O carcinoma epidermoide oral representa 90% das neoplasias malignas na cavidade oral. Por ser uma doença com grande incidência, é de suma importância entender como ela se manifesta, tanto clinicamente quanto histologicamente para obtenção de informações para elaborar o melhor tratamento para os pacientes. O sistema internacional mais utilizado para a classificação de tumores é o sistema de estadiamento clínico TNM. Porém, este sistema não garante uma análise morfológica e estrutural do carcinoma. Sendo assim, muitos pesquisadores têm dado atenção a este fato, elaborando gradações histológicas de malignidade a fim de auxiliar no tratamento. O objetivo do presente estudo foi avaliar o grau histológico de malignidade de 50 casos de carcinoma epidermoide oral localizados em língua, borda de língua e assoalho bucal por meio de sistemas de gradação histológica de malignidade por meio das gradações da Organização mundial da Saúde (OMS), de Brandwein-Gensler e Boxberg e relacionar seus resultados aos estudos mais recentes e morfológicos para enfim montar uma tabela com as características mais encontradas, discutidas e relevantes. As 50 lâminas foram coradas com HE e analisadas sob microscópio óptico de luz. Resultados: OMS, 15 bem diferenciado, 26 moderadamente diferenciado e 9 pouco diferenciado, seguido de Boxberg com 18, 27 e 5, respectivamente e Brandwein-Gensler, classificando 22 em baixo risco, 11 em risco intermediário e 17 em alto risco. Em relação aos achados do estudo, concluímos que os diferentes achados na literatura que estão em discrepância a todo o momento podem ser diminuídos caso a combinação de parâmetros morfológicos comece a ser estabelecida em uma nova gradação que inclua aspectos diversos dos carcinomas orais.

Squamous cell carcinoma represents 90% of malignant neoplasms in the oral cavity. Since it is a disease with high incidence, it is important to understand how it manifests both clinically and histologically for obtaining information to develop the best treatment and prognostic for patients. The international grading system used for classification is the clinical TNM staging system. However, this system does not guarantee a morphological and structural analysis of carcinoma. Thus, many researchers have given attention to this fact, making histological grading systems in order to assist in the treatment of patients with this neoplasia. The aim of the present study was to evaluate the histological grade of malignancy of 50 cases of oral squamous cell carcinoma located in tongue and buccal floor through histological grading systems of The World Health Organization (WHO), Brandwein-Gensler and Boxberg and relate their results to recent studies in order to highlight the most common and relevant morphological features. The 50 slides were stained with HE and analysed under optical microscope. Results: by WHO, 15 were well-differentiated, 26 moderately differentiated and 9 poorly differentiated. Boxberg with 18, 27 and 5 respectively and Brandwein-Gensler, with 22 low risk, 11 intermediated risk and 9 high risk. In relation to the findings of the study, we conclude that the different findings in the literature that are in discrepancy at all times and can be reduced if the combination of morphological parameters start being established in a new gradation that include several aspects of oral carcinomas.
