Em 2018, foi proposta a nova classificação das doenças periodontais e periimplantares pela Academia Americana de Periodontia e Federação Europeia de Periodontologia. Nesta nova classificação, os tipos de margens de restaurações foram considerados como fatores de risco locais para o desenvolvimento de gengivite, perda de inserção e perda óssea, tendo sido considerado que margens subgengivais de restaurações estariam mais correlacionadas à maior inflamação gengival e maior acúmulo de placa. No entanto, observamos resultados conflitantes na literatura. O objetivo primário deste estudo foi avaliar transversalmente as condições de saúde e doença gengival e periodontal ao redor de dentes com diferentes tipos de restaurações ou próteses com términos subgengivais (SBG) em comparação com términos supragengivais.(SPG) e a dentes hígidos, não restaurados (C). Foram incluídos no estudo 50 participantes, homens e mulheres, 18 anos de idade, apresentando restaurações ou reabilitações protéticas envolvendo as faces proximais em pelo menos 1 dente posterior. Todos os participantes responderam ao questionário de saúde e foram examinados periodontalmente de acordo com as medidas de profundidade de sondagem (P.S.), perda de inserção clínica (PIC), índice de sangramento gengival (ISG) e índice de placa (IPl). A perda óssea interproximal foi determinada em radiografias panorâmicas e intra-bucais presentes nos prontuários clínicos dos participantes. Os tipos de restaurações ou coroas protéticas foram determinados de acordo com o tipo de término e tipo de material restaurador. A amostra foi dividida em três grupos após a coleta de dados: SPG (n= 214 sítios), SBG (n= 42 sítios) e C (n= 379 sítios). Os dados foram comparados entre os grupos por meio de Anova para medidas lineares e paramétricas (P.S., PIC, perda óssea) e Kruskal Wallis para medidas não lineares e não paramétricas (ISG, IPl). A associação entre tipos de términos, de materiais restauradores e dos parâmetros clínicos e radiográficos periodontais foi investigada pelos testes de correlação de Pearson e Spearman, para dados paramétricos e não paramétricos, respectivamente. Houve maior prevalência de restaurações de amálgama no grupo SPG e de coroas metalocerâmicas no grupo SBG, com diferenças estatisticamente significantes entre os grupos (p < 0,001; teste Chi-quadrado). O grupo SBG apresentou maior P.S. (2,59 ± 0,98 mm) do que o grupo C (2,19 ± 0,80 mm), porém sem diferenças significantes em relação ao grupo SPG (2,32 ± 1,05 mm). O IPl foi significativamente maior no grupo SBG do que nos grupos SPG e C (p= 0,0003; Kruskal Wallis). Em média, as restaurações subgengivais estavam posicionadas 1,03 ± 1,96 mm subgengivalmente, significativamente diferente do grupo SBG (0,42 ± 1,77 mm). Houve maior prevalência de sítios saudáveis no grupo SPG do que nos grupos SBG e C (p< 0,05; teste Chiquadrado), Houve menor percentual de sítios com perda de inserção 5 mm no GC. Os resultados obtidos sugeriram que restaurações SBG estão associadas com maior acúmulo de placa, correlacionado especialmente com os materiais restauradores amálgama e coroa metalocerâmica. A prevalência de sítios com perda de inserção interproximal é maior em superfícies com margens subgengivais de restaurações. No entanto, restaurações subgengivais não estão associadas com maior inflamação gengival.

In this new classification, submarginal restorative margins were considered as local risk factors to the development of gingivitis, attachment loss and bone loss. It was considered that subgingival margins of restorations would be associated with gingival inflammation and greater plaque accumulation. However, conflicting results are found in literature. The primary objective of this study is to evaluate health and disease conditions around teeth with different types of restorations or prosthesis with subgingival margins (SBG) compared to supragingival (SPG) and unrestored teeth (C). A total of 41 participants 18 years presenting at least 1 interproximal restorations were included. All participants answered the health questionnaire and were examined periodontally according to the measurements of probing depth (DP.), clinical attachment loss (CAL), gingival bleeding index (GBI) and plaque index (PlI). Interproximal bone loss was determined on panoramic and intraoral radiographs present in participants' clinical records. The type of restoration were determined according to the type of marginal fit and restorative material. The sample was divided into three groups after data collection: SPG (n= 214 sites) , SBG (n= 42 sites) and C (n= 379 sites). Data were compared between groups using Anova for linear and parametric measurements (PD, CAL, bone loss) and Kruskal Wallis for nonlinear and non-parametric measurements (GBI, PlI). The association between terminations types, restorative materials and periodontal clinical and radiographic parameters was investigated by Pearson and Spearman correlation tests for parametric and nonparametric data, respectively. It was observed a greater prevalence of amalgam restorations in SPG and metalloceramic crowns at SBG, with significant differences between groups (p< 0.001; Chi-squared test). SBG showed greater PD (2.59 ± 0.98 mm) than C (2.19 ± 0.80 mm), porém sem diferenças significantes em relação ao grupo SPG (2.32 ± 1.05 mm). PlI was significantly greater at SBG than at SPG and C (p = 0.0003; Kruskal Wallis). In average, subgingival restorations were positioned at 1.03 ± 1,96 mm submarginally, significantly different from SBG (0.42 ± 1.77 mm). A greater prevalence of healthy sites were present at SPG than SBG and C (p< 0.05; Chi-squared), There was a lesser percentage of sites with CAL 5 mm at C. The results obtained suggest that SBG restorations are associated to greater plaque accumulation, especially in the presence of amalgam and metalloceramic crowns. The prevalence of sites with interproximal attachment loss is greater at subgingival margins of restorations. However, those restorations are not associated with greater gingival inflammation.
