Introdução: Os Inibidores da Apoptose (IAPs) podem interferir no processo inflamatório crônico, e auxiliar na formação da Rinossinusite Crônica com Pólipo (RSCcPN), ou interferir a susceptibilidade dos pólipos nasais ao tratamento clínico com corticoide tópico nasal; todavia estudos sobre o tema ainda são escassos na literatura. Objetivos: comparar a expressão dos IAPs entre pacientes com RSCcPN e indivíduos livres de doença, comparar a expressão dos IAPs entre pacientes com RSCcPN que obtiveram boa resposta ao corticoide tópico nasal e aqueles com resposta inadequada; e correlacionar a expressão dos IAPs a marcadores inflamatórios (clínicos e laboratoriais) em pacientes com RSCcPN. Pacientes e Métodos: Foram coletados dados clínicos dos pacientes com RSCcPN por meio de questionário de qualidade de vida - SNOT-22, realizadas medidas do escore endoscópico (Lund-Kennedy) e do escore tomográfico (Lund-Mackay). Foram colhidas amostras de pólipo nasal de pacientes com RSCcPN (sem vigência de tratamento clínico) e amostras de concha média dos controles para processamento. A expressão gênica dos IAPs (XIAP, BIRC2/IAP1 e BIRC3/IAP2) e das caspases (CASP3, CASP7, CASP9 e BCL2) foram medidos por qRT-PCR. As dosagens das citocinas pró-inflamatórias (IFN-γ, IL-5, IL-33, IL-10, IL-17 e TGF-β) foram medidas pelo método Luminex. A comparação entre o grupo de pacientes com pólipo nasal e os controles foi realizada por meio de testes paramétricos não-pareados. A análise de Componentes Principais (PCA) foi usada para correlacionar a expressão dos marcadores avaliados com a resposta ao corticoide tópico nasal nos pacientes com RSCcPN. Resultados: Foram encontradas menor expressão dos três genes IAPs e expressão significativamente maior das citocinas INF-γ, IL-5 e TGF-β nesses pacientes em comparação aos livres de doença. Ainda, observou-se que pacientes que responderam mal ao corticoide tópico tinham índices significativamente menor de BIRC2/IAP1 quando comparados aos que obtiveram melhor resposta. Ao se associar a expressão dos marcadores à resposta ao corticoide pelo método PCA, identificou-se que os marcadores BIRC2/IAP1, XIAP, BCL2, CASP9, IL-17 e IL-33 estiveram aumentados em pacientes com melhor resposta clínica, enquanto que CASP7 e TGF-β estiveram relacionados com pior resposta ao tratamento. Conclusões: Evidenciou-se inflamação com padrão misto (T1, T2 e T3) nos pacientes com RSCcPN, quando comparados aos controles. Estes dados sugerem que a diminuição dos IAPs seja um fator importante na fisiopatogenia da RSCcPN e na suscetibilidade ao tratamento clínico. Considerando que os IAPs modificam a cascata inflamatória inata, estes achados reforçam a importância do processo da imunidade inata como um elo essencial entre o ambiente, o eptitélio e a cronificação do processo inflamatório, além de abrir novas persepctivas sobre a importância da barreira epitelial da mucosa nasossinusal na RSCcPN.

Introduction: Inhibitors of Apoptosis (IAPs) can interfere with the chronic inflammatory process, and help in the formation of CRSwNP, or interfere with the susceptibility of nasal polyps to clinical treatment with nasal topical corticosteroids, however studies on the subject are still scarce in the literature. Objectives: to compare the expression of apoptosis inhibitors between patients with CRSwPN and disease-free individuals, compare the expression of apoptosis inhibitors between patients with CRSwPN who had a good response to nasal topical corticosteroids with those who had an inadequate response, and to correlate the expression of the inhibitors from apoptosis to inflammatory markers (clinical and laboratory) in patients with CRSwPN. Patients and Methods: Clinical data from patients with CRSwNP were collected through a quality of life questionnaire - SNOT-22, with measurements of the endoscopic score (Lund-Kennedy) and the tomographic score (Lund-Mackay). Nasal polyp samples were collected from patients with CRSwPN (without clinical treatment) and middle turbinate samples from controls for processing. The gene expression of apoptosis inhibitors (XIAP, BIRC2/IAP1 and BIRC3/IAP2) and caspases (CASP3, CASP7, CASP9 and BCL2) were measured by qRT-PCR. The dosages of pro-inflammatory cytokines (IFN-γ, IL-5, IL-33, IL-10, IL-17 and TGF-β) were measured by the Luminex method. Comparison between the group of patients with nasal polyps and controls was performed using unpaired parametric tests. Principal Component Analysis (PCA was used to correlate the expression of the markers evaluated here with the response to nasal topical corticosteroids in patients with CRSwPN. Results: We found lower expression of the three apoptosis inhibitor genes and significantly higher expression of the cytokines INF-γ, IL-5 and TGF-β in CRSwPN patients compared to disease-free patients. Furthermore, we observed that patients who responded poorly to topical corticosteroids had significantly lower rates of BIRC2/IAP1 when compared to those who responded better. By associating the expression of the markers with the response to steroids using the PCA method, we identified that the markers BIRC2/IAP1, XIAP, BCL2, CASP9, IL-17 and IL-33 were increased in patients with better clinical response, while CASP7 and TGF-β were related to worse response to treatment. Conclusions: Inflammation with a mixed pattern (T1, T2 and T3) was evidenced in patients with CRSwPN, when compared to controls. Our data suggest that the decrease in IAPs is an important factor in the CRSwPN pathophysiology and susceptibility to clinical treatment. Considering that IAPs modify the innate inflammatory cascade, the present findings reinforce the importance of the innate immunity process as an essential link between the environment, the epithelial and the chronification of the inflammatory process, in addition to opening new perspectives on the importance of the epithelial barrier of the inflammatory process nasosinusal mucosa in CRSwPN.
