A estenose carotídea é uma das principais causa de AVC isquêmico, sendo etiologia de uma das principais causas de mortalidade e incapacidade no mundo. Todos pacientes com manifestação clínica devem ser avaliados para estenose carotídea. O rastreio em pacientes assintomáticos não é recomendado. Pacientes com estenose grave ou estenose moderada sintomática tem elevado risco de AVC isquêmico em sua evolução natural, requerendo avaliação especializada para considerar revascularização carotídea. Os principais métodos diagnósticos são: arteriografia, ressonância magnética, tomografia computadorizada e ultrassonografia. O conhecimento da história natural e das evidências científicas atuais é imprescindível para escolha do melhor tratamento. O presente estudo serve como base para estabelecer conceitos básicos acerca da doença aterosclerótica carotídea, concluindo que o tratamento endovascular é seguro e eficaz para casos de estenose carotídea sintomática e assintomática.

Carotid stenosis is one of the main causes of ischemic stroke, being the etiology of one of the main causes of mortality and disability in the world. All patients with clinical manifestations should be evaluated for carotid stenosis. Screening in asymptomatic patients is not recommended. Patients with severe stenosis or symptomatic moderate stenosis are at high risk of ischemic stroke in its natural course, requiring specialized evaluation to consider carotid revascularization. The main diagnostic methods are: arteriography, magnetic resonance imaging, computed tomography and ultrasound. Knowledge of natural history and current scientific evidence is essential for choosing the best treatment. The present study serves as a basis for establishing basic concepts about carotid atherosclerotic disease, concluding that endovascular treatment is safe and effective for both symptomatic and asymptomatic carotid stenosis.
