A análise da qualidade óssea é um fator imprescindível para o sucesso do tratamento de implantes. Este estudo propôs uma avaliação do tecido ósseo em imagens de Tomografias Computadorizadas de Feixe Cônico (TCFC) após cirurgias de implantes, correlacionando com análises das Dimensões Fractais (DF), Intensidade de pixel (IP) e análises de estabilidade primária de implantes - Frequência de Ressonância (AFR) e Torque de Inserção (TI) e, verificou possível associação com o Índice de Qualidade Óssea subjetivo (IQO). Foram analisadas 846 regiões de interesse, com média de idade de 58,27±10,5 anos, cujas imagens e prontuários pertenciam ao banco de dados do Laboratório para Análise e Processamento de Imagens (LAPI-FOUSP). Os dados cirúrgicos (AFR e TI) foram obtidos no ato da instalação dos implantes, os dados quantitativos (DF, IP) obtidos das imagens tomográficas após a instalação dos implantes e, os dados qualitativos (IQO) foram analisados por dois observadores experientes na área. Foram realizados os testes estatísticos de Correlação de Pearson, Teste de Kolmogorov-Smirnov, Levene, ANOVA, Regressão Linear Múltipla e Qui-Quadrado, com nível de significância de 95% (p<0,05). Os resultados mostraram correlações significantes entre TI e AFR_MD e VL respectivamente (r=0.267 e r= 0.284); TI e IP cervical (r=0.259); TI e IP médio (r=0.268); TI e DF cervical (r=0.233); AFR e IP médio (r=0.229); AFR e IP apical(r=0.263); IP cervical e IP Médio (r=0.596); IP médio e IP apical (r=0.309); IP médio e DF médio(r=-0.142); IP apical e DF cervical(r=0.329). O teste ANOVA identificou diferença significativa (p<0,05) para as médias das variáveis IP Médio, DF Médio e AFR entre as arcadas superior e inferior. Na análise de Regressão Linear múltipla, destacou-se que as variáveis DF Médio e DF cervical são úteis para prever o torque de inserção. A análise de IQO subjetiva apresentou concordância razoável entre os observadores. O estudo concluiu que diante dos fatos apresentados a análise da qualidade óssea pode ser categorizada detalhadamente de acordo com as regiões coronais, médias e apicais, uma vez que cada região apresenta trabeculado ósseo e cortical diferentes. Os parâmetros de análise de estrutura óssea como DF e IP podem ser considerados métodos não invasivos pré-operatórios eficazes para o planejamento de implantes por meio de imagens tomográficas.

Bone quality analysis is an essential factor for the success of implant treatment. This study proposed a bone tissue evaluation on Cone Beam Computed Tomography (CBCT) images after implant surgery, correlating with analyzes of Fractal Dimension (FD), Pixel Intensity (PI), analysis of primary implant stability - Resonance Frequency (RF), Insertion Torque (IT) and verifying a possible association with the subjective Bone Quality Index (BQI). 94 regions of interest of 46 patients were analyzed, with a mean age of 58.27 ± 10.5 years, whose images and medical records belonged to the database of the Laboratory for Image Analysis and Processing (LAPI-FOUSP). The surgical data (RF and IT) was obtained during the implant installation, the quantitative data (FD, PI) was obtained from tomographic images acquired after implant installation, and the qualitative data (BQI) was analyzed by two experienced observers in the area. Pearson's Correlation, Kolmogorov-Smirnov, Levene, ANOVA, Multiple Linear Regression and Chi-Square for related samples tests were performed with a significance level of 95% (p <0.05). The results showed significant correlation between IT and RF (r = 0.267 and r = 0.284); IT and cervical PI (r = 0.259); IT and middle PI (r = 0.268); IT and cervical FD (r = 0.233); RF and middle PI (r = 0.229); RF and apical PI (r = 0.263); Cervical PI and middle PI (r = 0.596); middle PI and apical PI (r = 0.309); middle PI and middle FD (r = -0.142); Apical PI and cervical FD (r = 0.329). The ANOVA test identified significant difference (p <0.05) on the means of the variables: Middle PI, Middle FD and RF between the upper and lower arches. The multiple linear regression highlighted that the variables middle FD and cervical FD are useful to predict the insertion torque. The subjective BQI analysis showed reasonable agreement between the observers. This study concluded that, given the presented facts, the analysis of bone quality can be categorized in detail according to coronal, middle and apical regions, since each region presents different bone and cortical trabeculae. Bone structure analysis parameters such as FD and PI can be considered effective preoperative non-invasive methods for implant planning through tomographic images.
