As alterações do paladar são um dos efeitos colaterais mais comuns observados imediatamente após o transplante de células hematopoiéticas (TCH). A fotobiomodulação (FBM), quando executada com dosimetria apropriada, pode modificar a circulação sanguínea, a transmissão nervosa e as células da mucosa oral, favorecendo redução da inflamação, estimulação de nervos periféricos e ativação celular. O objetivo primário deste trabalho foi verificar o efeito da FBM sobre a acuidade do paladar em pacientes sob TCH; os objetivos secundários foram verificar se a FBM acarreta alterações no aspecto clínico das papilas linguais e se modifica a percepção do paciente quanto ao paladar em geral. Foram selecionados 85 pacientes sob TCH, os quais foram randomicamente divididos em: Grupo Placebo (n=43) pacientes sem FBM, expostos ao equipamento laser, porém com o mesmo desligado; Grupo Intervenção (n=42) pacientes expostos a FBM. A intervenção consistiu em FBM (laser InGaAIP, 660 nm, 0.09 cm2 de área do spot, irradiação por ponto, 8s por ponto, 100 mW, 1.1W/cm2, 8.8J/cm2 por ponto) realizada em borda lateral, base e ápice da língua (4 pontos em cada região) durante todos os dias do TCH, desde o início do condicionamento até a enxertia neutrofílica. Foi realizado teste de acuidade do paladar envolvendo os sabores doce, salgado, azedo e amargo, cada qual com duas soluções, uma menos e outra mais concentrada. Após o paciente experimentar cada solução, foi registrado o sabor e sua intensidade reportados pelo paciente. Na mesma sessão, foi feito exame clínico da mucosa do dorso da língua, observando-se se havia despapilação ou sinal de inflamação nas papilas linguais, bem como foi perguntado a ele como estava a sensação do paladar em geral. Esses testes foram feitos um antes da FBM (no período do condicionamento T0) e dois após o início da intervenção (um executado no paciente em neutropenia T1 e outro após a enxertia neutrofílica T2). Em geral, independentemente da intensidade do sabor, observouse que a intervenção acarretou maior frequência de acertos e menor risco de percepção errônea das soluções amargas, ácidas e salgadas, mas não da solução doce, tanto em T1 quanto em T2. Considerando a intensidade do sabor, em T1, a intervenção provocou redução significativa de ageusia dos sabores doce, amargo e salgado em baixas concentrações, de hipogeusia do amargo em altas concentrações e de disgeusia do azedo em baixas concentrações. Em T2, houve redução significativa da ageusia dos sabores amargo, azedo e salgado em baixas concentrações, de hipogeusia desses mesmos sabores em altas concentrações e de disgeusia do salgado em baixa e alta concentração. No grupo Intervenção, a frequência de hiperemia foi significativamente menor em T2 comparativamente a T1 (p=0.026), porém não houve diferenças em relação ao Placebo. A frequência de despapilação foi significativamente menor no grupo Intervenção comparativamente ao Placebo, tanto em T1 (p=0.001) quanto em T2 (p=0.007). Concluiu-se que a FBM reduziu a frequência de ageusia, hipogeusia e disgeusia nos períodos imediatos após a infusão celular, bem como reduziu a despapilação e melhorou a sensação do paladar em geral nos pacientes sob TCH.

Taste alteration is one of the most common side effects detected immediately after the Hematopoietic Cell transplantation (HCT). When Photobiomodulation (PBM) is administered with proper dosage, it can modify blood circulation, neural transmission, and oral mucosa cells, promoting reduction of inflammation, stimulation of peripheral nerves, and cellular activation. The primary goal of this research was to verify the effects of PBM upon the perception of taste in patients under HCT. The secondary goals had the intent to verify whether PBM indeed result in alteration in the clinical appearance of the lingual papillae, and whether the patients perception changes regarding the taste itself. For this research 85 patients under HCT were randomly selected and divided into: Placebo Group (n=43) patients without PBM, exposed to laser equipment, however with the device turned off. Intervention Group (n=42) patients exposed to PBM. The intervention was established in PBM (laser InGaAIP, 660 nm, spot area of 0,09 cm2, irradiation per spot, 8s each spot, 100mW, 1,1 W/cm2, 8,8J/cm2 per spot) executed at the lateral surroundings, body and apex of the tongue (4 spots in each area) during every day of HCT, from the start of conditioning until the graft neutrophilia. A test for the perception of tastes was performed involving sweet, salty, sour, and bitter tastes, each with two solutions, one less and one more concentrated. After the patient try each of the solutions, the taste and its intensity were reported. At the same session, a clinical exam of the dorsum of the tongue was made, watching closely if there was depapillation or any sign of inflammation of the lingual papillae, as well as asking how the taste sensation felt in general. These tests were made before the PBM (at the conditioning period T0), and also two days after the intervention start (one performed in the patient with Neutropenia T1, and another after the graft neutrophilic T2). In general, regardless of the intensity of the taste, it was observed that the intervention led to a higher frequency of assertiveness and a lower risk of misperception of the bitter, sour and salty solutions, but not of the sweet solution, both at T1 as well as T2. Considering the taste intensity, in T1 the intervention resulted in a significative mitigation of Ageusia of sweet, bitter and salty tastes when concentration is low, of Hypogeusia of bitter taste when concentration is high and Dysgeusia of sour when concentration is low. About T2, there was significative mitigation of Ageusia of bitter, sour and salty tastes when concentration is low, and Hypogeusia of those same tastes when concentration is high, also Dysgeusia of salty taste when concentration is high or low. In the Intervention group, the frequency of Hyperemia was significative lower at T2 than T1 (p=0,026), however there was no variations regarding the Placebo. The depapillation was significative lower than the Intervention group in comparison with the Placebo, as much as T1 (p=0,001) than T2 (p=0,007). In conclusion, the PBM mitigated the frequency of Ageusia, Hypogeusia and Dysgeusia related to the immediate period after the cellular infusion, as well as mitigated the depapillation and, in general, improved the sense of taste in patients under HCT.
