O conhecimento da anatomia interna e externa dos dentes é de fundamental importância para a prática clínica endodôntica. Dentes como os pré molares inferiores (PMI) que apresentam variações em sua anatomia necessitam de exames de imagem de qualidade para que suas estruturas e possíveis variações sejam observadas, prévias ao tratamento. Portanto o objetivo desse trabalho foi comparar a acurácia das imagens de pré molares inferiores na identificação de foraminas acessórias, realizadas por Tomografia Computadorizada de Feixe Cônico (TCFC), bem como o uso de um software para redução de artefato, com as imagens geradas pela Microtomografia Computadorizada (micro-CT), considerada padrão ouro. Foram selecionados 50 dentes PMI, estes foram submetidos a dois equipamentos de tomografia, microtomografia (micro-CT) e tomografia computadorizada de feixe cônico (TCFC). Em cada equipamento foram adquiridas uma imagem de cada dente, e essas imagens compõem o n dessa pesquisa, portanto o n=100 imagens. Através do teste de Bland-Altman obteve-se o resultado da média da diferença (viés), entre os métodos avaliados com o padrão ouro, sendo o viés da TCFC Sirona = 1,24 e do filtro e-Vol DX = 1,48, sendo significativamente diferentes de zero, isso demonstra que nenhum método obteve concordância com o padrão ouro. A acurácia da TCFC foi de 36,5% e do e-Vol DX 30,4%, demostrando que o uso do filtro nesse caso não apresentou melhora. Por fim, através do teste de Friedman, foi possível avaliar qualitativamente os métodos entre si, não havendo diferença significativa entre eles. Concluindo assim, que nenhum método utilizado apresentou resultados semelhantes de acurácia com o micro-CT.

Accuracy of cone beam computed tomography and the impact on endodontic treatment in mandibular premolar teeth

Knowledge of the internal and external anatomy of teeth is of fundamental importance for endodontic clinical practice. Teeth such as mandibular premolars that present variations in their anatomy require quality imaging exams so that their structures and possible variations can be observed, prior to treatment. Therefore, the objective of this work was to compare the accuracy of images of mandibular premolars in identifying accessory foramina, performed by Cone Beam Computed Tomography (CBCT), as well as the use of software to reduce artifact, with the images generated by Microtomography Computerized (micro-CT), considered the gold standard. Fifty mandibular premolars teeth were selected and subjected to two tomography equipment, microtomography (micro-CT) and cone beam computed tomography (CBCT). In each piece of equipment, an image of each tooth was acquired, and these images make up the n of this research, therefore the n=100 images. Using the Bland-Altman test, the result of the mean difference (bias) was obtained between the methods evaluated with the gold standard, with the bias of the CBCT Sirona = 1.24 and the e-Vol DX filter = 1.48, being significantly different from zero, this demonstrates that no method achieved agreement with the gold standard. The accuracy of CBCT was 36.5% and of e-Vol DX 30.4%, demonstrating that the use of the filter in this case did not improve. Finally, using the Friedman test, it was possible to qualitatively evaluate the methods among themselves, with no significant difference between them. In conclusion, no method used presented similar accuracy results with micro-CT.
