A tuberculose é considerada um importante problema na saúde global, sendo a segunda causa de morte entre as doenças infecciosas tendo como prioridade para o seu controle o diagnóstico precoce e tratamento adequado, necessitando de uma rede de atenção capaz de preservar a continuidade da assistência. Entre os tipos de continuidade, temos a relacional que implica o contato continuado entre o usuário e os serviços, com senso de responsabilidade com relação ao doente, dando sustentabilidade ao cuidado através do tempo. Assim, a análise de discurso de matriz Francesa, dentro desse contexto, torna-se um importante referencial teórico- metodológico capaz de compreender os elementos dessa continuidade presentes no discurso do doente. Esse estudo objetivou analisar a discursividade do doente sobre a continuidade da assistência durante o tratamento. Trata-se de uma pesquisa com abordagem qualitativa e delineamento descritivo. Foi realizada em um dos centros de referência para o atendimento da tuberculose no município de Ribeirão Preto, no período de julho à setembro de 2014. Os sujeitos do estudo foram dez doentes de tuberculose em tratamento diretamente observado e/ou em acompanhamento clínico nesse centro de referência. Para coleta de dados foram realizadas entrevistas em profundidades agendadas previamente. Esse estudo foi aprovado pelo Comitê de Ética em Pesquisa da Escola de Enfermagem de Ribeirão Preto, sendo ainda respeitados os preceitos éticos conforme recomendações contidas na resolução nº 466/2012 do Conselho Nacional de Saúde. A análise dos dados ocorreu por meio do referencial teórico- metodológico da análise de discurso de matriz Francesa, sendo que oito entrevistas foram utilizadas a fim de contemplar o objetivo do estudo. Ao estabelecermos o corpus para análise, quatro blocos discursivos foram identificados. O primeiro está relacionado com a produção de sentidos da doença para o doente, sendo eles o câncer, a aids, o uso de drogas, a população carcerária, o isolamento, o bicho de sete cabeças, o caráter punitivo por meio do discurso religioso e a normalidade do adoecimento por parte do profissional de saúde. O segundo está relacionado a produção de sentidos quanto ao vínculo entre doentes e profissionais de saúde, relacionados com a fragilidade do vínculo no que concerne a rotatividade dos profissionais, a objetivação do doente durante o atendimento e a não identificação de outras categorias profissionais, que não a médica, como integrantes do cuidado; e a potencialidade do mesmo a partir da acessibilidade, boa comunicação, e a disponibilização de incentivos (cesta básica e vale transporte). O terceiro está relacionado com a produção de sentidos quanto a modalidade de supervisão do tratamento e a regulação de autonomia do doente, visto tanto como fragilidade quanto como potencialidade. E como quarto bloco discursivo, o empoderamento foi identificado como um sentido circulante para continuidade do tratamento, sendo mediado pelo próprio sujeito por meio da internet ou como uma necessidade durante as consultas clínicas ou em visitas domiciliárias. Acreditamos que a compreensão desse processo pela perspectiva do doente com o aporte do referencial da análise de discurso trouxe subsídios importantes para a prática profissional com foco para qualificação do cuidado aos adoecidos por tuberculose

Tuberculosis is considered a major problem in global health, being the second cause of death among infectious diseases having as a priority for its control early diagnosis and proper treatment, and requiring a care network able to preserve the continuity of care. Among the types of continuity, we have the relational implying the continued contact between the user and services, with a sense of responsibility towards the patient, giving care sustainability over time. Therefore, the French pattern of discourse analysis, in this context, becomes an important theoretical and methodological framework able to understand the elements of the continuity presents in the patient's discourse. This study aimed to analyze the discourse of the patient about the continuity of care during his treatment. This is a research with qualitative and descriptive design approach. We held this research in one of the reference centers for care of tuberculosis in Ribeirão Preto, from July to September of 2014. The subjects of this were ten tuberculosis' patients on directly observed treatment and/or clinical follow up in this reference center. For data collection we conducted depths interviews scheduled. This study was approved by the Ethics in Research Committee of the College of Nursing of Ribeirão Preto, being respected the ethical principles as recommendations contained in Resolution No. 466/2012 of the National Health Council. Data analysis was carried out through the theoretical and methodological framework of the French pattern of discourse analysis, and eight interviews were used in order to contemplate the purpose of this study. To establish the corpus for analysis, we identified four discursive blocks. The first is related to the production of meanings of the disease to the patient, they being cancer, aids, drug use, prison populations, isolation, "bicho de sete cabeças", the punitive character through religious discourse and the normality of illness by the health care professional. The second is related to production of meanings about the link between patients and healthcare professionals, related to the bond's weakness was the rotation of personnel, the objectification of the patient during care and the failure to identify other professions, not just medical, as care members; and the potential of it was the accessibility, good communication, and the provision of incentives (food basket and transportation vouchers). The third is associated with the production of meanings related to treatment supervisory mode and regulation of patient autonomy, seen as both fragility and potentiality. And as the fourth discursive block, empowerment has been identified as a current sense for continuity of care, being mediated by the subjects themselves through the Internet or as a necessity during clinic visits or home visits. We believe that understanding this process by the patient's perspective with the contribution of discourse analysis framework has brought important benefits for professional practice focused in qualification of care to diseased with tuberculosis
