Introdução: O distúrbio da percepção de verticalidade visual (VV) atinge mais da metade dos pacientes após o Acidente Vascular Cerebral (AVC) e ainda não apresenta na literatura tratamento eficaz. Em estudos anteriores conseguimos demonstrar que a percepção de verticalidade visual pode ser manipulada pela estimulação transcraniana não invasiva de alta definição HD-ETCC na junção temporoparietal (JTP) em adultos saudáveis e pacientes com AVC, validando ser uma opção promissora de tratamento para essa população, necessitando ainda de estudos para a elaboração de protocolos eficientes. Objetivo: Comparar e avaliar a manipulação da VV com HD-ETCC na JTP no hemisfério direito e esquerdo com 2mA durante 20 minutos, e avaliar o efeito prolongado da estimulação por HD-ETCC na VV. Métodos: Estudo clínico duplo-cego controlado, no qual 40 indivíduos saudáveis receberam HD-ETCC 3x1 com centro catódico de 2mA por 20 minutos, na JTP direita e esquerda randomizados aleatoriamente. A avaliação da VV foi realizada com um sistema de realidade virtual, antes, durante e no período de 60 minutos após a estimulação. Resultados: A estimulação pelo HD-ETCC tanto no hemisfério direito como no hemisfério esquerdo apresentou alterações significantes na VV durante e após a estimulação. A estimulação na JTP do hemisfério direito apresentou efeitos maiores e mais duradouros que a estimulação na JTP do hemisfério esquerdo. Os resultados médios após a estimulação da JTP direita e esquerda foram estatisticamente (p <2e-16) e clinicamente diferentes. Conclusão: A estimulação HDETCC catódica sobre a JTP direita e esquerda induz a alteração da percepção de verticalidade visual para o lado contralateral ao estimulado. Os achados deste estudo apontam para a dominância do hemisfério cerebral direito para a VV em humanos. Os efeitos duradouros e relevantes propõem que o nosso protocolo de estimulação com HD-ETCC na JTP pode ser uma boa estratégia de tratamento para distúrbios da VV.

Background: Visual verticality (VV) disorder affects more than half of patients after stroke and has no present robust treatment. High-definition transcranial direct current stimulation (HD-tDCS) is a promising treatment option, but effects have not been fully explored on VV perception. We previously demonstrated that targeting the temporoparietal junction (TPJ) can manipulate VV perception using either bipolar conventional tDCS and right-hemisphere HD-tDCS. Objective: To verify whether our HD-tDCS montage promotes different effects on VV when applied over the right or the left TPJ. Also, to investigate the retention of VV effects of HD-tDCS applied over the right and the left TPJ. Methods: In a double-blind controlled clinical trial design, forty healthy right-handed young adults received cathode center HD-tDCS, at a current intensity of 2mA for 20 minutes, over the right and the left TPJ, in a randomized order. VV was assessed using the virtual reality system before, during, and up to 60 minutes after stimulation. Results: The right and left HD-tDCS conditions significantly altered online and offline VV perception away from the stimulation side. The average results after both left and right TPJ stimulation and their temporal dynamics were statistically (p<2e-16) and clinically different. The right TPJ stimulation resulted in higher and longer-lasting effects than the left TPJ stimulation. Conclusion: Cathode-center HD-tDCS over either the right or the left TPJ induces VV error away from the stimulation side. Moreover, the present study endorses the dominance of the right hemisphere for VV in humans. The consistent, clinically significant, and long-lasting results suggest that our stimulation protocol is a promising treatment strategy for VV disorders.
