A cirurgia ortognática é o procedimento cirúrgico que visa o reposicionamento da maxila e mandíbula, tendo como objetivo o reestabelecimento da harmonia facial. É fundamental na fase pré-operatória planejarmos o posicionamento tridimensional da localização dos incisivos centrais superiores, principalmente a provável futura relação deles com o lábio superior após a cirurgia ortognática. Este trabalho tem como objetivo estabelecer uma relação entre os movimentos da maxila realizados na cirurgia ortognática com a alteração da exposição dos Incisivos Centrais Superiores em repouso (ICS). Por meio de um estudo observacional longitudinal retrospectivo contendo uma amostra de 40 indivíduos com diagnóstico de discrepância maxilomandibular submetidos a cirurgia ortognática bimaxilar, analisou-se os movimentos realizados do ponto ICS por meio de sobreposição digital de tomografia computadorizada pré e pós-operatória, estabelecendo uma relação com a alteração ocorrida na exposição dos ICS em repouso. Os resultados indicaram tendência geral, com significância estatística, para as seguintes proporções: aumento de 0,25mm na exposição do ICS a cada 1mm de avanço da maxila (0,25:1) e, aumento ou diminuição de 0,78 na exposição do ICS a cada 1mm de extrusão ou intrusão de maxila respectivamente (0,78:1).

Orthognathic surgery is the surgical procedure aimed at repositioning the maxilla and mandible, with the objective of restoring facial harmony. It is essential in the preoperative phase to plan the three-dimensional positioning of the location of the upper central incisors (ICS), especially the likely future relationship between them and the upper lip after orthognathic surgery. This work aims to establish a relationship between the movements of the maxilla performed in orthognathic surgery with the alteration of the exposure of the Upper Central Incisors (UCI) at rest. Through a longitudinal observational retrospective study containing a sample of 40 individuals diagnosed with maxillomandibular discrepancy undergoing bimaxillary orthognathic surgery, the movements performed from the UCI point were analyzed by means of digital overlapping of pre and postoperative tomographies, establishing a relationship with the change in the exposure of the UCI at rest. The results indicated a general trend, with statistical significance, for the following proportions: increase of 0,25 mm in the exposure of the UCI for every 1 mm of advance of the maxilla (0,25: 1) and, increase or decrease of 0.78 in the exposure of the UCI every 1 mm of extrusion or intrusion of the maxilla respectively (0,78: 1).
