Introdução: A progressão da hepatite C crônica para fibrose hepática envolve diversos fatores, entre os fatores metabólicos destaca-se a esteatose hepática. A esteatose hepática na infecção pelo vírus da hepatite C (HCV) é complexa e envolve fatores virais e do hospedeiro. Alguns polimorfismos de nucleotídeos únicos (SNPs) parecem influenciar as concentrações dos lipídeos plasmáticos e o desenvolvimento da esteatose hepática em pacientes com HCV. Objetivo: Detectar e avaliar a influência dos SNPs -164T/C, -400A/T e H297Q no gene da proteína de transferência de triglicerídeo microssomal (MTTP) na esteatose hepática e nos níveis de lipídeos plasmáticos em pacientes com hepatite C crônica. Métodos: Os SNPs -164T/C, -400A/T e H297Q foram genotipados em 236 pacientes com hepatite C crônica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. A genotipagem foi realizada pela técnica de reação em cadeia da polimerase seguida de análise de polimorfismo de tamanho de fragmentos de restrição (PCR-RFLP), na qual após a amplificação de um fragmento específico do DNA foi realizada a digestão por enzimas de restrição e as bandas referentes a cada genótipo foram visualizadas em gel de agarose. Foi avaliada a associação de características com os genótipos de cada SNP em diferentes modelos genéticos (codominante, dominante e recessivo). Foram realizadas análises estatísticas bivariadas e multivariadas com o auxílio do programa IBM-SPSS. Resultados: No presente estudo, 56,4% dos participantes eram mulheres e a média de idade foi 55,5 anos (29-83 anos). O genótipo mais prevalente do SNP -164T/C foi o homozigoto selvagem (TT) e dos SNPs -400A/T e H297Q foram os heterozigotos (AT e HQ, respectivamente). As frequências dos alelos mutados dos SNPs -164T/C, -400A/T e H297Q foram de 0,30, 0,41 e 0,50, respectivamente. As distribuições genotípicas dos três SNPs no gene MTTP estão em equilíbrio de Hardy-Weinberg. O alelo mutado do SNP -164T/C foi associado com idade avançada, presença de hipertensão arterial, níveis elevados de ferro, ferritina e insulina (p < 0,05). Sendo que o nível elevado de insulina foi associado com o SNP -164T/C nos três modelos genéticos estudados. O alelo mutado do SNP -400A/T foi associado com diabetes (p=0,005) e o alelo selvagem com siderose (p=0,030) nos modelos genéticos co-dominante e dominante. O alelo mutado do SNP H297Q foi associado com o genótipo 3 do HCV (modelo co-dominante), presença de hipertensão arterial (modelos co-dominante e dominante) e nível elevado de insulina (modelo dominante) (p < 0,05). Nenhum dos SNPs foi associado com alterações nos níveis de lipídeos e com os diferentes graus de esteatose hepática. A presença do alelo mutado do SNP -164T/C em conjunto com a infecção pelo genótipo 3 do HCV, foi associada na análise multivariada com a esteatose (p=0,004). Ainda em relação ao SNP-164T/C, o nível elevado de GGT em conjunto com a presença do alelo mutado foi associado com a esteatose (p=0,006). A presença do alelo mutado do SNP -400A/T em conjunto com a infecção pelo genótipo 3 do HCV, também foi associada com a esteatose (p=0,032). A análise do SNP H297Q em conjunto com as características avaliadas não apresentou associação com a esteatose (p > 0,05). Conclusões: Foi detectada a presença de todos os genótipos dos SNPs -164T/C, -400A/T e H297Q no gene MTTP e não foram associados com alterações nos níveis de lipídeos. A esteatose hepática em pacientes com hepatite C crônica foi associada com os SNPs -164T/C e -400A/T em conjunto com a infecção pelo genótipo 3 do HCV, e com o SNP -164T/C em conjunto com elevação no nível de GGT. Portanto, em pacientes com hepatite C crônica, as características do hospedeiro e do vírus podem ser investigadas em conjunto

Introduction: The chronic hepatitis C progression to liver fibrosis involves several factors; among the metabolic factors, hepatic steatosis is observed. Hepatic steatosis in hepatitis C virus (HCV) infection is complex, involves both viral and host factors. Single nucleotide polymorphisms (SNPs) seem to influence the plasma lipid concentrations and the development of hepatic steatosis in HCV patients. Objective: Detect and evaluate the influence of the -164T/C, -400A/T, and H297Q SNPs in the microsomal triglyceride transfer protein (MTTP) gene on hepatic steatosis and plasma lipid levels in patients with chronic hepatitis C. Methods: The -164T/C, -400A/T and H297Q SNPs were genotyped in 236 patients with chronic hepatitis C of the Clinical Hospital of the School of Medicine, University of Sao Paulo. Genotyping was performed by the polymerase chain reaction assay followed by restriction fragment length polymorphism (PCR-RFLP) analysis, in which after the amplification of a specific DNA fragment, digestion was performed by restriction enzymes and each genotype bands were visualized on agarose gel. Several characteristics were evaluated with the genotypes of each SNP in different genetic models (codominant, dominant and recessive). Bivariate and multivariate statistical analyzes were performed using the software IBM-SPSS. Results: In the present study, 56.4% of the participants were women and the mean age was 55.5 years (29-83 years). The most prevalent genotype of the -164T/C SNP was the wild-type homozygous (TT) and the -400A/T and H297Q SNPs were the heterozygous (AT and HQ, respectively). The frequencies of mutated alleles in the -164T/C, -400A/T and H297Q SNPs were 0.30, 0.41 and 0.50, respectively. The genotype distributions of three SNPs in the MTTP gene were in Hardy-Weinberg equilibrium. The mutated allele of the -164T/C SNP was associated with advanced age, presence of arterial hypertension, elevated levels of iron, ferritin and insulin (p < 0.05). The elevated insulin level was associated with the SNP -164T/C in the three genetic models studied. The mutated allele of the -400A/T SNP was associated with diabetes (p=0.005) and the wild-type allele with siderosis (p=0.030) in co-dominant and dominant genetic models. The mutated allele of H297Q SNP was associated with HCV genotype 3 (co-dominant model), presence of arterial hypertension (codominant and dominant models) and elevated insulin level (dominant model) (p < 0.05). None of the SNPs were associated with alterations in lipid levels and with hepatic steatosis distinct degrees. The presence of mutated allele of the -164T/C SNP combined with HCV genotype 3 infection was associated in the multivariate analysis with steatosis (p=0.004). Also with regard to -164T/C SNP, elevated levels of GGT combined with the presence of the mutated allele were associated with steatosis (p=0.006). The presence of the mutated allele of the -400A/T SNP combined with HCV genotype 3 infection was also associated with steatosis (p=0.032). The analysis of the H297Q SNP combined with the characteristics evaluated was not associated with steatosis (p > 0.05). Conclusions: The presence of all genotypes of the -164T/C, -400A/T and H297Q SNPs in the MTTP gene was detected and was not associated with alterations in lipid levels. Hepatic steatosis in patients with chronic hepatitis C was associated with the -164T/C and -400A/T SNPs combined with HCV genotype 3 infection, and with the -164T/C SNP combined with elevated GGT level. Therefore, in patients with chronic hepatitis C, host and virus characteristics can be investigated in combination
