Introdução: A tendência mundial de envelhecimento da população evidencia a necessidade de ações para gerir a saúde dos idosos de forma integrada. Um dos aspectos desse cuidado é a abordagem da saúde sexual, o qual é frequentemente negligenciado. Objetivo: Investigar o estado de qualidade, escopo e consistência das diretrizes sobre cuidado não-farmacológico com a sexualidade do idoso com aplicação do instrumento AGREE-II, e utilizar os dados levantados para desenvolver um protocolo para desenvolvimento de uma diretriz sobre o tema adaptada ao contexto brasileiro. Métodos: Foi realizada revisão sistemática com busca nas bases de dados: MEDLINE/PubMed, Embase, LILACS, PsycINFO, Scopus, Web of Science, The Cochrane Library entre outros. Foram incluídos documentos em inglês, espanhol e português, sem restrição em relação ao tempo de publicação, que abordam o cuidado com a sexualidade do idoso, com intervenções não farmacológicas, preferencialmente no cenário da atenção primária à saúde. Foram excluídos trabalhos que discutem idosos com comorbidades específicas ou comprometimento cognitivo. (PROSPERO 2020 CRD42020165135) Resultados: Foram identificados 851 relatos, os quais foram analisados por dois revisores (MCM e YCR) de forma independente. Três trabalhos foram considerados elegíveis para síntese qualitativa, os quais tiveram avaliação global com pontuação acima da média (75%) pela aplicação do instrumento AGREE-II. Foi desenvolvido um protocolo para o desenvolvimento de uma diretriz para o cuidado não-farmacológico da saúde sexual do idoso adaptada ao contexto brasileiro. Conclusão: Considerando o baixo número de estudos elegíveis encontrados, é preciso fomentar a discussão dessa questão no meio científico, apoiar iniciativas como a deste trabalho que iniciou a construção de uma diretriz específica, adaptada ao contexto brasileiro, com a perspectiva do profissional de saúde e da pessoa idosa, e, por meio de trabalhos futuros, finalizar e implementar diretrizes de prática clínica para essa questão.

Introduction: The global trend of an aging population highlights the need for actions to manage the health of the elderly in an integrated manner. One aspect of this care is the sexual health approach, which is often overlooked. Objective: To investigate the state of quality, scope and consistency of the guidelines on non-pharmacological care for the sexuality of the elderly with the application of the AGREE-II instrument, and to use the data collected to develop a protocol for the development of a guideline on the theme adapted to the Brazilian context. Methods: A systematic review was carried out with a search in the databases: MEDLINE / PubMed, Embase, LILACS, PsycINFO, Scopus, Web of Science, The Cochrane Library, among others. Documents in English, Spanish and Portuguese were included, with no restriction on the time of publication, which address care for the sexuality of the elderly, with non-pharmacological interventions, preferably in the setting of primary health care. Studies that discuss elderly people with specific comorbidities or cognitive impairment were excluded. (PROSPERO 2020 CRD42020165135) Results: 851 reports were identified, which were analyzed by two reviewers (MCM and YCR) independently. Three papers were considered eligible for qualitative synthesis, which had an overall assessment with a score above the average (75%) by applying the AGREE-II instrument. A protocol was developed for the development of a guideline for the non-pharmacological care of sexual health of the elderly adapted to the Brazilian context. Conclusion: Considering the low number of eligible studies found, it is necessary to encourage the discussion of this issue in the scientific community, to support initiatives such as this work that started the construction of a specific guideline, adapted to the Brazilian context, with the perspective of the health professional and of the elderly person, and, through future work, finalize and implement clinical practice guidelines for this issue.
