais. Dentre os 52 pacientes que realizaram RNM cerebral, dez (19,2%) tinham epilepsia e oito (15,4%) deficiência intelectual. Todos os pacientes com malformação cortical e epilepsia pertenciam ao grupo de pacientes não deambulantes. Entre os pacientes com malformações corticais não foram observadas mutações com variantes missense. Malformações corticais foram mais frequentes entre pacientes com variantes que afetam o domínio LG (região que se liga a -distroglicana e integrinas) (p=0,016). Os pacientes não deambulantes apresentaram queda progressiva da função respiratória de 1,85%/ano (p=0.006) enquanto os que adquiriram a capacidade para marcha tiveram queda de 0,98%/ano (p=0.028). O domínio D2 da escala motora MFM foi o mais sensível para medir a progressão da doença entre não deambulantes, com piora de 4.2% da função motora/ano. A goniometria também mostrou uma piora progressiva do grau das retrações de joelhos e cotovelos ao longo do tempo. Dentre as mutações, foram identificadas 45 variantes diferentes, das quais 21 nunca foram previamente publicadas. Quatorze pacientes (22.6%) apresentaram uma variante nova fora de fase (c.1255del; p. Ile419Leufs*4) que parece ser um efeito fundador brasileiro. Pudemos observar uma relação entre a presença de ao menos uma variante não nula e a capacidade de se adquirir a marcha (p<0.0001). Conclusão: A DM-LAMA2 é uma doença de amplo espectro de gravidade com alterações que vão além do músculo esquelético. Existe uma relação entre a gravidade motora e o acometimento do SNC e existe uma relação entre o genótipo, força muscular e presença de malformação cortical. Esse estudo trouxe dados robustos e reprodutíveis que vão auxiliar na assistência e na antecipação dos cuidados ao paciente, contribuindo para o planejamento de estudos clínicos de tratamento

Introduction: LAMA2 related muscular dystrophy (LAMA2-RD) is an autosomal recessive genetic disease caused by mutations in the LAMA2 gene. It can present itself in a variable form, ranging from a severe congenital form to a milder form of late onset. Patients present, in addition to muscle weakness, joint contractures, scoliosis, and respiratory insufficiency. Most patients do not acquire the ability to walk. Patients also have CNS manifestations characterized by changes in the cerebral white matter on imaging tests and, more rarely, cortical malformations on MRI, cognitive impairment, and epilepsy. The genetic alterations that lead to more severe or milder forms of the disease, as well as the presence or absence of CNS manifestations, are not yet fully established. There is also a lack of relevant natural history studies that allow us to understand essential aspects of the disease evolution. Objective: The aim of this study was to evaluate the clinical, molecular and CNS manifestations of patients with LAMA2-RD. Methods: Patients with genetic or histological diagnosis of LAMA2-RD, under regular medical follow-up, were included. The patients were evaluated according to a general clinical/neurological protocol, in addition to motor scales, respiratory assessment, goniometry, brain MRI and muscle biopsy. The LAMA2 gene was evaluated through next generation sequencing. Results: 63 patients with LAMA2-RD were included, distributed into two distinct groups. The first comprised 52 patients (82.5%) who never acquired the gait and the second, 11 (17.5%) patients who were able to walk. Fifty-two patients underwent brain MRI, and all of them had some degree of alteration in the cerebral white matter; and 10 (19.2%) had cerebral cortical malformations. Among the 52 patients who underwent brain MRI, ten (19.2%) had epilepsy and eight (15.4%) intellectual disabilities. All patients with cortical malformation and epilepsy belonged to the non-ambulatory group. Among patients with cortical malformations, mutations with missense variants were not observed. Cortical malformations were more frequent among patients with variants that affect the LG domain (region that binds-dystroglycan and integrins) (p=0.016). Non-ambulatory patients had a progressive decrease in forced vital capacity of 1.85%/year (p=0.006) while those who acquired the ability to walk had a decrease of 0.98%/year (p=0.028). The D2 domain of the MFM motor scale showed a statistically significant reduction of 4.2% in motor function per year among non-ambulatory patients. The goniometry measurement showed a progressive worsening of the degree of knee and elbow retractions over time. Among the mutations, 45 different variants were identified, of which 21 were never previously published. Fourteen patients (22.6%) had a novel out-of-phase variant (c.1255del; p. Ile419Leufs*4) that appears to be a Brazilian founder effect. We could observe a relationship between the presence of at least one non-null variant and the ability to acquire gait (p<0.0001). Conclusion: DM-LAMA2 is a disease with a broad spectrum of severity with symptoms that go beyond skeletal muscle. There is a relationship between motor severity and CNS involvement and there is a relationship between genotype, muscle strength and the presence of cortical malformation. This study brought robust and reproducible data that will help patients assistance and contribute to the planning of clinical trials
