A determinação do grau de hemólise é um importante indicador utilizado para assegurar a qualidade de armazenamento dos concentrados de hemácias (CH), sendo um dos parâmetros avaliados nas análises de controle de qualidade de hemocomponentes. A hemólise em unidades de CH pode ser causada por diversos fatores, tais como: procedimentos inadequados durante a coleta eo processamento do sangue, condições impróprias de armazenamento, contaminação bacteriana, algumas drogas e outros fatores anormais no sangue do doador. Apesar da existência de diretrizes nacionais e internacionais que determinam o grau de hemólise aceitável nos CH, não há padronização em relação ao método ideal para quantificar o percentual de hemólise. Para determinação do grau de hemólise em CH são necessárias a análise da hemoglobina total, hemoglobina plasmática e hematócrito. Esses três testes podem ser mensurados por diversas técnicas. O objetivo deste trabalho foi avaliar a concordância entre três métodos para determinação da hemoglobina plasmática (Drabkin, Espectrofotométrico Ótico e Azida Metahemoglobina HemoCue® Plasma/Low Hb), dois métodos para dosagem do hematócrito (microhematócrito e analisador hematológico Cell-Dyn Ruby) e dois métodos para determinação da hemoglobina total (azida metahemoglobina HemoControl e analisador hematológico Cell-Dyn Ruby). Foram analisadas 59 bolsas de CH produzidas pelo Hemocentro de Belo Horizonte, Minas Gerais. As bolsas foram divididas em dois grupos amostrais, um composto de CH CPDA1 e outro CH CPD SAG manitol. Os testes foram realizados em todas as bolsas em três momentos do armazenamento. As análises foram feitas utilizando gráficos de Bland-Altman. Nas análises de hemoglobina plasmática foi observada uma boa concordância entre os métodos de Drabkin, Espectrofotométrico e HemoCue Plasma Low nas bolsas CPD/SAG manitol em todos os períodos do armazenamento, porém, nas bolsas CPDA1, ao final da validade, a mesma não foi verificada. Nessas bolsas a média da diferença entre os métodos Drabkin e Espectrofotométrico, assim como os limites de concordância foram muito elevados (média 0,42g/dL e limites entre -0,02 e -0,85 g/dL), indicando uma pior concordância entre os métodos nesse momento do armazenamento. Nas análises de hemoglobina total foi observada boa concordância entre o analisador hematológico Cell-Dyn Ruby e o hemoglobinômetro HemoControl EKF nos dois grupos de bolsas analisadas e em todos os períodos do armazenamento. Os métodos de microhematócrito e analisador hematológico Cell-Dyn Ruby foram concordantes para as análises do hematócrito, tanto em CH CPDA1 como em CH CPD/SAG manitol. A escolha do método de determinação de hemoglobina livre tem papel fundamental na determinação de resultados de hemólise, podendo gerar um percentual maior de não conformidades nas análises de controle de qualidade e aumento no número de descarte de bolsas por esse motivo, principalmente em CH CPDA1. Nesse trabalho, foi possível constatar que ao final do armazenamento, nas bolsas CPDA1, o método espectrofotométrico subestimou resultados de hemólise, principalmente nos hemocomponentes com altas concentrações de hemoglobina livre. Os métodos de Drabkin e HemoCue Plasma Low apresentaram uma melhor concordância na determinação da hemólise dessas bolsas ao final da validade.

The determination of the level of hemolysis is an important indicator that is used to ensure the quality of the storage of red blood cell (RBC). It is one of the parameters evaluated in the analyses of the quality control of blood components. The hemolysis in the units of red blood cell (RBC) can be caused by several factors, for example: inappropriate procedures during the blood processing, inappropriate conditions of storage, bacterial contamination, some drugs and another abnormal factors in the blood donor. Despite the existence of national and international guidelines that determinate the acceptable level of hemolysis in the red blood cells concentrate, there is no standardization in relation to the ideal method to quantify the percentage of the hemolysis. To determine the acceptable level of hemolysis in the red blood cells concentrate, the analysis of total hemoglobin, free hemoglobin and hematocrit is necessary. These three tests can be measured by several techniques. The aim of the presented study was to evaluate the concordance between the three methods to determinate free hemoglobin (Drabkin, Optical spectrophotometric e Azide Metehemoglobin HemoCue® Plasma/Low Hb), two method to hematocrit dosage, (microhematocrit e hematology analyser Cell Dyn Ruby) and two method to determination of hemoglobin (azide metehemoglobin HemoControl e hematology analyser Cell Dyn Ruby). For this purpose Bland-Altman graphics were used. 59 bags of red blood cell that was produced by Blood Center of Belo Horizonte (Hemominas), Minas Gerais were analyzed. The bags were divided in two sample groups, one compound of RBC CPDA1 and other RBC CPD SAG mannitol. The tests were accomplished in all bags in three moments of the storage. In the analysis of free hemoglobin a good agreement between Drabkin methods, spectrophotometric and HemoCue Plasma Low in bags CPD/SAG mannitol was observed in all periods of storage. Although, in the bags of CPDA1, in the end of validity, the same was not verified. In these bags the average of the difference between the methods of Drabkin and spectrophotometric, as well as the limits of agreement were very high (average 0,42g/dL and limits between -0,02 e -0,85 g/dL), that indicates worse agreement between the methods in this moment of the storage. In the analysis of the total hemoglobin a good agreement between the hematology analyzer Cell Dyn Ruby and the hemoglobinometer HemoControl EKF was observed in both groups of analysed bags and in all periods of the storage. The method of microhematocrit and hematology analyzer Cell Dyn Ruby were consistent for the analyses of the hematocrit, as in RBC CPDA1 as in RBC CPD/SAG mannitol. The choice of the determination of method of the free hemoglobin has a fundamental role in the detemination of secure and reliable hemolysis results, that may genarete a bigger percentage of non-compliance in the analysis of quality control and an increase in the number of discard bags, for this reason, mainly in RBC CPDA1. In this study, it was possible to observe that in the end of the storage, in CPDA1 bags, the spectrophotometric method underestimated the results of the hemolysis, mainly in hemocomponents with high concentration of the free hemoglobin. The methods of Drabkin e HemoCue Plasma Low presented a better agreement for the measure hemolysis of the bags in the end of vality.
