Os estudos em odontologia legal, principalmente os que utilizam análise do desenvolvimento dentário, auxiliam no trabalho pericial, especialmente na determinação da maioridade penal. É grande o número de trabalhos realizados para esta finalidade através da análise de radiografias panorâmicas, por ser este um exame utilizado com maior frequência para a avaliação dentária. No entanto, nota-se um aumento do uso da tomografia computadorizada de feixe cônico (TCFC) na odontologia. Existem vários estudos que apontam a necessidade da criação de bancos de dados que incluam diferentes populações para melhorar a confiança da estimativa de idade com implicações legais. Neste sentido, o presente estudo teve como objetivo determinar a maioridade legal a partir da mineralização dos terceiros molares por meio do método proposto por Demirjian, Goldstein e Tanne (1973) e modificado por Oliveira, F.T., 2010, através da comparação entre a reconstrução panorâmica (RP) e a reformatação multiplanar (RMP) da TCFC. Selecionamos do arquivo de imagens do Departamento de Cirurgia, Estomatologia, Patologia e Radiologia da Faculdade de Odontologia de Bauru da Universidade de São Paulo, FOB-USP, com base em critérios de inclusão e exclusão previamente determinados, duas amostras: Amostra 1 - composta por 20 radiografias panorâmicas (PC) e 20 TCFC, sendo PC e TCFC pertencentes ao mesmo indivíduo e realizadas na mesma data. Amostra 2 Composta por 100 exames de TCFC que continham os dentes 38 e/ou 48 (mandíbula) e 81 exames onde estavam presentes os dentes 18 e/ou 28 (maxila), de indivíduos de 9 a 25 anos de idade. Estes exames foram analisados em sala adequada, com uso de equipamento apropriado, por um profissional Cirurgião Dentista especialista em radiologia odontológica e imaginologia. Os resultados encontrados foram contabilizados e classificados em uma planilha do programa Excel® e analisados com o auxílio de programa estatístico (Statistica versão 12 - StatSoft Inc., Tulsa, USA). As pontuações foram atribuídas cuidadosamente, seguindo os critérios descritos para cada estágio dentário conforme diagrama fornecido por Demirjian, Goldstein e Tanne (1973). A análise da primeira amostra deste estudo foi realizada para avaliar a equivalência das imagens dos dentes 38 e 48 das PC e suas respectivas reformatações panorâmicas (RP) da TCFC. Para testar a associação dos estágios de desenvolvimento dentário entre os dois tipos de imagem, foi utilizado o teste Qui-Quadrado, com a finalidade de conferir se havia discrepância entre os estágios atribuídos aos dentes 38 e 48 na PC e os resultados obtidos na avaliação da RP gerada na TCFC. Para o dente 38 o valor de p=0,065 e p=0,943 para o dente 48, ou seja, houve associação entre as técnicas. Para comparação dos estágios encontrados na análise das imagens da segunda etapa da pesquisa, entre a RP e as RMP obtidas a partir da TCFC dos dentes 18, 28, 38 e 48, foi aplicado o Teste T pareado e os valores geraram tabelas e gráficos utilizados para elaborar as conclusões da pesquisa. Além disso, considerando que não houve resultados que apontaram diferenças estatísticas para os dentes 18 e 28 e para os dentes 38 e 48, foram elaboradas fórmulas para cálculo das idades cronológicas aproximadas a partir do estágio de mineralização destes dentes, quando avaliados através das imagens de RP e RMP geradas da TCFC. O estudo concluiu que existe uma alta probabilidade de maioridade legal quando os terceiros molares estão no estágio 8 de mineralização, conforme diagrama fornecido por Demirjian, Goldstein e Tanne (1973). Também, devido ao fato de que não foram encontradas diferenças estatisticamente significativas na população estudada ao se comparar os estágios de mineralização dos dentes terceiros molares (18, 28, 38 e 48) na PC e na RP (amostra 1) e não foram encontradas diferenças significativas ao se comparar os estágios de mineralização dos dentes terceiros molares (18, 28, 38 e 48) na RP e nas RMP da TCFC (amostra 2), os exames se equivalem para determinação do grau de mineralização dos terceiros molares em indivíduos com idade cronológica entre 9 e 25 anos.

Studies in legal dentistry, especially that use dental development analysis, help in expert work, especially in legal majority determination. There is a large number of studies carried out for this purpose through the analysis of panoramic radiographs, as this exam is most frequently used for dental evaluation. However, there is an increase in the use of cone beam computed tomography (CBCT) in dentistry. There are several studies pointing to the need to create databases that include different populations to improve the confidence of age estimation with legal implications. In this sense, the present study aimed to determinate the legal age of majority from the mineralization of the third molar, using the method proposed by Demirjian, Goldstein and Tanne (1973) and modified by Oliveira F.T., 2010, through the comparison between the panoramic reconstruction (RP) and the CBTC multiplanar reformatting (RMP). Two samples were selected from the image databased of the Department of Surgery, Stomatology, Pathology and Radiology of the University of São Paulo Bauru School of Dentistry, FOB-USP, based on previously determined inclusion and exclusion criteria: Sample 1 consisting of 20 panoramic radiographs (PC) and 20 CBCT, with PC and CBTC belonging to the same individual and performed on the same date. Sample 2 Comprised of 100 CBTC that contained teeth 38 and / or 48 (mandible) and 81 exams where teeth 18 and / or 28 (maxilla) were present, from individuals aged 9 to 25 years old. These examinations were analyzed in an appropriate room, using appropriate equipment, by a professional Dental Surgeon specialized in dental radiology and imaging. The results were counted and classified in an Excel® spreadsheet and analyzed with the aid of statistical software (Statistica version 12 - StatSoft Inc., Tulsa, USA). The scores were carefully assigned following the criteria described by each dental stage as per the diagram provided by Demirjian, Goldstein and Tanne (1973). The analysis of the first sample of this research was performed to assess the equivalence of the teeth 38 and 48 PC images end their respective panoramic reformations (RP) of CBCT. To test the association of the dental development stages between the two types of image, the Chi-Square test was used, in order to check whether there was a discrepancy between the stages attributed to teeth 38 and 48 in PC and the results obtained in the evaluation of RP generated in the CBCT. For tooth 38 the value of p = 0.065 and p = 0.943 for tooth 48, that is, there was an association between the techniques. To compare the stages found in the analysis of the images of the second stage of the research, between the RP and the RMP obtained from the CBCT of teeth 18, 28, 38 and 48, the paired T test was applied and the values generated tables and graphs used to elaborate the research conclusions. Furthermore, considering that there were no results that showed statistical differences for teeth 18 and 28 and for teeth 38 and 48, formulas were developed to calculate the approximate chronological ages from the mineralization stage of these teeth, when evaluated through images of RP and RMP generated from the TCFC. The study concluded that there is a high probability of legal adulthood when the third molars are in stage 8 of mineralization, according to the diagram provided by Demirjian, Goldstein and Tanne (1973). Also, due to the fact that no statistically significant differences were found in the studied population, when comparing the stages of mineralization of third molar teeth (18, 28, 38 and 48) in PC and RP (sample 1) and no significant differences were found when comparing the stages of mineralization of the third molar teeth (18, 28, 38 and 48) in the RP and in the RMP of the CBCT (sample 2), the exams are equivalent to determine the degree of mineralization of the third molars in individuals aged between 9 and 25 years.
