A curva logoaudiométrica é a avaliação mais completa para se obter o Índice de Reconhecimento da Fala (IRF). Contudo, essa prática foi abandonada em função do tempo dispendido na aplicação das listas de palavras comumente utilizadas na rotina clínica. Ao se utilizar a intensidade fixa da fala para a pesquisa do IRF, o desempenho do indivíduo poderá não ser o real. Objetivo: Analisar a aplicabilidade de listas de palavras monossílabas na pesquisa da curva logoaudiométrica em idosos com perda auditiva sensorioneural e verificar a influência da configuração audiométrica, do grau da perda auditiva, da idade e do gênero nos testes de detecção e reconhecimento da fala. Material e Métodos: Etapa 1 - Foram elaboradas 10 listas contendo 10 monossílabos em cada e com distribuição de frequência dos fonemas semelhante às listas originais com 25 monossílabos. Estas foram aplicadas em 45 indivíduos, 89 orelhas - 51 do gênero masculino e 38 do gênero feminino, idade entre 62 e 89 anos, diagnóstico de perda auditiva sensorioneural. O teste Análise de Variância (ANOVA) para medidas repetidas constatou diferença entre as listas de monossílabos, e o teste de Tukey verificou as diferenças entre as médias dos índices de acerto obtidos com as 10 listas. A partir dessa análise foram mantidas 6 listas para utilização na etapa 2. Etapa 2 Participaram 87 indivíduos, 174 orelhas - 90 do gênero masculino e 84 do gênero feminino, idade entre 60 e 90 anos, diagnóstico de perda auditiva sensorioneural. Foi realizada a pesquisa da curva logoaudiométrica obtendo-se o Limiar de Detecção da Fala (LDF), o Limiar de Reconhecimento da Fala (LRF), e o IRF variando-se a intensidade de apresentação da fala. A estatística contou com os testes: ANOVA, Tukey e Fisher, Coeficiente de Correlação de Pearson e Qui-Quadrado. Resultados: A configuração audiométrica, grau da perda auditiva e idade influenciaram o LDF. A configuração audiométrica, grau da perda auditiva, idade e gênero influenciaram o índice de reconhecimento de fala, sendo que a configuração plana, grau leve, idosos mais novos e gênero feminino apresentaram os melhores índices de acerto. A configuração audiométrica, grau da perda auditiva e gênero influenciaram o nível de sensação que gerou o Índice de Reconhecimento Máximo de Fala (IR-MAX), sendo que a configuração abrupta e o grau leve precisaram de maior intensidade para obter o IR-Max. Conclusão: Pôde-se verificar que a aplicação das listas de monossílabos utilizadas nesse estudo na pesquisa da curva logoaudiométrica, é viável, já que os achados relativos ao desempenho dos indivíduos no reconhecimento da fala, ao se considerar as variáveis configuração audiométrica, grau da perda auditiva, idade e gênero, foram compatíveis com os reportados em estudos prévios; e reproduziu o que é observado na rotina clínica, ou seja, houve relação entre o desempenho no reconhecimento da fala e todas as variáveis estudadas, bem como, entre o nível de sensação (NS) que gerou o IR-MAX e a configuração audiométrica, grau da perda auditiva e gênero. Não houve influência da idade para o NS que gerou o IR-MAX, e do gênero para a obtenção do LDF.

The performance/intensity function is the most complete assessment to obtain the Speech Recognition Score. However, this practice was abandoned due to the time spent in the application of the lists of words commonly used in clinical routine. When using fixed speech intensity for SRI research, the individual's performance may not be real. Objective: To analyze the applicability of monosyllable lists of words in the investigation of logoaudiometric curve in elderly persons with sensorineural hearing loss and to verify the influence of audiometric configuration, degree of hearing loss, age and gender in speech detection and recognition tests. Methods: Stage 1 - 10 lists were elaborated containing 10 monosyllables in each and with distribution of phoneme frequency similar to the original lists with 25 monosyllables. These were applied to 45 individuals, 89 ears - 51 males and 38 females, aged 62 and 89 years, diagnosed with sensorineural hearing loss. The Analysis of Variance test (ANOVA) for repeated measures showed a difference between the monosyllable lists, and the Tukeys test verified the differences between the means of correctness indexes obtained with the 10 lists. From this analysis, 6 lists were kept for using in stage 2. Stage 2 - 87 individuals participated, 174 ears - 90 male and 84 female, aged 60 and 90 years, diagnosis of sensorineural hearing loss. The investigation of logoaudiometric curve was conducted, obtaining the Speech Detection Threshold (SDT), the Speech Recognition Threshold (SRT), and the Speech Recognition Score by varying the intensity of speech presentation. The statistics included the following tests: ANOVA, Tukey and Fisher, Pearson's Correlation Coefficient and Chi-Square. Results: The audiometric configuration, degree of hearing loss and age influenced the SDT. The audiometric configuration, degree of hearing loss, age and gender influenced the speech recognition index, being the flat configuration, mild degree, younger elderly persons and female gender showed better correctness indexes. The audiometric configuration, degree of hearing loss and gender influenced the sensation level that generated the Maximum Speech Recognition Score (PB-MAX), being the abrupt configuration and mild degree required higher intensity to obtain the PB-MAX. Conclusion: It was possible to verify that the application of monosyllable lists used in this study in the investigation of logoaudiometric curve is feasible, since the findings regarding the individuals performance in speech recognition, when considering the variables audiometric configuration, degree of hearing loss, age and gender, were compatible with those reported in previous studies; and reproduced what is observed in clinical routine, that is, there was a relationship between performance in speech recognition and all the variables studied, as well as, between the sensation level (SL) that generated the PB-MAX and the audiometric configuration, degree of hearing loss and gender. There was no influence of age for SL that generated PB-MAX, and gender for obtaining SDT.
