INTRODUÇÃO: A fístula átrio-esofágica é uma das complicações mais temidas da ablação da fibrilação atrial. Sua fisiopatologia ainda não está completamente elucidada, porém, as lesões térmicas esofágicas são consideradas lesões precursoras. O aumento da temperatura intraluminal esofágica está associado ao seu surgimento. Diversas estratégias de prevenção dessas lesões já foram estudadas. A redução da potência de radiofrequência durante aplicações na parede posterior do átrio esquerdo e a monitorização da temperatura luminal esofágica são as estratégias mais usadas; porém, não há consenso quanto ao uso dessas abordagens. MÉTODOS: Pacientes encaminhados para ablação de fibrilação atrial foram randomizados em três grupos com vinte pacientes cada. Grupo 1: sem monitorização da temperatura esofágica e com redução de potência na parede posterior para 20 W. Grupo 2: termômetro unipolar (Braile Biomedical). Grupo 3 com termômetro multipolar (Circa S-Cath). Nos dois últimos grupos, caso a temperatura esofágica atingisse 37,5°C, a potência era reduzida para até 20 W. Em até três dias após a ablação, todos os pacientes foram submetidos a endoscopia digestiva alta e classificados de acordo com a presença de lesões esofágicas em quatro níveis. Grau I: hematoma; Grau II: eritema; Grau III: erosão; Grau IV: úlcera. Os pacientes foram seguidos por meio de contato telefônico e avaliados quanto à recorrência de fibrilação atrial por meio de Holter 24 horas e eletrocardiograma. RESULTADOS: As características basais dos pacientes eram semelhantes. Todos os pacientes foram submetidos à ablação de fibrilação atrial com isolamento das veias pulmonares e realizaram endoscopia. No grupo 3, não houve lesões esofágicas. Cinco pacientes tiveram lesões no grupo 1, sendo uma lesão grau II, duas grau III e duas grau IV. Por fim, seis no grupo 2: duas grau II, três grau III e uma grau IV (P=0,006). No grupo 3, foram atingidas temperaturas maiores do que no grupo 2 (37,9°C x 38,45°C, P=0,018). Não houve diferença entre os grupos no tempo para obter o isolamento das veias pulmonares nem no tempo total de aplicação de radiofrequência (P=0,250; P=0,253). Os pacientes com lesão esofágica foram submetidos a nova endoscopia sete a dez dias após o procedimento, e não foram encontradas lesões esofágicas. Os pacientes foram seguidos por seis meses; em dois pacientes, houve recorrência da fibrilação atrial; porém, sem diferença entre os grupos (P=0,45). CONCLUSÃO: O monitoramento da temperatura esofágica com termômetro multipolar reduziu a incidência de lesões esofágicas quando comparado ao uso de termômetro unipolar ou a não utilização de monitoramento esofágico durante ablação de fibrilação atrial

INTRODUCTION: Atrioesophageal fistula is one of the most feared complications of atrial fibrillation ablation. Its pathophysiology is not yet fully elucidated, but esophageal thermal injuries are considered precursor injuries. The increase in esophageal intraluminal temperature is associated with its appearance. Several prevention strategies for these injuries have already been studied. Reduction of radiofrequency power during applications on the posterior wall of the left atrium and monitoring of esophageal luminal temperature are the most used strategies, however, there is no consensus on the use of these approaches. METHODS: Patients referred for atrial fibrillation ablation were randomized into three groups of twenty patients each. Group 1: without esophageal temperature monitoring and with posterior wall power reduction to 20W. Group 2: unipolar thermometer (Braile Biomedical) and group 3 with multipolar thermometer (Circa S-Cath). In the last two groups, if the esophageal temperature reached 37.5°C, the power was reduced to up to 20W. Within three days after ablation, all patients underwent upper digestive endoscopy and classified according to the presence of esophageal lesions at four levels. Grade I: hematoma, grade II: erythema, grade III: erosion and grade IV: ulcer. Patients were followed up by telephone and assessed for recurrence of atrial fibrillation using 24-hour Holter and electrocardiogram. RESULTS: Baseline patient characteristics were similar. All patients underwent atrial fibrillation ablation with pulmonary vein isolation and underwent endoscopy. In group 3 there were no esophageal lesions. Five patients had injuries in group 1, one being grade II, two grade III and two grade IV, and six in group 2: two grade II, three grade III and one grade IV (P=0.006). In group 3 higher temperatures were reached than in group 2 (37.9°C x 38.45°C, P=0.018). There was no difference, between the groups, in the time to obtain the isolation of the pulmonary veins or in the total time of radiofrequency application (P=0.250; P=0.253). All patients with esophageal injury underwent a new endoscopy seven to ten days after the procedure and no esophageal injuries were found. Patients were followed for six months and there was recurrence of atrial fibrillation in two patients, however, there was no difference between the groups (P=0.45). CONCLUSION: Esophageal luminal temperature monitoring with a multipolar thermometer reduced the incidence of esophageal lesions when compared to using a unipolar thermometer or not using esophageal monitoring during atrial fibrillation ablation
