A longitudinal study of the effect of the surgical protocol on the dimensions of the dental arches of children with cleft lip and palate: 3D-3D superimposition technique

Stereophotogrammetry
Abstract in English
This study aimed to compare the dimensional changes of the dental arches in children with unilateral cleft lip and palate before and after two surgical rehabilitation protocols using 3D-3D superimposition technique. The sample consisted of 60 digitized models divided into the following groups: Group 1 (G1) - cheiloplasty (Millard technique) at 3 months and one-stage palatoplasty (von Langenbeck technique) at 12 months; and Group 2 (G2) - cheiloplasty (Millard's technique) and palatoplasty in two stages: closure of the anterior palate (Hans Pichler's technique) at 3 months and closure of the posterior palate (Sommerlad's technique) at 12 months. The models were digitized using a 3D scanner and the 3D-3D superimposition was performed using stereophotogrammetry system software. The superipositions were performed on the models of the same patients in phases T2 (pre palatoplasty) over T1 (pre cheiloplasty), T3 (post palatoplasty) over T2 (pre palatoplasty); T3 (post palatoplasty) over T1 (pre cheiloplasty) and T4 (at 5 years old) over T3 (post palatoplasty). The differences were evaluated by Root Mean Square (RMS) and expressed in millimeters (mm). Friedman test followed by post-hoc Bonferroni test and Mann-Whitney test were assessed to compare groups (α=5%). RMS was 2.04 mm (±0.45) in G1 and 1.75 mm (±0.36) in G2 in the overlap of T3 over T1 steps, with a statistically significant difference (p=0.012). In the other analyzed periods, there were no statistically significant differences. According to the results of this study, it is concluded that different surgical rehabilitation protocols result in similar growth potentials in children with complete cleft lip and palate. However, further studies are needed to quantify the morphological changes of different rehabilitation protocols in children with cleft lip and palate.
