Introduction: The aim of this study was to compare the skeletal and dentoalveolar stability produced by the Twin Block (TB) appliances through cephalometric measures. Methods: The treatment group sample comprised 11 patients with Class II malocclusion, previously treated with TB, evaluated at three stages: pretreatment, posttreatment, and long-term posttreatment. The control group comprised 11 subjects with normal occlusion. Lateral cephalometric films were obtained at the three stages. Were measured a customized cephalometric analysis generated 17 variables, 7 angular and 10 linear. Intragroup comparisons among the 3 stages were performed with repeated measures analysis of variance ANOVA, followed by Tukey tests. Intergroup comparisons of posttreatment changes and normal growth changes of the treatment group were performed with T-tests (P<0,05) Results: Lower anterior facial height, the angle formed by the maxillary incisors long axis and the NA line, and Overbite demonstrated significant relapses in relation to the control group. The same intragroup difference was found in the TB group. Conclusion: Most dentoalveolar changes obtained with the Twin Block followed by fixed appliances during treatment remained stable in the long term. However, lower anterior facial height, the angle formed by the maxillary incisors long axis and the NA line, and Overbite, demonstrated significant relapses in relation to the control group. Therefore, periodic control should be increased in the posttreatment period to prevent relapse possibilities.

Introdução: O objetivo deste estudo foi avaliar a estabilidade após 10 anos da correção da má oclusão de Classe II, subdivisão 1 em pacientes que foram tratados com o aparelho ortopédico funcional Twin Block seguido pela ortodontia corretiva. Métodos: A amostra do grupo de tratamento foi composta por 11 pacientes com má oclusão de Classe II, previamente tratados com TB, avaliados em três etapas: pré-tratamento, pós-tratamento e pós-tratamento de longo prazo. O grupo controle foi composto por 11 indivíduos com oclusão normal. As radiografias cefalométricas laterais foram obtidas nas três etapas. Foram medidas uma análise cefalométrica personalizada gerada 17 variáveis, 7 angulares e 10 lineares. As comparações intragrupo entre os 3 estágios foram realizadas com análise de medidas repetidas de variância ANOVA, seguida de testes de Tukey. Comparações entre grupos de alterações pós-tratamento e alterações de crescimento normal do grupo de tratamento foram realizadas com testes T (P <0,05) Resultados: O grupo TB apresentou maiores mudanças na comparação intergrupos, principalmente na altura facial anteroinferior, angulação dos incisivos superiores entre os pontos NA e o Overbite. Conclusão: A maioria das alterações dentoalveolares obtidas com o Twin Block seguido de aparelhos fixos durante o tratamento permaneceram estáveis a longo prazo. No entanto, a altura facial anterior inferior, o ângulo formado pelo longo eixo do incisivo superior e a linha NA e o Overbite demonstraram recidivas significativas em relação ao grupo controle. Portanto, o controle periódico deve ser aumentado no período pós-tratamento para evitar possibilidades de recidiva.
