Este trabalho parte da premissa de que o conceito de saúde norteia as ofertas de cuidado em uma instituição e, como tal, está permeado por ideais de bem-estar que incidem nos modos como são elaborados os dispositivos de atenção à saúde. O mesmo incorre no campo da saúde mental, ou seja, no que concerne ao sofrimento psíquico. A pesquisa analisa a teoria dos discursos de Jacques Lacan no hospital, visando investigar as consequências do discurso capitalista para o estatuto da produção e do tratamento do sofrimento correspondente à própria lógica discursiva, a fim de elucidar os desafios que dizem respeito à ética do sujeito em psicanálise. O que sustenta essa hipótese de pesquisa é a compreensão de que o mal-estar na instituição se autorregula no âmbito das respostas ofertadas, as quais corroboram sua manutenção. Entendemos que a política do discurso capitalista promove tanto um arranjo particular dos modos de funcionamento institucionais quanto subjetivos. A partir daí, extraímos as propriedades do discurso capitalista para fundamentar o que designamos desafios para o trabalho do psicanalista no hospital. Apresentamos essa análise através de quatro eixos: 1) Indivíduo eficaz e gestão: servimo-nos da inversão entre o significante mestre (S1) e o sujeito ($) na passagem do discurso do mestre ao capitalista para situar a mutação do sujeito ao indivíduo e a abolição da divisão subjetiva. 2) Medicalização: articulamos o objeto a à função psiquiatria ao identificarmos o estatuto do objeto no discurso capitalista com o que é produzido pelo saber tecnocientífico. Nesse sentido, a psiquiatria torna-se representante de um campo que promove ofertas de produtos frente às demandas configuradas pelos ideais de saúde. 3) Supereu: analisamos o circuito do supereu na relação com o discurso capitalista, na medida em que essa instância parece propulsionar esse discurso a partir de suas injunções de gozo. 4) Temporalidade lógica: apresentamos a ausência de ruptura no discurso como promotora de uma mutação no tempo: nonstop! a nosso ver, esse eixo impõe desafios para a clínica do sujeito, afinal, quando não há intervalo, há sujeito? Por fim, analisamos a especificidade do discurso do psicanalista frente ao que constitui os seus desafios na vertente clínica e institucional. Concluímos que esse discurso possibilita introduzir um corte no discurso capitalista diante da posição que o psicanalista assume na relação com o outro; posição essa que privilegia incluir o que o discurso capitalista exclui, a saber: a dimensão do real. Restitui-se, então, um lugar para o sujeito, ao mesmo tempo em que identificamos o potencial de contribuição do psicanalista na gestão, dado que a inclusão do real favorece os modos de enfrentamento do que se constitui como problemas institucionais pela lógica gerencialista. Depreende-se, assim, a dimensão da ética da psicanálise como orientadora da escuta e da construção das situações clínicas, denotando uma práxis particular no campo institucional

This work is based on the premise that the concept of health guides the offers of care in an institution and, as such, is permeated by ideals of well-being that affect the ways in which health care devices are elaborated. The same applies to the field of mental health, that is, with regard to psychic suffering. The research analyzesJacques Lacan's theory of discourses in the hospital, aiming to investigate the consequences of the capitalist discourse for the statute of production and treatment of suffering, corresponding to the discursive logic itself, in order to elucidate the challenges regarding the ethics of the subject in psychoanalysis. What supports this research hypothesis is the understanding that the malaise in the institution is self-regulated within the scope of the answers offered, which corroborate its maintenance. We understand that the politics of capitalist discourse promotes both a particular arrangement of institutional and subjective modes of operation. From there, we extract the properties of the capitalist discourse to support what we designate as challenges for the work of the psychoanalyst in the hospital. We present this analysis through four axes: 1) Effective individual and management: we make use of the inversion between the master signifier (S1) and the subject ($) in the passage from the discourse of the master to the discourse of the capitalist to situate the mutation of the subject to the individual and the abolition of subjective division. 2) Medicalization: we articulate the object a to the psychiatry function when we identify the status of the object in the capitalist discourse with what is produced by techno-scientific knowledge. In this sense, psychiatry becomes a representative of a field that promotes product offers in face of the demands shaped by the ideals of health. 3) Superego: we analyze the circuit of the superego in relation to the capitalist discourse, insofar as this instance seems to propel this discourse from its injunctions of jouissance. 4) Logical temporality: we present the absence of rupture in the discourse as a promoter of a mutation in time: nonstop! in our view, this axis poses challenges to the clinics of the subject; after all, is there a subject when there is no interval? Finally, we analyze the specificity of the psychoanalyst's discourse in relation to what constitutes its challenges in the clinical and institutional aspects. We conclude that this discourse makes it possible to introduce a cut in the capitalist discourse in face of the position that the psychoanalyst assumes in the relationship with the other; a position that privileges the inclusion of what the capitalist discourse excludes, namely: the dimension of the real. A place is then given back to the subject, while we identify the potential contribution of the psychoanalyst to management, given that the inclusion of the real favors the ways of facing what is constituted as institutional problems by the managerialist logic. Thus, the dimension of the ethics of psychoanalysis as a guide to listening and the construction of clinical situations can be understood, denoting a particular praxis in the institutional field
