A terapia de fotobiomodulação (FBM) é utilizada há mais de 20 anos na prática odontológica, no entanto ainda há questionamentos sobre os parâmetros utilizados nas diferentes situações clínicas. Na prática, ainda existem muitas controvérsias quanto a confiabilidade da aferição dos desfechos encontrados em ensaios clínicos. Neste contexto, procuramos responder à pergunta focal: A fotobiomodulação pode favorecer um melhor reparo de feridas cirúrgicas em mucosa oral? Para isso, foi realizada uma revisão sistemática, apontada no portal Internacional de registros prospectivos de Revisões Sistemáticas - PROSPERO (CRD42020202883), seguindo as recomendações Preferenciais para Revisões Sistemáticas e Meta-Análises (PRISMA). Uma busca booleana foi executada por dois revisores independentes com palavras-chave específicas nas principais bases de dados: PubMed, Web of Science e Scopus. Estabeleceu-se critérios de elegibilidade específicos sendo que em um deles a revisão deveria considerar estudos que apresentassem desfechos de aferição quanto ao reparo da mucosa oral após intervenção cirúrgica. Foram encontrados inicialmente 1131 artigos, destes, 75 estudos estavam duplicados e 1026 não atendiam aos critérios de inclusão e foram excluídos. Considerou-se elegíveis para revisão 30 artigos completos. Após leitura completa outros 9 artigos foram excluídos e a amostra final foi constituída por 21 artigos. Deste total, 7 estudos utilizaram laser vermelho e 14 estudos fizeram uso do laser infravermelho. Utilizando-se de um formulário de extração de dados para esta revisão foram listadas características metodológicas dos estudos e construiu-se tabelas resumo de cada estudo. Após, os dados metodológicos extraídos foram avaliados quanto aos riscos de vieses metodológicos e estudos semelhantes foram agrupados para metanálise comparativa de desfechos utilizando ferramentas estatísticas do programa RevMan5.3 (Review Manager Version 5.3. Copenhagen: The Nordic Cochrane Centre, The Cochrane Collaboration, 2014). 12 As tabelas resumo mostraram a grande variabilidade de parâmetros utilizados na intervenção da terapia de FBM, as divergências de posologias e de aferições de desfechos e critérios metodológicos para a mesma condição clínica. A maioria dos estudos selecionados apresentavam baixo risco de viés, mas alta heterogeneidade metodológica quando comparados entre si. Do total dos estudos selecionados, apenas quatro foram eleitos para comparação, sendo dois que utilizaram o laser vermelho e dois o infravermelho. A metanálise mostrou que a FBM por laser vermelho ou infravermelho não apresenta evidências de favorecer significativamente o reparo em mucosa oral após uma intervenção cirúrgica. Nos limites desta revisão, podemos concluir que os estudos precisam ser cuidadosamente delineados sob o ponto de vista de um ensaio clínico reprodutível e transparente com parâmetros comparáveis entre si e utilizando maiores amostras da população incluindo critérios de aferição de desfechos semelhantes para que se possa verificar a existência de efeito favorecedor da FBM na reparação tecidual pós-operatória da mucosa oral.

Photobiomodulation therapy (PBM) has been used for more than 20 years in dental practice, however there are still issues to be solved about the parameters used in different clinical situations. In practice, there are still many controversies regarding reliability of outcomes that were found in clinical trials. In this context, we aimed to answer the focus question: Can photobiomodulation favor a better repair of surgical wounds in oral mucosa? For this, a systematic review was carried out, pointed out in the International registration portal for systematic Reviews - PROSPERO (CRD42020202883), following the preferred recommendations for Systematic Reviews and Meta-Analyses (PRISMA). Two independent reviewers performed a Boolean search with specific keywords in the main medical science databases PubMed, Web of Science, Scopus. Specific eligibility criteria were established and in one of them the review should consider studies that presented outcomes measurement regarding oral mucosa repair after a surgical intervention. Initially, 1,131 articles were found, of which 75 were duplicated and 1,026 did not meet the inclusion criteria and were excluded. Thirty full papers were considered eligible for review. After full reading, nine other articles were excluded, and the final sample consisted of 21 studies. Of this total, seven used red laser and 14 studies used infrared laser. Using a data extraction form for this review, methodological characteristics of the studies were listed, and summarized in tables. After, the methodological data extracted were evaluated for risks of methodological bias and similar studies were grouped for comparative meta-analysis of outcomes and heterogeneity using statistical tools of the RevMan 5.3 program (ReviewManager Version 5.3. Copenhagen: The Nordic Cochrane Centre, The Cochrane Collaboration, 2014. The summary tables showed the great variability of parameters used in FBM therapy intervention, there were divergences of dosages and measurements of outcomes and methodological criteria for the same clinical condition. Most of the selected 14 studies had a low risk of bias, but a high heterogeneity of methods when compared to each other. From the total of the selected studies, only four were elected for comparison, two used the red laser and two used infrared lasers. Meta-analysis showed that red or infrared laser PBM therapy has no evidence to significantly favor repair in the oral mucosa after a surgical intervention. Within the limits of this review, we can conclude that the studies employing PBM therapy need to be carefully delineated from the point of view of a reproducible and transparent clinical trial with parameters comparable to each other and using larger samples of the population including measurement of similar outcomes criteria to verify the existence of a favorable effect of FBM on postoperative tissue repair of the oral mucosa.
