A doença aterosclerótica intracraniana configura-se como uma das principais causas de acidente vascular cerebral, e apesar de reconhecidamente importante dentre os asiáticos é presente em todos os grupos étnicos, sobretudo em pacientes com idade avançada, sendo um importante problema de saude pública. O diagnostico pode ser realizado por métodos de imagem, dentre eles o a ultrassonografia transcraniana com Doppler, a angiotomografia e angioressonância e a angiografia por subtração digital. A partir do diagnostico o desafio torna-se então o manejo da doença na tentativa de evitar a ocorrência de eventos isquêmicos cerebrais, sendo atualmente o tratamento baseado em mudanças do estilo de vida, com controle de fatores de risco como hipertensão arterial, diabetes mellitus, sedentarismo e cessação do tabagismo, uso de estatizas de alta potencia e antiagregação plaquetária, diretrizes formuladas com embasamento em diversos ensaios clínicos, mas mesmo assim o risco de eventos cerebrovasculares recorrentes permanece elevado, sendo essencial o desenvolvimento de novas estratégias de estratificação e tratamento. É nesse cenário que despontam novos métodos de diagnostico, como protocolos de ressonância magnética focados no estudo de características da placa ateromatosa e da repercussão hemodinâmica e o retorno da terapia endovascular, respaldada em novos dispositivos mais modernos e projetados para o microambiente da circulação intracraniana.

The intracranial atherosclerotic disease is one of the main causes of stroke, and although particularly prevalent among Asians, it is present over all ethnic groups, especially in elderly patients, being an important public health problem. The diagnosis can be done using different imaging methods, as transcranial Doppler ultrasound, Angiotomography and angioMRI and digital subtraction angiography. After the diagnosis, the challenge is the management of the disease, an attempt to prevent the occurrence of cerebral ischemic events. Treatment is currently based on lifestyle changes, with the control of several risk factors such as arterial hypertension, diabetes mellitus, physical inactivity and smoking cessation, the use of high-power statins and antiplatelet aggregation, those guidelines were formulated based on several clinical trials, but even so the risk of recurrent cerebrovascular events remains high, and the development of new stratification and treatment strategies is essential. It is in this scenario that new diagnostic methods emerge, especially magnetic resonance protocols focused on the study of atheromatous plaque characteristics and its hemodynamic repercussions and the return of endovascular therapy, supported by modern devices designed for the intra-cranial circulation micro-environment.
