Mesmo com os constantes avanços científicos em relação ao tratamento e prevenção do HIV/Aids, a epidemia continua representando um desafio aos pesquisadores e à saúde pública. A incidência desta doença permanece elevada em todo o mundo, com cerca de 1,5 milhões de novas infecções no ano de 2020. A prevenção combinada é atualmente a estratégia preconizada no combate à epidemia, destacando-se dentre os diversos métodos preventivos a Profilaxia Pré-Exposição (PrEP), que utiliza medicamentos orais para indivíduos de alta vulnerabilidade. A PrEP passou a fazer parte do arsenal de estratégias de prevenção disponíveis no Sistema Único de Saúde (SUS) brasileiro no início do ano de 2018. A eficácia da PrEP para prevenir a infecção pelo HIV foi comprovada em diversos estudos, mas benefícios adicionais desta nova estratégia, tais como melhora da qualidade de vida sexual, afetiva e emocional, foram pouco explorados até o momento. O direito à saúde sexual e reprodutiva, incluindo atenção à função e satisfação sexual, tem sido enfatizado por diversos organismos internacionais e pelo Ministério da Saúde do Brasil. Este estudo avaliou os efeitos da PrEP na qualidade de vida sexual e saúde mental de seus usuários após a adoção desta medicação. Para isso, utilizamos metodologia de estudo observacional de corte transversal. Participantes em potencial, usuários de PrEP, foram recrutados nos serviços de saúde e convidados a responder ao questionário de avaliação da qualidade de vida sexual e inventários de ansiedade e depressão. Participantes selecionados foram convidados a uma entrevista em profundidade. No total, 221 participantes com mediana de idade de 33 anos responderam ao questionário do estudo, dentre os quais 216 (98%) identificaram-se homens; tinham alta escolaridade (89% graduação completa) e a maioria (71%) raça/cor branca. Observamos efeitos relevantes ao uso da PrEP sobre aspectos da libido, excitação, ereção e satisfação sexual, sendo que 41,6% afirmaram melhora em ao menos um dos itens após início da PrEP. Outro achado relevante que foi nomeado Alívio, 54,3% respondem positivamente a todos os questionamentos que se referem a diminuição da interferência por receio do HIV durante ou depois das relações sexuais, especificamente 69% dos participantes relataram pensar menos/muito menos no HIV; 73% relataram ficar menos/muito menos preocupados com a possibilidade de contrair HIV; 73% relataram que a possibilidade de infecção pelo HIV atrapalha menos/muito menos frequentemente na qualidade da relação. Observamos presença de ansiedade em 44% e depressão em 20% da amostra. As entrevistas em profundidade contaram com 10 participantes, sendo 9 homens cisgênero e 1 homem transgênero; 8 refereriram-se gays e 2 bissexuais. A faixa etária variou entre 23 e 60 anos; 7 referiram raça/cor branca e 3 referiram ser pretos/pardos; todos tinham alta escolaridade (5 graduados, 4 pós-graduados e 1 graduando). As entrevistas indicaram presença de benefícios que extrapolam a proteção biológica da PrEP ao HIV, estendendo-se para a saúde mental/emocional, diminuição na percepção de preconceito e estigmas sociais, ampliação da consciência, acesso aos cuidados com sua própria saúde e melhora na qualidade de vida sexual

Despite the scientific improvements in the treatment and prevention of HIV/Aids, the epidemic remains a significant challenge to researchers and to public health. HIV incidence is still high worldwide, with approximately 1.5 million new cases in 2020. Currently, combination prevention is the recommended strategy against the disease, with special focus on the pre-exposure prophylaxis (PrEP); PrEP consists in offering oral antiretroviral medications to individuals under high vulnerability to HIV. PrEP became part of the publicly available HIV prevention strategies in Brazil in 2018. PrEP efficacy in preventing HIV infection was shown in several clinical studies, but additional benefits of this strategy, including improvements in sexual quality of life as well as in affective/emotional aspects, have been scarcely explored to date. Aspects of sexual and reproductive rights, including sexual function and satisfaction, have been emphasized by several international organisms as well as the Ministry of Health of Brazil. This study addressed benefits of PrEP for the sexual quality of life and mental health among PrEP users. We used an observational, cross-sectional strategy. Potential participants identified as PrEP users were recruited in health facilities and invited to answer a sexual quality of life questionnaire as well as the anxiety and depression scale. In total, 221 participants with median 33 years old were included; 216 (98%) identified as males; most participants had high schooling (89% had higher education) and most (71%) self-reported to be white/Caucasian. Anxiety was detected in 44% and depression was detected in 20% of the sample. We found significant effects of PrEP use on aspects of sexual drive, arousal, penile erection and satisfaction, with 42% of the study participants reporting improvement in at least one of these items following PrEP initiation. Furthermore, 54% reported improvement in all three items of the questionnaire related to thinking of HIV, concerns and anxiety related to the risk of acquiring HIV through a sexual intercourse. We found no statistically significant associations between sociodemographics, anxiety and depression and the study outcomes. Our findings highlight the existence of benefits of PrEP use beyond HIV prevention, extending to sexual health and sexual satisfaction, comprising an additional stimulus to the adoption of this prevention method
