A mucosa ceratinizada (MC) desempenha um papel importante na homeostasia do periodonto marginal e também na estética tecidual. Uma altura mínima de 2 milímetros é necessária para manutenção da saúde periodontal. O procedimento mais comum quando há quantidades inadequadas de MC é o enxerto gengival livre (EGL). Contudo essa técnica tem desvantagem estética, pois o enxerto herda características do palato que resultam na expressão genotípica do tecido conjuntivo, conferindo à área receptora uma coloração pálida, diferente da mucosa adjacente, podendo comprometer o resultado onde a estética é preponderante. Portanto, considerou-se a possibilidade de posicionar o EGL de forma invertida, com o lado mais próximo da lâmina basal em contato com o periósteo da área receptora, procurando neutralizar o efeito da interação epitélio-conjuntivo sobre a coloração do enxerto, permitindo aos ceratinócitos migrarem das margens do leito receptor, mantendo as características da área receptora. O objetivo deste trabalho foi realizar um estudo clínico comparativo entre EGL, enxerto gengival livre de tecido conjuntivo (EGLC) e enxerto gengival livre de tecido conjuntivo invertido (EGLCI), em boca dividida, analisando seu resultado estético e o grau de contração. Foram avaliados 9 pacientes, que inicialmente apresentaram faixa inadequada de MC na região de prémolares inferiores, bilateralmente, e receberam EGL (grupo controle 1 GC1), EGLC (grupo controle 2 GC2) e EGLCI (grupo experimental GE). Foram realizadas medições dos enxertos do primeiro ao sexto mês e fotografias no último mês. As fotografias foram analisadas de três maneiras: por três periodontistas experientes usando o Pink Esthetic Score (PES), sob o ponto de vista dos pacientes e de forma objetiva pelo programa ImageJ, comparando coloração e textura dos enxertos com a gengiva adjacente. Para análise estatística de contração foi utilizado o teste ANOVA de medidas repetidas, para comparar os scores do PES foi utilizado o teste de Friedman e para comparar a opinião dos pacientes foi utilizado o teste Qui-quadrado. Como resultado, os grupos GC2 e GE não mostraram diferença estatística significante entre si para contração, mas apresentaram maior grau de contração em relação ao GC1, o qual não apresentou contração estatisticamente significante ao longo do tempo. Os resultados obtidos considerando o PES indicaram que o GC1 foi estatisticamente diferente dos outros dois grupos, sendo que os escores desses dois grupos (GC2 e GE) mostraram maior semelhança com a MC adjacente tanto em coloração quanto em textura. Quanto aos resultados da análise pelo ImageJ, os grupos GC2 e GE foram estatisticamente diferentes do GC1, e mais parecidos com a coloração da gengiva adjacente, sem diferença estatisticamente significante entre GC2 e GE. Na análise dos pacientes o enxerto preferido foi o GC2, em seguida GE, sendo GC1 o mais rejeitado. Assim, podemos concluir que o GC1 apresentou maior diferença estética em todas as formas de análise, enquanto GC2 e GE não apresentaram diferenças entre si. Os grupos GC2 e GE contraíram expressivamente em relação ao GC1. Não foi possível afirmar que a inversão do enxerto conjuntivo livre tenha trazido alguma vantagem estética.

The keratinized mucosa (KM) plays an important role in the homeostasis of the marginal periodontium and also in tissue aesthetics. A minimum height of 2 millimeters is required to maintain periodontal health. The most common procedure when there are inadequate amounts of KM is the free gingival graft (FGG). However, this technique has an aesthetic disadvantage, since the graft inherits characteristics from the palate, resulting in the genotypic expression of the connective tissue and giving the recipient area a pale coloration, different from the adjacent mucosa, which may compromise the result where the aesthetics are predominant. Therefore, the possibility of placing an inverted de-epithelized FGG was considered, keeping the side close to the basal lamina in contact with the periosteum of the recipient area. This approach was conceived as a way to neutralize the effect of the epithelial-conjunctive interaction on the graft color by allowing the keratinocytes to migrate from the margins of the receiving bed and keeping the characteristics of the receiving area. The aim of this study was to conduct a comparative clinical study between FGG, connective tissue free gingival graft (CTFGG) and inverted connective tissue free gingival graft (ICTFGG) in a split mouth design, analyzing the aesthetic result and the degree of contraction of the grafts. Nine patients with bilateral inadequate KM width in the region of the lower premolars were selected and received FGG (control group 1 - CG1), CTFGG (control group 2 - CG2) and ICTFGG (experimental group - EG). Graft measurements were performed from the baseline up to a follow-up of six months and photographs were taken in the last month. The photographs were analyzed in three ways: by three experienced periodontists using the pink esthetic score, from the point of view of the patients and, objectively, using the ImageJ software to compare the color and texture of the grafts with the adjacent gingiva. For statistical analysis of contraction, the ANOVA test of repeated measures was used, to compare the scores of the pink aesthetic index, the Friedman test was used and to compare the opinion of the patients, the Chi-square test was used. As a result, the CG2 and EG groups did not show a statistically significant difference between themselves for contraction, but showed a greater degree of contraction compared to CG1, which did not present a statistically significant contraction over time. The results related to the pink esthetic score indicated that CG1 was statistically different from the other two groups, and the scores of these two groups (CG2 and EG) showed greater similarity with the adjacent KM for both color and texture. For the results of the analysis by ImageJ, the groups CG2 and EG were statistically different from CG1, and more similar to the coloring of the adjacent gingiva. No statistically significant difference between CG2 and EG. In the analysis of patients, the preferred graft was CG2, then EG, with CG1 being the most rejected. Thus, we can conclude that CG1 showed the greatest aesthetic difference in all forms of analysis, while CG2 and EG did not present differences between them. Groups CG2 and EG contracted significantly compared to CG1. It was not possible to affirm that the inversion of the free conjunctive graft has brought any aesthetic advantage.
