Introdução: A dermatite atópica (DA) é doença inflamatória cutânea crônica de prevalência elevada e etiopatogenia multifatorial. Defeitos na barreira cutânea e disfunção imune, gerando inflamação, são os fatores de relevância na imunopatogenia desta enfermidade. O tratamento da DA baseia-se no reparo da barreira cutânea e no controle da inflamação; em pacientes com DA refratária à terapêutica tradicional, o metotrexato (MTX) pode ser uma alternativa de tratamento. Entretanto, há poucos relatos que analisam a ação do MTX sobre o perfil de citocinas envolvidas no processo inflamatório da DA. Objetivos: O objetivo geral do estudo foi avaliar o efeito do MTX nas manifestações clínicas e no perfil da expressão cutânea de citocinas e quimiocinas envolvidas no processo inflamatório da DA nos pacientes adultos tratados com metotrexato por 24 semanas. Como objetivos específicos, visamos avaliar nos pacientes supracitados: a evolução dos escores de gravidade EASI (Eczema Area and Severity Index) e SCORAD (SCOring Atopic Dermatitis), do prurido, dos níveis séricos de IgE e da espessura epidérmica média; e o perfil evolutivo da expressão de citocinas inflamatórias, tais como a IL-31, a subunidade do receptor alfa de IL-31 (IL-31RA), o receptor oncostatina M (OSMR), TSLP e Ki67 por imunoistoquímica e de IL-4, IL-6, IL-10, IL-31, IL-31RA, TNF-, IFN-, TARC e CCL-22 por Real Time-PCR. Métodos: Foi realizado estudo prospectivo aberto, com 12 adultos com diagnóstico de DA (segundo critérios de Hanifin & Rajka) moderada a grave. MTX na dose semanal de 15 mg foi prescrito por via oral por período de 24 semanas. Ácido fólico foi indicado na dose de 5 mg/semana 48 a 72 horas após a ingestão do MTX. Foram realizadas visitas nas semanas inicial, 2, 4, 8, 12, 16 e 24 e possíveis efeitos colaterais foram monitorados. Os pacientes foram avaliados na semana inicial, semana 12 e semana 24 quanto aos escores de gravidade EASI e SCORAD, quanto ao prurido (escala analógica visual VAS) e por fotografias clínicas; e na semana inicial e semana 24 do estudo, quanto aos níveis séricos de IgE (nefelometria). Eles foram ainda biopsiados em dois pontos (pele com lesão e pele sem lesão) na semana inicial e após 24 semanas de uso do MTX. Foram selecionados dez controles não-atópicos, pareados por gênero e idade, para fins de padronização das análises laboratoriais. O material obtido foi utilizado para se avaliar a espessura epidérmica média, a marcação por imunoistoquímica de IL-31, IL-31RA, OSMR, TSLP e Ki67 e a expressão de RNAm de IL-4, IL-6, IL-10, IL-31, IL-31RA, TNF-, IFN-, TARC e CCL-22 por Real Time-PCR. Resultados: Após 24 semanas de tratamento com MTX, xxvi observou-se, nos doentes com DA, redução estatisticamente significativa do EASI (p = 0,006), do SCORAD (p < 0,001), do prurido (p = 0,028) e da IgE sérica (p = 0,034). Houve aumento transitório da transaminase glutâmicopirúvica (TGP) sérica (n = 3 / 12), náuseas no dia do uso da medicação (n = 2 / 12) e epigastralgia (n = 1 / 12), sem necessidade de suspensão do MTX nos doentes atópicos. Observou-se também redução significativa da espessura epidérmica média na pele lesada (p = 0,021). Na avaliação imunoistoquímica, comparando-se a pele dos doentes com DA com lesão antes e depois de 24 semanas de MTX, houve aumento da expressão de IL- 31RA na epiderme (p = 0,016). Pela técnica de Real Time-PCR, observou-se diminuição da expressão gênica de IL-31 (p = 0,019) na pele com lesão de DA. Não se observou alteração significativa na expressão cutânea das demais citocinas e quimiocinas inflamatórias analisadas. Conclusão: Este estudo evidencia a eficácia e segurança do metotrexato no tratamento de doentes adultos com DA grave e refratária. Nossos dados evidenciaram redução da expressão gênica de IL-31 e aumento da expressão proteica epidérmica de IL-31RA após 24 semanas de uso do MTX por via oral. Estes achados contribuem para a compreensão dos possíveis mecanismos de ação desta medicação na resposta inflamatória da DA, sobretudo sobre o prurido, sintoma principal e sempre presente nesta dermatose.

Introduction: Atopic dermatitis (AD) is a chronic inflammatory skin disease with a high prevalence and multifactorial etiopathogenesis. Defects in the skin barrier and immune dysfunction, generating inflammation, are key factors to the immunopathogenesis of the disease. The treatment of AD is based on the repair of the skin barrier and control of inflammation; in patients with AD refractory to traditional therapy, methotrexate (MTX) may be an alternative treatment. There are few reports that analyze the action of MTX on the profile of cytokines involved in the inflammatory process of AD. Objectives: The general objective of the study was to evaluate the effect of MTX on the clinical manifestations and on the cutaneous expression profile of cytokines and chemokines involved in the inflammatory response of AD in adult patients treated with methotrexate for 24 weeks. As specific objectives, we aimed to evaluate in the aforementioned patients: the follow-up of EASI (Eczema Area and Severity Index), SCORAD (SCOring Atopic Dermatitis), pruritus, circulating IgE levels, and mean epidermal thickness. We also evaluated the profile of the expression of IL-31, IL-31 alpha receptor subunit (IL-31RA), oncostatin receptor M (OSMR), TSLP and Ki67 by immunohistochemistry and of IL-4, IL-6, IL-10, IL-31, IL-31RA, TNF-, IFN-, TARC, and CCL-22 by Real Time PCR. Methods: An open prospective study was conducted with 12 adult patients with moderate to severe AD (according to Hanifin & Rajka criteria). MTX in weekly dose of 15 mg was administered orally for 24 weeks. Folic acid was indicated at a 5-mg dose once a week 48 to 72 hours after MTX intake. Visits were performed at baseline and at week 2, 4, 8, 12, 16 and 24, and possible side effects were monitored. The patients were evaluated for EASI, SCORAD, pruritus (visual analogue scale VAS), and photographs at initial week, week 12 and week 24, and for serum IgE levels (nephelometry) at initial week and week 24. They were also biopsied in two points (non-lesional and lesional skin). Ten non-atopic controls, matched by gender and age, were selected for standardizing laboratory analysis. Evaluations were performed on pre-treatment and after 24 weeks of use of MTX. The obtained material was used to evaluate the mean epidermal thickness, immunohistochemistry staining of IL-31, IL-31RA, OSMR, TSLP and Ki67, and the expression of IL-4 mRNA, IL-6, IL-10, IL-31, IL-31RA, TNF- , IFN-, TARC, and CCL-22 by real time PCR. Results: After 24 weeks of treatment with MTX, there was a statistically significant reduction of EASI (p = 0.006), SCORAD (p < 0.001), pruritus (p = 0.028), and serum IgE (p = 0.034). There were transient increases in serum glutamic-pyruvic xxviii transaminase (n = 3 / 12), nausea on the day of medication uptake (n = 2 / 12) and epigastric pain (n = 1 / 12), with no need to discontinue MTX in atopic patients. There was also a significant reduction in the mean epidermal thickness of the lesional skin in AD patients (p = 0.021). Immunohistochemistry revealed higher IL-31RA expression in epidermis (p = 0.016) when comparing lesional skin of the AD patients pre-and posttreatment with MTX. There was a decrease on the genic expression of IL-31 (p = 0.019) on the lesional AD skin after 24 weeks of therapy detected by Real time PCR. There were no significant changes in the cutaneous expression of the other evaluated cytokines and chemokines. Conclusion: This study highlights the efficacy and safety of methotrexate in the treatment of patients with severe and refractory AD. Our findings revealed reduction of IL-31 gene expression and an increase in IL-31RA epidermal protein expression after 24 weeks of MTX in AD skin. These data are helpful to clarify the possible mechanisms of action of MTX in the inflammatory response of AD, especially on the control of pruritus, the major and always present symptom of AD.
