INTRODUÇÃO: Nos pacientes com doenças crônicas como a doença renal, nos quais nem sempre é possível a cura, o bem-estar mental e funcional deve sempre figurar entre os principais objetivos no seu tratamento. Entretanto, poucos estudos se dispõem a avaliar o bem-estar de indivíduos submetidos a paratireoidectomia por hiperparatireoidismo secundário, complicação comum da doença renal crônica. O presente estudo tem como objetivo determinar se existe melhora da qualidade de vida nos pacientes submetidos a paratireoidectomia subtotal ou a paratireoidectomia total com autoenxerto imediato, se essa melhora é duradoura e se o tipo de operação influi no padrão de melhora. MÉTODOS: Foram aplicados o questionário SF-36 e a escala numérica da dor em 64 pacientes randomizados submetidos a paratireoidectomia por técnicas diferentes (paratireoidectomia subtotal, paratireoidectomia total com autoenxerto de 45 fragmentos e paratireoidectomia total com autoenxerto de 90 fragmentos) em 3 períodos distintos, a saber: (1) pré-operatório, (2) seis meses e (3) 12 meses após a cirurgia. RESULTADOS: Analisando os períodos pré-operatório e seis e 12 meses da operação, houve melhora na qualidade de vida tanto no componente físico do SF-36 (32,6 no período 1 para 51,8 e 54,4, nos períodos 2 e 3 , respectivamente, p<0,0001) quanto no mental (45,8 para 53,6 e 55,4, p=0,0003). Entre os períodos pósoperatórios não houve diferença significativa. Na avaliação da dor houve melhora significativa quando analisados os períodos antes e após a cirurgia (p<0,0001), não havendo mudança do padrão da melhora nas avaliações de 6 e 12 meses (p=0,71). Não houve diferença entre os tipos de operação em nenhum dos componentes do SF-36 estudados, nem no padrão da dor nos diferentes períodos pré e pós-operatório. CONCLUSÃO: Após a paratireoidectomia em doentes renais crônicos em diálise com hiperparatireoidismo secundário avançado há significativa e duradoura melhora da qualidade de vida, independente da técnica cirúrgica empregada, seja a paratireoidectomia subtotal ou total com autoenxerto imediato

INTRODUCTION: In patients with chronic renal failure and end-stage renal disease, in whom the cure is not always possible, mental and physical well-being should always figure among the main objectives of care. However, there are few studies available that analyze Quality of Life of subjects who underwent parathyroidectomy for secondary hyperparathyroidism, a common consequence of end-stage renal disease. The primary goal of the present study is to determine if there is any difference in Quality of Life of patients submitted to subtotal parathyroidectomy or to total parathyroidectomy with immediate autotransplantation, if the difference lasts and if the type of operation matters. METHODS: The SF-36 Health Survey and the numeric pain scale were applied at 64 randomized patients submitted to parathyroidectomy through different approaches (subtotal parathyroidectomy, total parathyroidectomy with autotransplantation of 45 fragments, total parathyroidectomy with autotransplantation of 90 fragments) at three different moments: (1) preoperatively, (2) 6 months and (3) 12 months after surgery. RESULTS: When analyzed preoperatively and 6 and 12 months after surgery, Quality of Life was enhanced in both physical (32.6 in period 1 to 51.8 and 54.4, in periods 2 and 3, respectively, p<0.0001) and mental (45.8 to 53.6 and 55.4, p=0.0003) components. The difference between both postoperative periods was not significant. There was significant improvement in the pain scores when comparing preoperative and postoperative periods (p<0.0001), however there was no change on this improvement between 6 and 12 months after surgery (p=0.71). There was no difference between the three surgical techniques in neither components of SF-36, nor in the pain scale throughout the whole period studied. CONCLUSION: After parathyroidectomy in dialysis renal patients with advanced secondary hyperparathyroidism there is significant and long lasting improvement of Quality of Life, regardless of the surgical method
