Introdução: O tratamento endodôntico pode ser desafiador quando os condutos radiculares se encontram parcialmente ou completamente obliterados, mesmo contando com o auxílio de um microscópio operatório. Principalmente nesses casos, as guias cirúrgicas endodônticas podem ser confeccionadas para amparar o clínico a obter sucesso terapêutico. Objetivo: Esta Revisão Sistemática analisou se as guias cirúrgicas endodônticas apresentam efetividade em estudos ex vivo. Materiais e métodos: Protocolo PROSPERO número CRD42021278614. Dois revisores realizaram uma busca formal nos bancos de dados eletrônicos, sem restrição de idioma ou data, nas seguintes bases de dados: MEDLINE via PubMed, EMBASE, Scopus, LILACS, Web of Science, and SIGLE via OpenGrey. Os artigos encontrados inicialmente passaram por análise de título e resumo aplicando-se os critérios de elegibilidade para seleção. Os artigos que contemplaram os critérios de elegibilidade passaram por análise integral. Resultados: Dos 1017 artigos analisados inicialmente, 4 estavam totalmente adequados aos critérios de elegibilidade. Após análise quantitativa e qualitativa dos estudos selecionados, a efetividade das guias endodônticas foi de 87.3% nas amostras dos estudos incluídos. Sendo que no restante dos casos foi possível acessar os canais com pequenas manobras complementares. Conclusão: As guias endodônticas apresentam alta efetividade e acurácia que facilitam e asseguram o acesso aos condutos radiculares, sendo capazes de aumentar as chances de sucesso no tratamento endodôntico. Mais estudos principalmente que contemplem regiões de molares são necessários para corroborar a validação da técnica.

Introduction: Endodontic Treatment can show up in a challenge way due to the canal system being partly or completely obliterated, even with the support of an operating microscope. Mainly in these cases, the endodontics surgical guides can be used to aid the clinician to get success in endodontic therapy. Aim: This Systematic Review analyzed whether the Endodontics Surgical Guides are effective in ex vivo studies. Materials and methods: PROSPERO protocol number CRD42021278614. Two reviewers accomplish a formal search on electronic databases, without language or date restrictions, on the fallowing databases: MEDLINE via PubMed, EMBASE, Scopus, LILACS, Web of Science, and SIGLE via OpenGrey. The initially gathered articles underwent title and abstract analyses applying the eligibilities criteria for selection. The articles that contemplated the eligibility criteria underwent full analyses. Results: From the 1017 articles first analyzed, 4 were fully compatible with the eligibility criteria. After qualitative and quantitative analysis of the selected studies, the effectiveness of the endodontic surgical guides was 87.3% in the samples of the included studies. While in the others it was possible to get access after minor handling. Conclusion: Guided endodontics shows high effectiveness and accuracy that facilitate and ensure access to root canals, increasing the chances of success in endodontic therapy. Further studies, mainly involving molar regions, are needed to corroborate the validation of the technique.
