O objetivo do estudo foi avaliar a concordância entre os diagnósticos clínicos e histopatológicos das lesões do complexo maxilo-facial, no período compreendido entre 1986 a 2016, realizadas no serviço de Estomatologia da Universidade Federal do Rio de Janeiro, Rio de Janeiro, Brasil. Foram analisados 15.047 prontuários, dentre os quais, 4.304 pacientes atendiam aos critérios de inclusão no trabalho. Foram avaliados os critérios relativos à queixa principal, hipótese diagnóstica, diagnóstico final clínico e histopatológico, tendo sido realizados 4.091 biópsias. Os dados foram submetidos a análise estatística através programa SigmaPlot 12.0 (Systat Software Inc.), através do teste Qui Quadrado com um intervalo de confiança de 95%, nível de significância de 5% (p<0,05), e submetidos à técnica de estatística descritiva. A concordância entre os diagnósticos clínico e histopatológico foi de 90,37%. Dentre os 10 grupos considerados no estudo, o que apresentou maior nível de concordância entre os diagnósticos foi o das lesões proliferativas não neoplásicas (34,32%), sendo a hiperplasia fibrosa inflamatória a afecção de maior predominância (70,08%). Quando realizado o comparativo entre os gêneros, houve a prevalência do feminino (60,86%) em relação ao masculino (39,14%). Desta forma, observou-se alta correlação entre o diagnóstico clínico e histopatológico. Entretanto, apesar do alto índice de acerto, continua sendo necessário a execução de ambos os tipos de diagnósticos e a realização de mais estudos sobre a relação entre estes, principalmente nos casos com fatores de risco associados.

The aim of the study was evaluate the agreement among the clinical and histopathological diagnoses of maxilo-facial lesions, during the period of 1986 to 2016, at the Stomatological Clinic of the Federal University of Rio de Janeiro, Rio de Janeiro, Brazil. The study analysed 15.047 medical records. Among them, was considered 4.304 medical records of patients that comply with the criterias of the study. Parameters as the main complaint, diagnostic hypothesis, clinical diagnosis and histopathological diagnosis were standardized for assessment, and have been done 4.091 biopsies. Datas were analysed using SigmaPlot 12.0 (Systat Software Inc.), with chi-square test and the level of significance was 5% (p<0,05), and submited to a descriptive technical. The agreement between the clinical and histopathological was 90,37%. Among the 10 groups analysed, the most common disorder were non-neoplastic proliferative lesions (34,32%), and inflammatory fibrous hyperplasia was the most prevalent lesion into this group (70,08%). When the genders were analysed, females (60,86%) were prevalent in comparison with males (39,14%). Thus, it was observed a high level of agreement between the clinical and histopathological diagnoses. However, despite the rate of concordance, it remains necessary the accomplishment of both procedures of diagnoses and more studies about the relationship between them, mainly in cases with risk factors associated.
