O alto e, muitas vezes, inadequado consumo de antimicrobianos associado às consequências desse comportamento, como desenvolvimento de resistência bacteriana e infecções por germes resistentes em ambiente hospitalar ou em comunidade é considerado uma crise emergencial para a saúde pública. No ambiente hospitalar, 30% dos pacientes hospitalizados vão receber algum tipo de antibiótico e existe algum tipo de inadequação em até 50% de todas as prescrições desse tipo de medicamento. Para melhorar esse cenário, é obrigatório, no Brasil, que os hospitais possuam programas de uso racional de antimicrobianos. Dentro do contexto de estratégias para uso racional de antimicrobianos, este estudo retrospectivo observacional visa avaliar o impacto da implementação da pré-autorização obrigatória para o uso de antimicrobianos específicos sobre o custo e consumo de antimicrobianos no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo. A medida restritiva teve início formal, neste serviço, em setembro de 2014 e continua a ser realizada até o presente momento, juntamente com estratégias de consultoria, auditoria e feedback. Para a avalição proposta, foi realizada análise de custo pelo método de microcusteio e análise de consumo pela metodologia DDD/ATC da Organização Mundial da Saúde, extraindo-se dados mensais referentes ao uso de 15 antimicrobianos restritos e 8 não restritos, por paciente, entre janeiro de 2010 e dezembro de 2018. Para compor a avaliação econômica também avaliamos o investimento com a equipe médica responsável pelo gerenciamento do uso de antimicrobianos. A análise estatística das séries temporais geradas foi feita aplicando-se a técnica de segmentação binária para busca de pontos de mudança nas médias mensais de custo e consumo. O investimento com a equipe médica se mostrou favorável para a economia hospitalar. Houve redução de 40% do custo e redução de 15% em relação ao consumo dos antimicrobianos restritos, temporalmente relacionadas ao início da medida. Tais reduções deveram-se principalmente à diminuição do consumo com antifúngicos: Voriconazol (redução imediata do consumo em 58,5%) e Anfotericina B Lipossomal (redução imediata do consumo em 24,4%). Dentre os antibióticos restritos, houve expressiva redução do consumo de Amicacina, 62,2%, com consequente aumento do consumo de Gentamicina em 69,2%, medicamento não restrito. Durante o período pós restrição outros antimicrobianos não restritos como Meropenem e Vancomicina também tiveram consumo aumentado. Diante dos resultados, a medida restritiva demonstrou capacidade de reduzir consumo e custo, principalmente com antifúngicos, mas faz-se necessário atentar para efeitos adversos indesejados da restrição, como o aumento de consumo de antimicrobianos não restritos de forma secundária.

The high, and often inappropriate, consumption of antimicrobials and the associated consequences of this behavior, such as the development of bacterial resistance and infections by resistant germs in a hospital or community environment is considered an emergency crisis for public health. In a hospital environmental, 30% of hospitalized patients will receive some type of antibiotic and there is some kind of inadequacy in up to 50% of all prescriptions for this type of medication. To improve this situation in Brazil, it is mandatory that hospitals implement programs for the rational use of antimicrobials. Within the context of strategies for the rational use of antimicrobials, this retrospective and observational study aims to assess the impact of the implementation of mandatory pre-authorization for the use of specific antimicrobials on the cost and consumption of antimicrobials at the Hospital das Clínicas of the Ribeirão Preto Medical School from the University of São Paulo. The restrictive measure started formally in September 2014 and continues to be carried out along with consulting, auditing, and feedback strategies. For the proposed assessment, a cost analysis was performed using the micro currency method and consumption analysis using the DDD / ATC methodology of the World Health Organization. Monthly data regarding the use of fifteen restricted antimicrobials and eight unrestricted, per patient, was extracted from between January 2010 and December 2018. To perform the economic assessment, we also evaluated the financial investment into the medical team responsible for managing the use of antimicrobials. To perform the statistical analysis, the binary segmentation technique was applied to the generated time series to search for points of change in the monthly averages of cost and consumption. The investment in the medical team proved financial favorable for the hospital. There was a 40% reduction in cost and 15% reduction in the consumption of restricted antimicrobials, temporally related to the beginning of the measure. Such reductions were mainly due to the decrease in consumption of antifungals: Voriconazole (reduced consumption by 58.5%) and Amphotericin B Lipossomal (reduced consumption by 24.4%). Among the restricted antibiotics, there was a significant reduction in the consumption of Amikacin, 62.2%, and a consequent increase in the consumption of Gentamicin in 69.2%, an unrestricted medication. During the post-restriction period, other unrestricted antimicrobials such as Meropenem and Vancomycin also saw increased consumption. In view of the results, the restrictive measure demonstrated the ability to reduce consumption and cost, especially with antifungal agents, but it is necessary to recognize the unwanted adverse effects of the restriction, such as the increase in the consumption of unrestricted antimicrobials in a secondary way.
