As cadeias de suprimentos de produtos hospitalares no Brasil possuem diversos agentes e seus dados estão registrados em sistemas de gestão empresarial de maneira descentralizada e heterogênea. Esse cenário dificulta a rastreabilidade e a garantia da qualidade dos medicamentos, com risco de comercialização de produtos roubados, adulterados ou falsificados, além de aumentar o custo da saúde e contribuir para a diminuição do nível de serviço prestado pelos hospitais aos pacientes. O objetivo desta pesquisa foi propor uma arquitetura para a rastreabilidade de medicamentos para a cadeia de suprimentos hospitalar utilizando Blockchain. A metodologia utilizada engloba o levantamento de requisitos funcionais e não funcionais na literatura e a avaliação das principais arquiteturas empregadas atualmente em diferentes cadeias de suprimentos. A partir desse levantamento foi concebida e avaliada uma arquitetura para o atendimento dos requisitos e com potencial de: (i) reduzir o risco de aquisição de medicamentos roubados, falsificados ou adulterados em toda a cadeia de suprimentos hospitalar; (ii) reduzir os custos das transações; (iii) aumentar a gestão da qualidade dos produtos; (iv) aumentar a transparência da cadeia; e (v) prover maior agilidade na gestão da cadeia de suprimentos. A arquitetura proposta foi então comparada com o estado da arte da literatura, ressaltando como diferenciais: (i) o atendimento a todos os requisitos identificados, suprindo as lacunas observadas na literatura; e (ii) a consideração de entidades e dados exclusivos das cadeias de suprimentos hospitalares brasileiras.

The supply chains of hospital products in Brazil have several agents and their data are recorded in business management systems in a decentralized and heterogeneous way. This scenario makes it difficult to certify and guarantee the quality of stolen products, with the risk of commercialization of sold, adulterated, or counterfeit products, in addition to increasing the cost of health and also contributing to the reduction of the level of service provided to patients. The objective of this research was to propose an architecture for drug traceability for a hospital supply chain using Blockchain. The methodology used encompasses the survey of a functional and nonfunctional evaluation in the literature of the main architectures currently used in different requirements and supply chains. Based on this survey, an acquisition or potential acquisition of drugs was designed to meet the requirements of (i) reducing the risk of theft of goods throughout the hospital supply chain; (ii) reducing transaction costs; (iii) increasing product quality management; (iv) increase chain transparency; and (v) provide greater agility in supply chain management. The differentiated architecture was then proposed based on all the requirements of the literature, highlighting as state of the art (i) the differentiated service, filling the gaps observed in the literature; and (ii) the consideration of entities and data unique to Brazilian hospital supply chains.
