A reparação dos tecidos periapicais depende da obturação total do sistema de canais radiculares e seu selamento hermético por meio de materiais compatíveis física e biologicamente, constituindo objetivo primordial do tratamento endodôntico. Os problemas endodônticos de difícil resolução através da terapêutica convencional podem ser solucionados utilizando-se o recurso da cirurgia paraendodôntica. Para a avaliação da capacidade de selamento apical de quatro técnicas de vedamento retrógrado, empregando-se dois materiais retrobturadores diferentes, utilizaram-se noventa dentes caninos superiores humanos, que tiveram seus canais instrumentados e obturados. Após a ressecção da porção apical radicular em ângulo de aproximadamente noventa graus em relação ao longo eixo da raiz e impermeabilização da superfície dentária externa, as raízes foram divididas mesclando seus comprimentos em nove grupos. As técnicas utilizadas foram as seguintes: obturação retrógrada (grupos I e II), retroinstrumentação com retrobturação (grupos III e IV), retroinstrumentação com retrobturação associada à obturação retrógrada (grupos V e VI) , canalização (grupo VII e VIII) e apicectomia (grupo IX-controle). Cada técnica utilizou os materiais Super-EBA e o MTA. Após a realização dos procedimentos operatórios, as raízes foram imersas em rhodamine B a 0,2%, por 72 horas, a 37 graus celsius. Decorrido esse período foram lavadas e a camada impermeabilizante removida. Procedeu-se, então, ao desgaste longitudinal da raiz até a exposição do material retrobturador, possibilitando a análise de extensão da infiltração da rhodamine B. Essa avaliação foi realizada com a técnica micrométrica de superfície com auxílio de um microscópio óptico. Os dados originais e as médias das infiltrações, medidas em milímetros, foram devidamente tabulados e submetidos a análise estatística, levando-nos às seguintes conclusões: nas técnicas onde o material retrobturador Super-EBA foi utilizado não ocorreu diferença estatística significante entre elas; quando o MTA foi utilizado como material retrobturador, houve diferença estatística significante entre as técnicas de obturação retrógrada e canalização (p< 0,05) e entre as técnicas de retroinstrumentação com retrobturação associada à obturação retrógrada e canalização (p< 0,05); não houve diferença estatística significante entre os materiais retrobturadores Super-EBA e MTA empregados nas diferentes técnicas cirúrgicas estudadas; quanto à interação entre as técnicas e materiais empregados, partindo-se do grupo experimental com melhor selamento marginal para o pior, a ordenação foi a seguinte: grupo VIIItécnica da canalização com MTA; grupo VII- técnica da canalização com Super-EBA; grupo I - obturação retrógrada com Super-EBA; grupo IX - grupo controle (apicectomia); grupo IV - técnica da retroinstrumentação com retrobturação com MTA; grupo III - técnica da retroinstrumentação com retrobturação com Super-EBA; grupo V - técnica da retroinstrumentação com retrobturação associada à obturação retrógrada com Super-EBA; grupo II - obturação retrógrada com MTA; grupo VI - técnica da retroinstrumentação com retrobturação associada à obturação retrógrada com MTA.

In vitro evaluation of the sealing ability of the Super-EBA and MTA in four different types of retrograde filling.

The periapical tissue repair depends on the complete filling of the root canal system and its hermetic sealing by means of physically and biologically compatible materials, which is the primary goal of the endodontic treatment. Endodontic complications that are difficult to solve through conventional therapy may be treated means of endodontic surgery. The apical sealing ability of four techniques of retrograde filling was evaluated with employment of two different retrograde filling materials. For that purpose, a total of ninety extracted human maxillary canines were submitted to root canal instrumentation and filling. After resection of the apical portion of the root in an approximately 90- degree angle in relation to the tooth long axis and impermeabilization of the external surface of the tooth, the roots were divided in nine groups, with blending of their lengths. The employed techniques were as follows: retrograde filling (groups I and II), retrograde instrumentation and obturation (groups III and IV), retrograde instrumentation and obturation with retrograde filling (groups V and VI), class II retrograde filling (groups VII and VIII), and apicectomy (group IX – control group). Each technique employed the materials Super-EBA and MTA. After accomplishment of all operative procedures the roots were immersed in 0,2% rhodamine B for 72 hours at 37°C. After this period, the roots were washed and the impermeable coating layer was removed. Then, the longitudinal grinding of the roots was performed until the retrograde filling material was exposed, which allowed analysis of the extension of rhodamine B leakage. This evaluation was carried out through the micrometric technique by means of a ligth microscope. The original data and leakage means, measured in millimeters, were properly disposed in tables and submitted to statistical analysis, allowing the following conclusions: the techniques employing the Super-EBA retrograde filling material did not present any statistically significant differences between them; when MTA was employed as the retrograde filling material, there was statistically significant difference between the retrograde filling and class II retrograde filling techniques ( p<0,05) and between the retrograde instrumentation and obturation with retrograde filling and class II retrograde filling techniques (p< 0,05); there was no statistically significant difference between the Super-EBA and MTA retrograde filling materials employed for the different surgical techniques studied; regarding the interaction between the techniques and materials employed, the experimental groups can be displayed in the decreasing order according to their marginal sealing as follows: group VIII - class II retrograde filling with MTA; group VII - class II retrograde filling with Super-EBA; group I - retrograde filling with Super-EBA; group IX - apicectomy (control group); group IV - retrograde instrumentation and obturation with MTA; group III - retrograde instrumentation and obturation with Super-EBA; group V - retrograde instrumentation and obturation with retrograde filling with Super-EBA; group II - retrograde filling with MTA; group VIretrograde instrumentation and obturation with retrograde filling with MTA.
