Estudos recentes têm sugerido que a gengivite e as lesões de manchas brancas durante o tratamento ortodôntico, ocorrem mais frequentemente em indivíduos obesos do que com peso normal, por esta razão o objetivo deste estudo foi avaliar a incidência de lesões de manchas brancas e gengivite, após a realização do tratamento ortodôntico. Além disso, foi investigada a cooperação do paciente durante o tratamento ortodôntico. A amostra foi constituída por 85 prontuários de pacientes atendidos em uma clínica escola, sendo distribuídos em dois grupos de acordo com o IMC no início do tratamento: GPN- pacientes com peso normal e GPO- pacientes com sobrepeso/obesidade. Foram utilizadas fotografias digitais pré e pós tratamento, para avaliar a presença de lesões de manchas brancas, gengivite e dados dos prontuários. Mann Whitey foi adotado para comparar as medidas ordinais entre os grupos e o teste de Wilcoxon na comparação entre os períodos, pré e pós-tratamento (p<0,05). Houve maior incidência de manchas brancas após o tratamento ortodôntico em todos os pacientes, não havendo diferenças significativas entre os grupos, exceto para o dente 11. O grupo de pacientes obesos apresentou maior incidência de gengivite do que o grupo de eutróficos e houve piora significativa da severidade da doença periodontal entre os dois períodos, para ambos os grupos. A colaboração do paciente durante o tratamento não apresentou diferenças significativas, entre os grupos avaliados (p<0,05). Pode-se concluir que ocorre aumento na incidência de manchas brancas de pacientes submetidos ao tratamento ortodôntico, independentemente de serem obesos ou eutróficos. Em indivíduos obesos ocorre aumento na severidade da gengivite após o tratamento ortodôntico.

Recent studies have suggested that gingivitis and white spot lesions during orthodontic treatment are more frequent in obese individuals than in the eutrophic ones, Therefore, this study aimed to evaluate the incidence of white spot lesions and gingivitis after the orthodontic treatment. In addition, patient cooperation during orthodontic treatment was also investigated. The sample consisted in 85 medical records of patients treated at a school clinic, which were divided into two groups according to the BMI at the treatment beginning: GPN- normal weight patients and GPO- overweight / obese patients. Pretreatment and posttreatment digital photographs were used to evaluate the presence of white spot lesions and gingivitis, as well as, data from the medical records. For ordinal measurements comparisons between the groups the Mann Whitey test was used and for the pretreatment and posttreatment comparisons the Wilcoxon test was performed (p <0.05). The results showed a higher incidence of white spots after the orthodontic treatment in all patients, however, with no significant differences between groups, except for tooth 11. The obese patients group presented a higher incidence of gingivitis than the eutrophic group and there was a significant increase of the periodontal disease severity at the posttreatment period for both groups. No significant differences were found between the groups when the patient collaboration during treatment was evaluated. It was concluded that there was an increased in the incidence of white spots in patients undergoing orthodontic treatment, regardless of whether they were obese or eutrophic. After the orthodontic treatment, the gingivitis severity can increase in obese individuals.
