Esta revisão sistemática visa avaliar os trabalhos sobre os resultados de comparações entre os dentifrícios convencionais e os dentifrícios que são compostos por produtos herbais (naturais). Foram usados bancos de dados eletrônicos (MEDLINE, PubMed, EMBASE e LILACS) somado a pesquisas manuais para selecionar os artigos. Foram incluídos ensaios clínicos randomizados com textos completos a partir de 1995 até março de 2022 nas línguas inglesa, espanhola e portuguesa, levando em consideração os ingredientes naturais (herbais) em comparação com os dentifrícios fluoretados convencionais. O texto dessa revisão sistemática foi estruturado de acordo com as diretrizes do PRISMA (Itens de Relatório Preferidos para Revisões Sistemáticas e Meta-análises), do Cochrane Handbook of Systematic Reviews of Interversions, e Check Review checklist30. A pesquisa gerou 199 resultados sendo que após a triagem inicial foram selecionados 56 artigos para leitura completa por dois avaliadores independentes, sendo selecionados 24 trabalhos para análise. As discordâncias foram resolvidas por discussão com um terceiro revisor. Os estudos que preencheram os critérios de inclusão foram submetidos a uma avaliação de validade e extração de dados. A meta-análise foi realizada com os dados coletados de 14 RCTs. As estimativas indicaram diferenças significativas entre os grupos para o índice de placa (WMD: 0,22; IC 95%: 0,18,0,26; p < 0,00001; i2 100%). Já para o índice gengival não há diferenças estatisticamente significantes (WMD: 0,00; 95% CI: 0,02,0,01; p < 0,00001; i2 92%). Feita análise do índice gengival e índice de placa, concluiu-se que os dentifrícios herbais não acrescentam benefícios se comparados aos comumente fluoretados convencionais.

This systematic review aims to evaluate the work on the results of comparisons between conventional toothpastes (fluoride) and toothpastes that are composed of herbal products (natural). Electronic databases (MEDLINE, PubMed, EMBASE and LILACS) plus manual searches were used to select articles. Randomized clinical trials with full texts from 1995 to March 2022 in English, Spanish and Portuguese were included, taking into account the natural (herbal) ingredients in comparison with the fluoridated dentifrices considered. The text of this systematic review was followed according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines, the Cochrane Handbook of Systematic Reviews of Interversions, and the Check Review checklist30. The research generated 199 results, and after an initial screening, 56 articles were selected for complete reading by two independent evaluators, with 24 articles being selected for analysis. Disagreements were resolved by discussion with a third reviewer. Studies meeting the inclusion criteria were accepted for an assessment of validity and data assurance. A meta-analysis was performed on data collected from 14 RCTs. Estimates indicated differences between groups for plaque index (WMD: 0.22; 95% CI: 0.18,0.26; p < 0.00001; i2 100%) As for the gingival index, there are no statistically differences (WMD: 0.00; 95% CI: 0.02,0.01; p < 0.00001; i2 92%). After analysis of the gingival index and plaque index, it was concluded that herbal dentifrices do not add benefits compared to the commonly conventional fluoridated ones.
