Introdução: O transtorno de déficit de atenção e hiperatividade (TDAH) é o transtorno do neurodesenvolvimento mais comum na prática clínica e é frequente a ocorrência de comorbidades, como transtornos de comunicação e de aprendizagem. Assim como a atenção e as funções executivas são primordiais como funções corticais superiores e as disfunções executivas já são bem reconhecidas no transtorno, é notória a interface dessas com a linguagem. Quanto à metalinguagem, sob a perspectiva psicolinguística e tais alterações no TDAH, foram encontrados poucos estudos na literatura avaliando múltiplas habilidades linguísticas em uma única amostra. Objetivos: Caracterizar as habilidades linguísticas: consciência fonológica (fonêmica e silábica), consciência sintática, semântica e pragmática em uma amostra de pacientes com TDAH atendidos ambulatorialmente no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo (HCFMRP-USP) e comparar as características das habilidades linguísticas nas diferentes apresentações de TDAH. Material e Métodos: Estudo realizado de forma retrospectiva, por meio de análise de prontuários e banco de dados de pacientes acompanhados no Ambulatório de Distúrbios do Comportamento e Aprendizado do HCFMRP-USP entre os anos de 2015 e 2019. Foram coletados dados linguísticos a partir de provas fonoaudiológicas e neuropsicológicas disponíveis. Resultados: Foram analisados 143 prontuários, desses, após avaliação multiprofissional completa, 46 pacientes apresentavam diagnóstico bem fundamentado de TDAH. Seguindo os critérios de inclusão e exclusão, a amostra compreendeu 26 pacientes. Quatorze (53,8%) foram classificados com TDAH predomínio desatento e 12 (43,2%) com TDAH combinado. As habilidades linguísticas estiveram alteradas em 22 pacientes, o que corresponde a 84,6% da amostra; 63,3% apresentaram mais de uma habilidade linguística alterada. Quanto à consciência fonológica, nove pacientes apresentaram resultados alterados (34,6%) na consciência silábica e 11 (42,3%) na consciência fonêmica. Na consciência sintática foram 10 (38,5%). Quanto ao vocabulário e à compreensão, houve alterações em 12 (46,2%) e em 10 (38,5%), respectivamente. Quanto ao QIT (Coeficiente intelectual total), 13 pacientes (50%) se enquadraram entre média inferior e limítrofe. O ICV (Índice de compreensão verbal) esteve alterado em 15 pacientes (57,7%) e o IMO (Índice de memória operacional) e IVP (Índice de velocidade de processamento) igualmente em 15 (57,7%). Houve associação mais pronunciada entre o IMO e o QIT nos pacientes do grupo TDAH predomínio desatento. Os dados das provas atencionais estiveram alterados em 11 pacientes (42,3%). Em relação às demais funções executivas, foram observadas alterações em 17 pacientes (65,4%). Quanto aos dados linguísticos coletados, não houve diferença estatística entre os dois grupos: TDAH predomínio desatento e combinado. Conclusão: Concluiu-se que as habilidades linguísticas estiveram alteradas em 84,6% da amostra de pacientes com TDAH, sendo possível observar em maior frequência alterações no vocabulário, seguido de alterações em consciência fonêmica, após compreensão e consciência sintática; e por último, consciência silábica. O presente estudo se faz relevante devido ao número expressivo de alterações encontradas e por avaliar as habilidades linguísticas globais em uma única amostra. O tamanho da amostra e a ausência de um grupo de pacientes com apresentação predominantemente hiperativa/impulsiva podem ser apontados como possíveis explicações da ausência de significado estatístico e apresentam-se como as principais limitações do estudo.

Background: Attention Deficit Hyperactivity Disorder (ADHD) is the most common neurodevelopment disorder in clinical practice, being close to other comorbidities such as communication and learning disorders. Attention and executive functions are paramount as higher cortical functions, and executive dysfunctions are already well recognized in ADHD, participating in its interface with language. Few studies evaluate multiple language skills of metalanguage changes in ADHD from the psycholinguistic perspective. Aim: To characterize the language skills: phonological awareness (phonemic and syllabic), syntactic, semantic, and pragmatic awareness in ADHD patients managed in outpatient method and compare features of language skills in ADHD specter presentation. Material and Methods: Retrospective study, through analysis of medical records and database of patients, followed up at the outpatient clinic for Behavioral and Learning Disorders of the Hospital das Clínicas, Ribeirão Preto Medical School at the University of São Paulo (HCFMRP-USP) between 2015 to 2019. We evaluate the linguistic data from available speech-language pathology and neuropsychological records. Results: We analyzed 143 medical records. After a complete multidisciplinary evaluation, 46 patients had a well-founded diagnosis of ADHD. Following the inclusion and exclusion criteria, the sample comprised 26 patients. Fourteen patients (53.8%) were ADHD inattentive, and 12 (43.2%) were a combined type. Language skills were abnormal in 22 patients (84.6%); 63.3% had more than one altered language skill. Phonological awareness: nine patients had abnormal results (34.6%) for syllabic awareness and 11 (42.3%) for phonemic awareness. Syntactic awareness presented 10 (38.5%) abnormal results. As for vocabulary and comprehension, there were changes in 12 (46.2%) and 10 (38.5%), respectively. 13 patients (50%) were between lower mean and borderline intellectual quotient (QI). 15 Patients (57.7%) had abnormal verbal comprehension index, while 15 (57.7%) also presented abnormal results for operating memory index (OMI) and processing speed. There was a tendency for an association between OMI and QI in patients in the ADHD predominant inattentive group. Eleven patients (42.3%) presented abnormal attentional records. Seventeen patients (65.4%) had other abnormalities of executive functions. There was no statistical difference between the two groups: ADHD inattentive and combined types. Conclusions: We concluded that language skills were abnormal in 84.6% of the patients, with changes in vocabulary being more frequent, followed by changes in phonemic awareness, comprehension and syntactic awareness, and syllabic awareness. Our study is relevant due to the expressive number of alterations observed and for evaluating global language skills in a single sample. The sample size and the absence of a group of patients with hyperactive/impulsive type could explain the lack of statistical significance and are the limitations of this study.
