Esta revisão sistemática investigou a associação entre o alto peso de nascimento e a presença de fatores de risco para doença cardiovascular na vida adulta. Tendo como principal objetivo o de avaliar a presença de fatores de risco para doença cardiovascular como sobrepeso ou obesidade, hipertensão arterial, dislipidemias e/ou diabetes mellitus, na vida adulta de indivíduos nascidos macrossômicos e não macrossômicos. Foram incluídos estudos observacionais transversais, caso-controle, coortes retrospectivos e prospectivos, que respondessem a todos os pontos da pergunta PECO, no caso do desfecho, quando o artigo apresentava ao menos um desfecho, foi incluído. As buscas foram realizadas nas bases de dados PubMed, Embase, Web of Science, Scopus, Biblioteca Virtual em Saúde (BVS) e Livivo, além de buscas manuais na literatura cinzenta. Não houve restrição de língua nem de ano de publicação nestas buscas. A busca nas bases de dados foi finalizada no dia 23/03/2022. A análise do risco de viés dos estudos primários incluídos foi o proposto pelo Instituto Joanna Briggs, adaptado para estudos observacionais. Uma síntese narrativa foi realizada estruturada com base nas populações gerais estudadas, assim como a síntese quantitativa com base nos dados da metanálise, sendo diferença estatística considerada significativa, quando P < 0,05. Foram incluídos 15 estudos primários que apresentavam dados como idade, sexo, glicemia, pressão sistólica e diastólica, colesterol total, colesterol HDL e LDL, triglicérides, peso, altura, índice de massa corporal e circunferência da cintura. Além disso, os participantes desses estudos foram divididos pelo seu peso de nascimento, ou seja, indivíduos macrossômicos e com peso de nascimento normal. O estudo incluiu um total de 90.899 participantes, sendo 83.601 do grupo controle e 7.298 do grupo experimental. Os resultados obtidos na metanálise mostraram que não houve diferença estatística entre todos os desfechos analisados, apenas em pressão diastólica, IMC, altura, peso, circunferência da cintura e colesterol HDL, sendo apenas o último favorável ao grupo controle, resultados que vão de encontro aos obtidos na literatura. Mais pesquisas sobre os efeitos do alto peso ao nascer precisam ser desenvolvidas para que se obtenha maior grau de certeza com os possíveis efeitos gerados na vida adulta, uma vez que há dados limitados e controversos na literatura. This systematic review investigates the association between high birth weight and the presence of risk factors for cardiovascular disease in adult life. Having as main objective to evaluate the presence of risk factors for cardiovascular disease such as overweight or obesity, hypertension, dyslipidemia and/or diabetes mellitus, in adult life of individuals born macrosomic and non-macrosomic. Cross-sectional observational, case-control, retrospective and prospective cohort studies were included, who answered all the points of the PECO question, in the case of the outcome, when the article presented at least one outcome, it was included. Searches were performed in the PubMed, Embase, Web of Science, Scopus, Virtual Health Library and Livivo databases, in addition to manual searches in the gray literature. There were no restrictions on language or year of publication in these searches. The search in databases was completed in 03/23/2022. The risk of bias analysis of the primary studies included was that proposed by the Joanna Briggs Institute, adapted for observational studies. A structured narrative synthesis were performed based in the general populations studied, as well as a quantitative synthesis based on the meta-analyses data. With a statistical difference considered significant when P < 0,05. We included 15 primary studies that presented data such as age, gender, blood glucose, systolic and diastolic blood pressure, total cholesterol, HDL and LDL cholesterol, triglycerides, weight, height, body mass index and waist circumference. In addition, participants in these studies were divided by their birth weight, that is, macrosomic individuals and normal birth weight. The study included a total of 90.899 participants, 83.601 from the control group and 7.298 from the experimental group. The results obtained in the metaanalyses showed that there was no statistical difference between all the analyses outcomes, only in diastolic blood pressure, BMI, height, weight, waist circumference and HDL cholesterol, with only the latter being favorable to the control group. More research on the effects of high birth weight needs to be developed in order to obtain a greater degree of certainty with the possible effects generated in adult life, since there are limited and controversial data in the literature.
