O transplante alogênico de células tronco hematopoiéticas (TCTH) é um tratamento com potencial curativo para doenças oncohematológicas, falências medulares e doenças autoimunes. As complicações bucais e sistêmicas mais comuns durante a fase inicial do transplante são mucosite, infecção, febre e bacteremia. O objetivo desse trabalho foi avaliar a condição de saúde bucal no pré-TCTH como fator de risco para complicações bucais e sistêmicas durante o período pré-enxertia neutrofílica e para o desenvolvimento de Doença do Enxerto Contra Hospedeiro aguda (DECHa) e óbito até o D+100, em pacientes submetidos ao primeiro TCTH alogênico. Para isso, foi realizado estudo de coorte transversal prospectivo observacional, em pacientes maiores de 12 anos de idade. As variáveis de saúde bucal: higiene oral, perda óssea periodontal, foco infeccioso bucal e o índice de dentes cariados, perdidos e obturados (CPOD) no pré-TCTH foram tratadas como exposição. Idade, sexo, doença de base, compatibilidade do transplante, tipo de condicionamento e a realização de irradiação corporal total (ICT), foram tratados como variáveis de confusão. Os desfechos foram: incidência, grau e duração da mucosite oral (MO), infecção bucal, febre e hemocultura positiva na fase pré-enxertia. Além do desenvolvimento de DECHa e óbito até 100 dias após TCTH. O Risco Relativo (RR) e o Risco Relativo ajustado (modelo múltiplo) foram calculados utilizando o modelo de regressão de Poisson com variância robusta. Foram incluídos 81 pacientes, mediana de 26 anos de idade; 54,3% do sexo masculino e 45,7% do sexo feminino; 48,1% doença de base benigna; 82,7% compatível e 17,3% não compatível; 60,5% dos transplantes envolveram regime de condicionamento de intensidade reduzida ou não-mieloablativo e 39,5% condicionamento mieloablativo; 24,7% realizaram ICT. Na avaliação de saúde bucal, 44,4% dos pacientes apresentaram higiene bucal insatisfatória, 14,8% perda óssea periodontal e 30,9% possuíam foco infeccioso em cavidade bucal; a média geral do CPOD foi de 8,3. A incidência global de MO foi de 58%, e 28,4% dos pacientes apresentou infecção em boca durante o período pré-enxertia. 71,6% desenvolveram febre; 34,6% hemocultura positiva, 22,2% DECHa e 14,8% morreram até o D+100. Nenhuma condição bucal foi fator de risco para MO; a presença de foco infeccioso em boca no pré-TCTH aumentou em 6,86 vezes (IC:2,30-20,47, p=0,00) o risco de infecção em cavidade bucal. A higiene bucal insatisfatória aumentou em 2 vezes o risco de febre (IC:1,45-2,80, p=0,00). A perda óssea periodontal apresentou forte tendência de risco (RR:5,44; IC:0,99-29,8; p=0,05) para o desenvolvimento de DECHa. O modelo simples mostrou a higiene bucal insatisfatória (RR:13,13), a perda óssea periodontal (RR: 2,03) e maior CPOD (RR:1,05), como risco para infecção bucal. Além de apontar a presença de foco infeccioso em boca (RR:1,37) como risco para desenvolvimento de febre. Nenhuma das condições de saúde bucal foi risco para hemocultura positiva e óbito. Podemos concluir que a resolução de focos infecciosos e a otimização da higiene bucal tem potencial papel na redução de infecção em cavidade bucal e na ocorrência de febre, durante o período do condicionamento até a enxertia neutrofílica. A relação entre a atividade de doença periodontal e DECHa deve ser investigada.

Oral health condition as a risk factor for early complications in allogeneic hematopoietic stem cell transplantation

Allogeneic hematopoietic stem cell transplantation (HSCT) is a potentially curative treatment for hematological and autoimmune diseases. The most common oral and systemic complications during the early phase of HSCT are mucositis, infection, fever and bacteremia. The aim of this study was to evaluate pre-HSCT oral health condition as a risk factor for oral and systemic complications until neutrophilic engraftment and for the development of acute graft versus host disease (aGVHD) and death before D+100 in patients undergoing their first allogeneic HSCT. This prospective observational cross-sectional study included patients older than 12 years of age. The oral health variables: oral hygiene; periodontal bone loss; oral infectious focus; and the index of decayed, missing and filled teeth (DMFT) pre-HSCT were treated as exposure. Age, gender, type of disease, type of transplant, type of conditioning and use of total body irradiation (TBI) were treated as confounding variables. The outcomes were: incidence, degree, and duration of oral mucositis (OM), oral infection, fever and positive blood cultures in the pre-engraftment phase. The development of aGVHD and death within 100 days after HSCT were also outcomes in this study. The relative risk (RR), and the adjusted relative risk (multiple model) were calculated using the Poisson regression model with robust variance. In total, 81 patients were included, with a median age of 26 years old; 54.3% male and 45.7% female; 48.1% with benign disease; 82.7% undergoing match-HSCT and 17.3% mismatch; 60.5% of transplants involved a reduced intensity or non-myeloablative conditioning regimen, 39.5% used myeloablative conditioning, and 24.7% of patients were submitted to TBI. In the oral health assessment, 44.4% of the patients had poor oral hygiene, 14.8% had periodontal bone loss and 30.9% had at least one infectious focus in the oral cavity; the overall average of DMFT was 8.3. The overall incidence of OM was 58% and 28.4% of patients developed oral infection during the pre-engraftment period. Overall, 71.6% of patients had fever; 34.6% had positive blood cultures, 22.2% developed aGVHD and 14.8% had died at D+100. Oral condition was not a risk factor for OM; the presence of an infectious focus in the oral cavity before HSCT increased the risk of oral infection 6.86-fold (CI: 2.30-20.47, p = 0.00). Poor oral hygiene increased the risk of fever occurrence (CI: 1.45-2.80, p = 0.00). Periodontal bone loss showed a strong risk trend (RR: 5.44; CI: 0.99-29.8; p = 0.05) for the development of aGVHD. The simple risk model showed poor oral hygiene (RR: 13.13), periodontal bone loss (RR: 2.03) and higher DMFT (RR: 1.05) as risk factors for oral infection. In addition, the presence of an infectious focus in the oral cavity (RR: 1.37) was a risk factor for fever. Oral health condition was not a risk factor for positive blood cultures and death. We can conclude that the treatment of infectious foci and the optimization of oral hygiene have a potential role in reducing infection in the oral cavity and the occurrence of fever during the period between conditioning and the neutrophilic graft. The relationship between periodontal disease activity and aGVHD should be investigated.
