O diagnóstico da migrânea ainda é desafiador em virtude da ampla variedade de apresentação clínica. Muitos pacientes com migrânea apresentam queixas inespecíficas relacionadas ao sistema vestibular, como desequilíbrio e instabilidade, com intensidades variadas, e que podem estar relacionadas à migranea propriamente dita ou sobreposta com outra condição clínica. Além dos aspectos otoneurológicos, aspectos funcionais e emocionais também merecem atenção nesses pacientes, e a compreensão dessa relação ainda é pouco elucidada. Nesse projeto comparamos o grau de incapacidade, os aspectos emocionais e a função vestibular de pacientes com migrânea a um grupo controle, e avaliamos a relação destas repercussões funcionais com o tipo e cronicidade da migrânea. Foram selecionadas mulheres entre 18 e 55 anos diagnosticadas em migrânea sem aura (n=30), migrânea com aura (n=30), migrânea crônica (n=30) e controle (n=30). A incapacidade relacionada aos sintomas vestibulares foi avaliada através do Dizziness Handcap Inventory (DHI); o escore de depressão pelo Personal Health Questionnaire Depression Scale (PHQ-9); e a avaliação vestibular pela eletronistagmografia e prova rotatória pendular decrescente. Os dados sobre sintomas e os testes vestibulares foram analisados através de teste exato de Fisher. Os resultados mostraram que os migranosos apresentam mais tontura que o grupo controle, sendo mais prevalentes episódios de tontura não rotatória e de curta duração. A presença de foto e fonofobia durante os episódios de tontura foram mais prevalentes na presença de aura, seguido do grupo migrânea crônica e sem aura. Os resultados do DHI e do PHQ-9 indicaram impacto da tontura nos pacientes migranosos no âmbito físico e funcional, sendo os sintomas depressivos relacionados com a cefaleia. A avaliação da função vestibular evidenciou alterações de etiologia mista nos testes dos grupos com migrânea crônica e com aura, enquanto o grupo sem aura apresentou mais alterações periféricas; porém sem diferença da função vestibular entre os grupos estudados. Dessa forma observamos que o exame de eletronistagmografia com prova rotatória não auxilia na avaliação dos sintomas vestibulares dos pacientes com migrânea. Porém tais sintomas devem ser considerados relevantes, necessitando de abordagem multidisciplinar.

The diagnosis of migraine is still challenging due to the wide variety of clinical presentations. Many migraine patients have nonspecific complaints related to the vestibular system, such as imbalance and instability, with varying intensities, which may be related to the migraine itself or superimposed on another clinical condition. In addition to otoneurological aspects, functional and emotional aspects also deserve attention in these patients, and the understanding of this relationship is still poorly understood. In this project, we compared the intensity of disability, emotional aspects and vestibular function of patients with migraine to a control group, and evaluated the relationship of these functional repercussions with the type and chronicity of migraine. Women between 18 and 55 years old diagnosed with migraine without aura (n=30), migraine with aura (n=30), chronic migraine (n=30) and control (n=30) were selected. Disability related to vestibular symptoms was assessed using the Dizziness Handcap Inventory (DHI); the depression score by the Personal Health Questionnaire Depression Scale (PHQ-9); and vestibular evaluation by electronystagmography and decreasing rotational pendulum test. Data on symptoms and vestibular tests were analyzed using Fisher's exact test. The results revealed that migranosus presented more dizziness than the control group, with episodes of non-rotating and short-term dizziness being more prevalent. The presence of photo and phonophobia during episodes of dizziness were more prevalent in the presence of aura, followed by the chronic migraine group and without aura. The DHI and PHQ-9 results indicated an impact of dizziness in migraine patients in the physical and functional scope, with depressive symptoms related to headache. The evaluation of vestibular function showed alterations of mixed etiology in the tests of the groups with chronic migraine and with aura, while the group without aura presented more peripheral alterations; however, there was no difference in vestibular function between the studied groups. Thus, we observed that the electronystagmography exam with rotational test does not help in the evaluation of vestibular symptoms in patients with migraine. However, such symptoms should be considered relevant, requiring a multidisciplinary approach.
