Objetivo: o propósito desse estudo foi avaliar as técnicas endovasculares empregadas, eficácia e a taxa de complicações no tratamento de aneurismas da artéria cerebral média (ACM). Métodos: esse estudo de coorte, incluiu 219 pacientes portadores de 240 aneurismas de ACM tratados consecutivamente. Distribuíram-se os aneurismas em duas topografias (ACM proximal e bifurcação) e quatro técnicas com dispositivos distintos utilizados na abordagem. Os desfechos incluíram a taxa de oclusão dos aneurismas, avaliada por métodos padronizados de imagem, imediatamente, 6 meses e 18 meses ou mais tardiamente após o tratamento. Avaliando-se também performance clínica dos pacientes e complicações ocorridas. Resultados: os aneurismas se localizavam em sua maioria na bifurcação da ACM (82,08%), seguidos pelos de ACM proximal (17,92%). As técnicas empregadas incluíram: técnica simples (7,11%), remodelamento por balão (28,7%), remodelamento por stent (46,44%) e reconstrução luminal com Stent diversor de fluxo (SDF) (17,57%). Obteve-se oclusão completa em 181 de 230 aneurismas após 6 meses (78,7% [95% IC 69,73,4; 83,99%]) e em 174 de 226 aneurismas após 18 meses (77% [95% IC 71,5; 82,48%]). A oclusão total foi mais frequente no grupo com uso de SDF (87,5%). A taxa de oclusão satisfatória (Raymond A+B) alcançou 95,13% [95% IC 92,33; 97,94%]). Os desfechos angiográficos parciais e a piora na performance funcional foram influenciados estatisticamente pelos maiores diâmetros dos aneurismas (valor-p < 0,005). Houve 34 complicações relatadas, com registro de seis óbitos e dois casos com sequelas neurológicas permanentes (mRs > 2), sendo mais frequentes nas técnicas com uso de stents. A taxa de morbimortalidade foi de 3,6%. Conclusão: o tratamento endovascular dos aneurismas de artéria cerebral média localizados na bifurcação e proximais apresenta baixa morbimortalidade, com taxas de oclusão elevadas e reprodutível no cenário nacional por diferentes técnicas.

Objective: The purpose of this study was to evaluate the effectiveness and the rate of complications of the different endovascular techniques for the of middle cerebral artery (MCA) aneurysms. Methods: this cohort study included 219 patients with 240 MCA aneurysms treated consecutively. We evaluated parameters such as aneurysm topographies, treatment techniques and devices used in the approach. The main outcome was rate of aneurysm occlusion, as assessed by standardized imaging methods, immediately, at 6 months, and 18 months or later after treatment. We also evaluated the patient clinical performance and treatment-associated complications. Results: Most aneurysms were located in the bifurcation of the middle cerebral artery (82.08%), followed by proximal MCA (17.92%). The treatment techniques included: simple coiling (7.11%), balloon remodeling (28.7%), stent remodeling (46.44%) and luminal reconstruction with a flow-diverting stent (FDS) (17.57%). Complete occlusion was achieved in 181 of 230 aneurysms after 6 months (78.7% [95% CI 69.73.4; 83.99%]) and in 174 of 226 aneurysms after 18 months (77% [95% CI 71.5; 82.48%]). Total occlusion was more frequent in the SDF group (87.5%). The satisfactory occlusion rate (Raymond A+B) reached 95.13% [95% CI 92.33; 97.94%]). Partial angiographic outcomes and worsening of functional performance were statistically influenced by larger aneurysm diameters (p value < 0.005). There were 34 reported complications, including six deaths and two patients with permanent neurological sequelae (mRs > 2), more frequent when stents were employed. The morbidity and mortality rate were 3.6%. Conclusion: the endovascular treatment of middle cerebral artery aneurysms both located in the bifurcation and proximal has low morbidity and mortality with high occlusion rates and is reproducible in the national scenario by different techniques.
