INTRODUÇÃO: As taxas de infecção pelo HIV entre homens que fazem sexo com homens permanecem elevadas, especialmente em países onde a epidemia se apresenta de modo concentrado em populações-chave. Com o intuito de diminuir a vulnerabilidade dessas populações à infecção, medidas combinadas de prevenção vêm sendo implementadas. Dentre essas, o autoteste é uma estratégia diagnóstica capaz de permitir a ampliação da cobertura de detecção da infecção, ao alcançar indivíduos que não procuram serviços de saúde para realização da testagem convencional. Dessa forma, o autoteste, como medida de autocuidado, pode contribuir para a promoção, proteção e realização de direitos. OBJETIVO: Analisar, na perspectiva dos direitos humanos, os significados atribuídos ao autoteste anti-HIV em fluido oral por homens que fazem sexo com homens e identificar os limites e potencialidades dessa estratégia na realização de direitos. MÉTODOS: Para este estudo com abordagem qualitativa, conduzido na cidade de São Paulo, foram convidados homens que fazem sexo com homens, com idade igual ou superior a 18 anos, que tivessem realizado o autoteste anti-HIV no âmbito de sua participação na pesquisa A Hora é Agora-SP. Os dados foram coletados por meio de entrevista semiestruturada, utilizando-se abordagem que proporcionasse a emergência de discursos livres por parte dos entrevistados. Os depoimentos foram gravados, transcritos e analisados segundo a técnica de Análise de Conteúdo de Bardin. A síntese interpretativa do material empírico da pesquisa, baseou-se no referencial dos Direitos Humanos Aplicados à Saúde. RESULTADOS: A amostra estudada foi composta por 18 homens que fazem sexo com homens, dos quais 56% se encontrava na faixa etária entre 20 a 25 anos e 83% havia ingressado no ensino superior. Apenas um informou estar desempregado no momento da entrevista; 61% se considerou homossexual e 33% bissexual; 67% relatou ser sua orientação sexual assumida publicamente; apenas 11% informou nunca ter vivenciado alguma forma de preconceito, estigma ou discriminação; e 78% dos participantes já haviam realizado algum tipo de teste anti-HIV anteriormente. Na análise qualitativa dos depoimentos, emergiram quatro categorias: motivação para a realização do autoteste, acesso ao autoteste, confiança/desconfiança no autoteste, e sentimentos e percepções decorrentes da testagem. O autoteste demonstrou potencial em contribuir para a promoção, proteção e realização do direito à saúde (em seus componentes de acessibilidade e aceitabilidade), do direito à igualdade e à não discriminação, do direito à informação, do direito à tomada de decisão informada, do direito à privacidade e à confidencialidade, e do direito ao usufruto dos benefícios do progresso científico e suas aplicações. CONCLUSÕES: A vulnerabilidade de um grupo envolve múltiplas violações de direitos humanos ou sua não plena realização. Nossos resultados permitem concluir que o autoteste anti-HIV ajudaria a mitigar essa dimensão da vulnerabilidade ao contribuir para a promoção e a proteção de relevantes direitos humanos. O referencial dos Direitos Humanos aplicados à saúde mostrou-se útil para evidenciar as potencialidades e limitações de uma nova tecnologia como promotora de direitos, que deverão ser consideradas para a implantação do autoteste em larga escala no Sistema Único de Saúde

INTRODUCTION: HIV infection rates among men who have sex with men remain high, especially in countries where the epidemic is concentrated in key populations. In order to reduce the vulnerability of these populations to infection, combined prevention measures have been implemented. Among these, self-testing is a diagnostic strategy capable of expanding the coverage of detection of HIV infection, by reaching individuals who do not seek health services for conventional testing. As such, the self-test, as a self-care measure, can contribute to the promotion, protection, and realization of rights. AIM: To analyse, from a human rights perspective, the meanings attributed to the anti-HIV selftest in oral fluid by men who have sex with men, and to identify the limits and potentialities of this strategy in the realization of relevant human rights. METHODS: This is a qualitative study, conducted in the city of São Paulo, with men who have sex with men aged 18 years or older, who had performed an anti-HIV self-test within the scope of their participation in the The Time is Now-SP study. Data were collected through semistructured interviews to allow emergence of free speeches of the interviewees. The interviews were recorded, transcribed, and analysed using Bardin's Content Analysis technique. The interpretative synthesis of collected data was based on the Human Rights Applied to Health framework. RESULTS: The studied sample consisted of 18 men who have sex with men, of whom 56% were aged 20 to 25 years, and 83% had started higher education. Only one participant reported being unemployed at the time of the interview; 61% considered themselves homosexual and 33% bisexual; 67% reported that their sexual orientation was publicly acknowledged; only 11% reported never having experienced any episode of prejudice, stigma, or discrimination; and 78% of participants had already undergone anti-HIV testing before. In the qualitative analysis of the interviews, four categories emerged: motivation to perform the self-test, access to the self-test, trust/distrust in the self-test, and feelings and perceptions resulting from the testing experience. The HIV self-test showed potential in contributing to the promotion, protection, and realization of the right to health (in its accessibility and acceptability components), the right to equality and non-discrimination, the right to information, the right to informed decision-making, the right to privacy and confidentiality, and the right to enjoy the benefits of scientific progress and its applications. CONCLUSIONS: The vulnerability of a social group involves multiple violations of human rights or their nonfulfilment. Our results allow us to conclude that the HIV self-test would help mitigate this dimension of vulnerability by contributing to the promotion and protection of relevant human rights. The framework of Human Rights applied to health proved to be useful to highlight the potential and limitations of a new technology as a promoter of rights, which should be considered for the implementation of the self-test on a large scale in the Brazilian Unified Health System (SUS)
