A lesão medular consiste em um dano à medula espinhal que tem como sequela disfunções variadas, dentre elas o comprometimento da participação e autonomia do indivíduo por ela acometido. Visando avaliar a participação e a autonomia de indivíduos adultos e idosos com doenças crônicas o instrumento Impact on Participation and Autonomy (IPA) foi elaborado na Holanda com base na Classificação Internacional de Funcionalidade, Incapacidade e Saúde. A avaliação é realizada mediante 32 itens, sendo também verificada como tal limitação é tida como problemática pelo indivíduo por meio de 8 itens. O objetivo deste trabalho foi validar o IPA para indivíduos com lesão medular no Brasil. O instrumento foi submetido à validação semântica com 18 indivíduos com lesão medular com idade entre 21 e 68 anos, sendo a maioria deles (16) homens com lesão completa da medula espinhal torácica devido a acidentes de carro. Para análise das propriedades psicométricas da versão adaptada o instrumento foi enviado online para indivíduos com lesão medular sendo obtidas 212 respostas válidas relativas à versão do IPA com quatro domínios e 98 com cinco. Os respondentes, de ambas versões, eram em sua maioria, do sexo masculinho (64,2%; 66,3%, respectivamente) com lesão medular cervical e torácica. A análise psicométrica de ambas versões do IPA revelou ausência dos efeitos floor e/ou ceiling, satisfatória validade de construto convergente por meio da análise Multitraço-multimétodo (valores superiores à 0,40) e divergente (índices de ajuste superiores a 85%), e satisfatória fidedignidade quanto à consistência interna conforme o Coeficiente Alfa de Cronbach (valores superiores a 0,80). A comprovação da estrutura fatorial foi realizada para a versão do IPA com 4 domínios (GFI: 0,987; AGFI: 0,984; PGFI: 0,824; SRMR: 0,071; NFI: 0,985). Os resultados das análises psicométricas de fidedignidade e validade de construto convergente e divergente apontam na direção de que a estrutura fatorial para os construtos "participação e autonomia" com 5 domínios, doravante denominada IPA-Br5, deverá também ser confirmada e, igualmente à versão com 4 domínio (IPA-Br4), poderá ser utilizada no Brasil em estudos clínicos e de intervenção.

The spinal cord injury is a damage to the spinal cord that leads sequels, like the commitment of the subject's participation and autonomy. To evaluate these variables in aduls and elderly with crhonic diseases the questionnaire Impact on Participation and Autonomy (IPA) was developed in Holanda based on the International Classification of Functioning, Disability and Health. The evaluation is done by 32 items. Besides, 8 items check how the limitation is problematic to the person. This study aimed to validate the IPA to subjects with spinal cord injury in Brazil. The questionnaire was submitted to the semantical validation with 18 patients with spinal cord injury aging between 21 and 68 years, being most of them (16) men with complete thoracic spinal cord injury due to car accidents. To the psychometric properties of the adapted version the questionnaire was sent online to the participants. It was obtained 212 valid answers related to the four-domain IPA and 98 to the five-domain. In both verions the subjects were primarily male (64,2%; 66,3% respectively). The psychometric analysis of both domains showed no floor and/or ceiling effects, good convergent validity through the multitrat multimethod (values higher than 0.40) and divergent validity (indices higher than 85%), and satisfactory reliability regarding the internal consistency according to Cronbach's Alpha (higher than 0.80). The factorial structure was perfmed by the confirmatory factor analysis to the four-domain IPA (GFI: 0.987; AGFI: 0.984; PGFI: 0.824; SRMR: 0.071; NFI: 0,985). The results of the psychometric analysis reliability and construct validity point to the direction that the factorial structure for the constructs "participation and autonomy" with 5 domains, hereinafter referred to as IPA-Br5, should also be confirmed and, similarly to the 4 domain (IPA-Br4), it may be used in Brazil in clinical and intervention studies.
