Introdução: A inflamação crônica de baixo grau pode estar associada ao desenvolvimento de doenças crônicas não transmissíveis em populações jovens, geralmente durando até a idade adulta. Estudos mostram que a dieta está relacionada à inflamação crônica. O índice inflamatório da dieta (IID) é um indicador que mede o potencial inflamatório da dieta, com o auxílio de ferramentas validadas que avaliam o consumo alimentar. A validação de ferramentas que avaliam padrões inflamatórios alimentares em populações jovens que permitam resultados válidos e confiáveis é essencial para orientar estratégias de prevenção de doenças na vida adulta. Objetivo: Estimar o índice inflamatório da dieta (IID) em crianças e adolescentes na América do Sul e testar sua confiabilidade e validade por meio de um QFA e um biomarcador inflamatório. Método: Este trabalho consiste em um estudo de validação em uma amostra de crianças e adolescentes desenvolvido na América do Sul (Estudo SAYCARE). O consumo habitual de alimentos que contribuíram para o cálculo do IID foi obtido por meio de um QFA e um R24H. A confiabilidade foi testada usando o QFA (QFA1xQFA2), usando o coeficiente de correlação de Spearman para estimar a concordância entre as medidas. A validade do IID foi testada usando um 24HDR e um biomarcador inflamatório (PCR) usando correlação de Spearman e regressão linear multinível. Resultados: Para crianças e adolescentes, os grupos de alimentos pró e anti-inflamatórios apresentaram coeficiente de correlação de Spearman variando de 0,31 a 0,66, validade convergente variando de 0,09 a 0,40 e validade de critério para confiabilidade variando de -0,03 a 0,18. O IID apresentou coeficiente de correlação de Spearman para confiabilidade variando de 0,61 a 0,69, validade convergente de 0,16 a 0,23 e validade de critério de -0,03 a 0,24. Conclusão: O IID apresentou confiabilidade aceitável, validade convergente fraca e validade de critério fraca em crianças e adolescentes

Background: Low-grade chronic inflammation may be associated with developing chronic non-communicable diseases in young populations, often lasting into adulthood. Studies show that the diet is related to chronic inflammation. The Dietary Inflammatory Index is an indicator that measures the inflammatory potential of the diet, with the help of validated tools that assess food consumption. The validation of tools that assess inflammatory dietary patterns in young populations that allow valid and reliable results is essential to guide disease prevention strategies for adulthood. Objectives: This study aimed to estimate the Dietary Inflammatory Index (DII) in children and adolescents in South America, and to test its reliability and validity using an FFQ and an inflammatory biomarker. Methods: This work consists of a validation study in a sample of children and adolescents developed in South America (SAYCARE Study). The usual consumption of foods that contributed to the calculation of the IID was obtained through a FFQ and a 24HDR. Reliability was tested using the FFQ (FFQ1xFFQ2), using the Spearman correlation coefficient to estimate the agreement between measurements. The validity of the DII was tested using a 24HDR and an inflammatory biomarker (CRP) using Spearman correlation and multilevel linear regression. Results: For children and adolescents, the pro and anti-inflammatory food groups showed Spearman's correlation coefficient ranging from 0.31 to 0.66, convergent validity ranging from 0.09 to 0.40, and criterion validity for reliability ranging from -0.03 to 0.18. The DII showed Spearman's correlation coefficient for reliability ranging from 0.61 to 0.69, convergent validity from 0.16 to 0.23, and criterion validity from -0.03 to 0.24. Conclusion: The DII showed acceptable reliability, weak convergent validity and weak criterion validity in children and adolescents
