Introdução: A maioria dos meninos com atraso puberal apresenta uma variante clínica da normalidade denominada retardo constitucional de crescimento e puberdade (RCCP). Entretanto, em alguns pacientes, o atraso puberal é patológico e denominado hipogonadismo. A diferenciação entre RCCP e hipogonadismo é um desafio frequente, pois o diagnóstico definitivo não pode ser feito no momento das primeiras avaliações e o seguimento longitudinal, muitas vezes, é a única maneira de confirmá-lo. Apesar de o RCCP ser a principal causa de atraso puberal, a evolução puberal e o desfecho estatural de meninos com RCCP ainda não são bem conhecidos. Objetivos: Avaliar e descrever o padrão de desenvolvimento puberal e pondero-estatural, bem como a estatura final, em pacientes do sexo masculino com RCCP. Pacientes e Métodos: Estudo retrospectivo com dados de prontuários dos pacientes do sexo masculino com diagnóstico de RCCP atendidos no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo em um período de 35 anos (de 1984 a 2019). Prontuários de 46 meninos com RCCP foram analisados. Informações clínicas, laboratoriais e radiológicas foram descritas e analisadas através de medidas-resumo. Avaliação antropométrica seriada foi convertida em escore-Z e analisada através do coeficiente de observação de correlação seriada de Bland & Altman. A predição de estatura através do calculo do alvo parental e da predição de estatura adulta de Bayley- Pinneau foi realizada e comparada com a estatura final através do teste de concordância de Bland & Altman. Resultados: A maioria dos pacientes nasceu a termo (78,3%) e com peso adequado para idade gestacional (84,8%). História familiar de atraso puberal estava presente em 39,2% dos casos. A média de idade na primeira avaliação foi de 14,3 anos (4,8 - 16,2). Baixa estatura no período prépuberal foi a razão de busca de atendimento em 47,9% dos casos. Na primeira avaliação a mediana de escore-Z de estatura era -2,5 (-3,4 a 0,8), sendo 93,4% abaixo do escore-Z -2 DP e 53,4% estava abaixo da estatura-alvo parental. Na última visita (mediana de idade de 17,7 anos), a mediana de escore-Z de estatura era -1,3 (-2,8 a 0,8), apenas 17,4% dos pacientes tinham escore-Z de estatura menor que -2DP e 4,6% estava abaixo do canal familiar. Para pacientes cuja estatura final é conhecida (n = 23), A estatura final foi semelhante à estatura alvo (mediana de 170,5 cm e 169,6 cm, respectivamente). Na primeira avaliação, o atraso da idade óssea foi observado em 82,6% dos pacientes. A previsão da estatura adulta pelo método Bayley-Pinneau superestimou em 5 cm (-0,9 a 8) a estatura na maioria das vezes. A mediana de idade de início espontâneo da puberdade (Tanner G2) foi de 15 anos e sua duração mediana foi de 2,1 anos (1,2 - 4,8; n = 33). A mediana da idade para atingir o estágio G5 de Tanner foi de 17,1 anos. Conclusões: Os meninos com RCCP iniciaram e concluíram a puberdade espontaneamente por volta dos 15 e 17 anos, respectivamente. A baixa estatura observada na maioria dos pacientes foi transitória e melhorou espontaneamente até o final da puberdade. Os pacientes com RCCP atingiram a estatura alvo, na maioria das vezes, e a estatura adulta prevista pelo método de Bayley-Pinneau superestimou a estatura final.

Background: Most boys with delayed puberty have a constitutional delay of growth and puberty (CDGP). However, some of them have hypogonadism. Differentiating CDGP of hypogonadism is a clinical challenge. Longitudinal follow-up is often the only way to confirm the final diagnosis. Although CDGP is frequent in boys, there are few data in the literature reporting on pubertal evolution and growth and final height in these patients. Objectives: To evaluate the pattern of pubertal development, growth, and final height in male patients with CDGP. Patients and Methods: Retrospective study from a single tertiary care center involving male patients evaluated between 1984 and 2019. Medical charts of 46 boys presenting CDGP were analyzed. Clinical, laboratory, and radiological information was described and analyzed using summary measures. The serial anthropometric assessment was converted into Z-score and analyzed using the Bland & Altman repeated correlation observation coefficient. The height prediction using the target height and the Bayley-Pinneau predicted adult height was performed and compared with the final height through the Bland & Altman method of comparison studies. Results: Most patients were born at term (78.3%) and were adequate for gestational age (84.8%). Familial history of pubertal delay was present in 39.2% of the patients. The median age at the first evaluation was 14.3 years (4.8 - 16.2). Prepubertal short stature was the reason to seek for medical attention in 47.9% of cases. At the first evaluation, the median Z-score for height was -2.5 (-3.4 to 0.8), 93.4% of boys were under -2 SD of height, and 53.4% were under the growth channel. The height Z-score increased during age progression. At the last visit (median age 17.7 years old), the median height Z-score was -1.3 (-2.8 - 0.8), only 17.4% of cases were still under -2 SD of height Z-score, and 4.6% were under the growth channel. For patients whose final height is known (n=23), the final height was similar to the target height (median of 170.5 and 169.6 cm, respectively). At the first evaluation, delayed bone age was observed in 82.6% of the patients. The predicted adult height by the Bayley-Pinneau method overestimated height by a median of 5 cm (-0,9 to 8). The median age of spontaneous pubertal start was 15 years. The median pubertal length was 2.1 years (1.2 - 4.8; n= 33), and the median age to reach Tanner stage G5 was 17.1 years. Conclusion: Boys with CDGP spontaneously started and concluded puberty by 15 and 17 years, respectively. Short stature observed in most patients was transient and improved spontaneously until the end of puberty. The patients with CDGP reached the target height most of the time, and the predicted adult height by the Bayley-Pinneau method usually overestimated the final height.
