A Migração Internacional é atualmente considerada um grande desafio global. No final de 2019, segundo o Alto Comissariado das Nações Unidas para Refugiados, 79,5 milhões de pessoas se encontravam em situação de deslocamento forçado em todo o mundo. Entre estes, havia 26,0 milhões de refugiados, sendo 6,6 milhões deles provenientes da República Árabe Síria. Este país encontra-se em guerra civil desde 2011, vivendo a maior crise humanitária mundial. O Brasil está longe de ser o principal destino dos refugiados sírios, porém estes constituem o principal grupo presente no país, instalados sobretudo na cidade de São Paulo. O tema da saúde dos refugiados é uma questão bastante relevante, sendo a Atenção Primária à Saúde uma estratégia e um lugar privilegiado para o cuidado da saúde destas pessoas. Ao alcançarem as cidades de países, em sua grande maioria, em desenvolvimento, se deparam com realidades diversas sob vários aspectos, desde culturais até com relação ao modo como se organiza o sistema de saúde local, e sobretudo a qual cardápio de serviços poderá ter acesso. Para os serviços de Atenção Primária, também é um grande desafio atender as demandas e necessidades de saúde desses usuários, exigindo plasticidade, e sobretudo do médico, o desenvolvimento de competência cultural. No caso do Brasil, a Estratégia da Saúde da Família é o principal dispositivo reorganizador do modelo tecnoassistencial no âmbito da Atenção Primária à Saúde. Assim, o objetivo deste trabalho é analisar a interação entre os refugiados sírios e uma unidade básica de saúde que funciona sob a lógica da Estratégia Saúde da Família, localizada na região central da cidade de São Paulo. Em especial, interessa-nos analisar a percepção dos médicos das Equipes de Saúde da Família e desses usuários, a partir de suas demandas e necessidades de saúde. Trata-se de uma pesquisa de referencial qualitativo, que utilizou a técnica de entrevistas semiestruturadas com refugiados sírios e médicos. Para a análise das entrevistas, utilizou-se a técnica de análise temática de conteúdo, considerando as conjunturas, as razões e as lógicas, bem como as ações e as inter-relações estabelecidas com o coletivo e as instituições. Os resultados exibem as particularidades da relação deste grupo de refugiados com o sistema de saúde, em especial com a Atenção Primária à Saúde. Podemos reconhecer a permanência das dificuldades comunicacionais, e tentativas de solucioná-las. Os impactos sobre a saúde desses indivíduos decorrentes da exposição à precariedade das condições de trabalho, podem ser identificadas. A saúde mental dos refugiados sírios é desafiada em todos os momentos do processo migratório, que tem ponto de partida, porém o destino é incerto. Este tensionamento se expressa em manifestações clínicas, mas também em manifestações de resiliência. Durante o atendimento médico aos refugiados sírios identificamos demandas que tem suas raízes em racionalidades religiosas e culturas, e que exigem destes profissionais o desenvolvimento da capacidade de manejá-las sem juízo de valor, e a necessidade de articular-se com outros recursos existentes nas comunidades

International migration is currently considered a major global challenge. At the end of 2019, according to the United Nations High Commissioner for Refugees, 79.5 million people were displaced worldwide. Among these, there were 26.0 million refugees, 6.6 million of whom came from the Syrian Arab Republic. This country has been in civil war since 2011, experiencing the biggest humanitarian crisis in the world. Brazil is far from being the main destination for Syrian refugees, but they are the main group present in the country, installed mainly in the city of São Paulo. The issue of refugee health is a very relevant issue, with Primary Health Care being a strategy and a privileged place for the health care of these people. When reaching the cities of countries, most of which are in development, they face different realities in various aspects, from cultural to the way in which the local health system is organized, and above all to which the menu of services may have access. For Primary Care services, it is also a great challenge to meet the health demands and needs of these users, requiring plasticity, and above all from the doctor, the development of cultural competence. In the case of Brazil, the Family Health Strategy is the main reorganizing device of the techno-assistance model in the context of Primary Health Care. Thus, the objective of this work is to analyze the interaction between Syrian refugees and a basic health unit that works under the logic of the Family Health Strategy, located in the central region of the city of São Paulo. In particular, we are interested in analyzing the perception of doctors from the Family Health Teams and these users, based on their health demands and needs. It is a qualitative reference research, which used the technique of semi-structured interviews with Syrian refugees and doctors. For the analysis of the interviews, the thematic content analysis technique was used, considering the circumstances, the reasons and the logics, as well as the actions and the interrelations established with the collective and the institutions. The results show the particularities of the relationship of this group of refugees with the health system, especially with Primary Health Care. We can recognize the permanence of communication difficulties, and attempts to solve them. The impacts on the health of these individuals resulting from exposure to precarious working conditions can be identified. The mental health of Syrian refugees is challenged at all times during the migration process, which has a starting point, but the destiny is uncertain. This tension is expressed in clinical manifestations, but also in manifestations of resilience. During medical care for Syrian refugees, we identified demands that have their roots in religious rationalities and cultures, and that demand from these professionals the development of the capacity to manage them without judgment, and the need to articulate with other resources existing in the communities
