No contexto da prevenção da infecção cruzada na radiologia odontológica, superfícies que entram em contato direto com as mãos dos profissionais, como as superfícies da câmara escura portátil, devem ser descontaminadas. A recomendação clássica da descontaminação com álcool 70% p/v após limpeza prévia, nem sempre é seguida justificada pela não praticidade, acrescida do tempo curto disponível entre um atendimento e outro, limitando-se à aplicação direta do álcool o que, a priori, contraria as Boas Práticas de Controle de Infecção. A presente investigação, de caráter experimental laboratorial, controlado, randomizado e unicegado, teve como objetivo avaliar comparativamente a eficácia desinfetante do álcool 70% (p/v) sob fricção, com e sem limpeza prévia nas superfícies de câmaras escuras portáteis como procedimento de limpeza/desinfecção concorrente em radiologia odontológica. Três grupos de estudo foram constituídos: Grupo controle: superfícies previamente contaminadas com desafio (micro-organismo teste Serratia marcescens ATCC 14756, acrescido de 10% de saliva humana), submetidas à limpeza clássica por meio de água e detergente sob fricção, e posterior enxágue e consecutivamente, desinfecção com aplicação do álcool 70% p/v. Grupo experimental: superfícies previamente contaminadas com desafio, à semelhança do grupo controle, submetidas à aplicação direta do álcool 70% p/v sob fricção, SEM limpeza prévia. Grupo controle positivo: superfícies contaminadas com desafio. O tamanho amostral para os grupos experimental e controle foi de 85 unidades. Os experimentos foram realizados no Laboratório de Ensaios Microbiológicos do Departamento de Enfermagem Médico-Cirúrgica da Escola de Enfermagem da Universidade de São Paulo. Após o procedimento de contaminação, desafio da área superior da câmara escura portátil e sua posterior secagem, no grupo experimental a superfície foi diretamente friccionada com álcool 70% (p/v) durante 30 segundos. No grupo controle, o mesmo procedimento foi realizado, após a limpeza prévia. Imediatamente, após a evaporação do álcool da superfície, foi realizada a coleta microbiológica por meio da fricção com swab. Em seguida, a ponta do swab foi quebrada e mergulhada em um tubo de ensaio esterilizado com 1 mL de SF e agitado em Votex® por 30 segundos, intermitente e todo o conteúdo do tubo de ensaio foi vertido no centro de uma placa de Petri e, sobre ele, derramados 15 a 20 mL do meio Trypticase Soy Agar à temperatura aproximada de 30ºC. As placas foram deixadas na posição invertida, em ambiente à temperatura aproximada de 22ºC por 72 horas, com leitura diária da recuperação das UFC da Serratia marcescens. O controle positivo foi colhido da mesma forma que o dos Grupos Controle e Experimental, porém sem a aplicação de métodos para descontaminação. As médias das UFC foram comparadas entre os grupos experimental e controle, por meio do teste estatístico t de Student. Os resultados permitiram concluir que o álcool 70% p/v, quando aplicado diretamente sobre a superfície contaminada com matéria orgânica sob fricção, é um desinfetante eficaz, comprovado pela redução de seis logaritmos da população microbiana intencionalmente desafiada nos experimentos, não diferindo da redução observada no grupo controle (p=0,440). Estes achados permitiram atestar a segurança desta prática na limpeza/desinfecção concorrente durante as práticas de radiologia odontológica.

Aiming to avoid cross infection in the context of dental radiology, surfaces that get in direct contact with the hands of the professionals involved in dental procedures are ought to be decontaminated, such as the portable darkroom surfaces. The classical recommendation for the alcohol 70% w/v decontamination (which requires previous cleaning process) is not always applied due to its lack of practicality in addition to the short amount of time between dental procedures; so the decontamination is restricted only to the application of alcohol contradicting then, a priori, the good Practices of Infection Control. This investigation, which was conducted following laboratorial and experimental character, controlled, randomized and blinded, has had as its aim: evaluating comparatively the disinfecting efficacy of rubbing the surfaces of portable darkrooms with 70% w/v alcohol, with and without previous cleaning processes, as an alternative procedure for cleaning/disinfection in dental radiology. Three study groups were established: Control Group: previously contaminated surfaces with the worst case contaminant (test microorganism Serratia marcescens ATCC 14756 added to 10% of human saliva) firstly, traditionally cleaned by rubbing with water and detergent, rinsed afterwards then decontaminated using alcohol 70% w/v; Experimental Group: previously contaminated surfaces with the worst case contaminant similarly to Control Groups sample, however submitted straight away to the decontamination by rubbing with alcohol 70% w/v, with NO previous cleaning processes; Positive control Group: surfaces contaminated with the worst case contaminant. The sampling for both, the experimental and control groups, was 85 units. The experiments were conducted at the Microbiology Laboratory of the Medical-Surgical Nursing Department of the University of Sao Paulos Nursing School. After the worst case contamination procedure of the portable darkrooms upper area and drying, at the experimental group: the surface was directly rubbed with alcohol 70% w/v for thirty seconds. At the control group, the same procedure was performed, however, with previous cleaning following the traditional processes. Immediately after the alcohols evaporation from the surface, a microbiological collection was performed by swab friction. Afterwards, the swabs tip was broken and put in a sterile test-tube with 1mL of sterile saline solution and shaken in Vortex® for thirty seconds, and all its content poured into a Petri dish. Fifteen to 20 mL of Trypticase Soy Agar at approximately 30ºC was poured over the Petri dish. The dishes were left inverted, in an acclimatized environment (approximately at 22ºC) for 72 hours, with daily assessment of the Serratia marcescens CFUs (Colony-forming unit) growths. The positive control was collected following the same procedure as the other groups (control and experimental), with no decontamination methods applied. The CFU levels were compared between the Control and Experimental groups, regarding the statistic test of the t test of Student. The results allowed concluding that the alcohol 70% w/v rubbing, when applied straight to the contaminated surface, is an efficient disinfection method, proved by the 6 logs reduction of the worst case bacterial population; no different from the reduction observed at the control group (p=0,440 test t of Student). These results allow attesting and ratifying the safety of this alternative cleaning/disinfection processes in the context of dental radiology.
