Na doença periodontal associada à placa dentobacteriana, produtos bacterianos difundem-se através dos tecidos periodontais, induzindo mecanismos de defesa não específicos e específicos, com síntese de vários mediadores pró-inflamatórios, incluindo o óxido nítrico (NO). A enzima responsável pela produção de NO, a NO sintase (NOS), foi identificada em vários tipos celulares e possui ação direta sobre o metabolismo ósseo, bem como sobre fenômenos destrutivos teciduais e imunopatológicos. Com o objetivo de analisar a presença de NO na doença periodontal, realizamos um estudo quantitativo geral e proporcional com análise estatística de células iNOS positivas em amostras de tecidos gengivais clinicamente saudáveis, gengivites associadas à placa dentobacteriana e periodontites crônicas localizadas, através da técnica imuno -histoquímica. Significante aumento do número de células iNOS positivas por mm2 foi observado em amostras de gengivites e periodontites, comparadas ao grupo controle. Em todos os grupos, a maioria das células iNOS positivas eram mononucleares apresentado fraca a moderada imunorreatividade. Por outro lado, as células polimorfonucleares iNOS positivas demonstraram intensa imunomarcação, independente do estágio da doença, e apenas a porcentagem de células polimorfonucleares iNOS positivas apresentaram significante aumento comparada àquela do grupo controle. Nossos resultados indicam que, nos tecidos periodontais, o NO aumenta na presença de doença inflamatória. Neste contexto, o NO pode estar associado com a regulação da destruição tecidual e reabsorção óssea ou ainda com fenômenos imunopatológicos. Nossos resultados indicam também uma via adicional de ativação nas células inflamatórias na doença periodontal, em especial os polimorfonucleares neutrófilos, que expressam iNOS significativamente, provavelmente representando uma importante origem de óxido nítrico nesta doença.

Study of the expression of nitric oxide inducible synthase enzyme (iNOS) in plaq plaque ue--induced gingivitis and localized chronic periodontitis

In periodontal disease, bacterial products of the subgingival plaque diffuse through the periodontal tissue, inducing non-specific and specific defense mechanisms, with the consequent synthesis of various pro-inflammatory mediators, including nitric oxide (NO). The enzyme responsible for NO production, the NO synthase (NOS), has been identified in several cell types and the NO has a direct action on osseous metabolism and may also take on the tissue destruction and in immunopathological phenomena. In order to further characterize the presence of NO in human periodontal disease, we undertook a general and proportional quantitative study with statystical analysis of iNOS positive cells in sam ples of clinically healthy gingival tissues, plaque-induced gingivitis and localizated chronic periodontitis using immunohistochemistry. A significant increase in the number of iNOS+ cells per mm2 was found in the samples of the gingivitis and periodontitis compared to those of the control. In all groups almost all of the iNOS+ cells were mononuclear demonstrating weak to moderate immunoreactivity. Conversely, polymorphonuclear cells showed intense immunoreactivity for iNOS independent of the disease stage, and only the percentage of iNOS+ polymorphonuclear cells in the inflammatory infiltrate demonstrated a significant increase when compared with the control. Taken together, our results indicate that NO augments in the presence of periodontal disease, which may be associated with the regulation of tissue destruction, bone resorption and immunopathological phenomena. In addition, our findings also suggest that the inflammatory cells present an additional activation pathway on inflammatory cells in the periodo ntal disease, specially of the polymorphonuclear cells, that it express significant iNOS and probably represent an important source of nitric oxide in human periodontal disease.
