A Doença Pulmonar Obstrutivo Crônica (DPOC) é uma doença crônica e progressiva que provoca o comprometimento do sistema respiratório, com presença de sintomas clínicos, além de prejuízos na qualidade de vida do indivíduo, como a limitação física, perda de funcionalidade na vida diária e a presença de comorbidades psicológicas. O objetivo geral do estudo é investigar as associações entre as variáveis clínicas da doença e as variáveis psicológicas e psiquiátricas em uma amostra de pacientes com DPOC e compará-los com um grupo controle de sujeitos sem a doença respiratória (GC) e entre os subgrupos de gravidade dos parâmetros espirométricos e sintomatológicos da DPOC (VEF1, CVF, Tosse, Dispnéia, Saturação de O2 e Estadiamento). A amostra total foi composta por 90 participantes, sendo 40 do grupo DPOC e 50 do grupo GC, pareados por idade e sexo. Para a avaliação dos aspectos clínicos foram analisados os dados de exames de Espirometria e Escalas de Dispnéia, Tosse e Expectoração. Para a avaliação psicológica foram aplicados os seguintes instrumentos: Questionário Sobre a Saúde do Paciente PHQ-9 para avaliação de sintomas de depressão; Inventário de Ansiedade Traço-Estado (IDATE-T); Escala de Autoestima de Rosemberg (EAR); Brief Cope e Escala de Vivências Afetivas e Sexuais dos Idosos (EVASI). Os dados foram analisados, por meio de estatística descritiva e de testes de comparação de grupo (x2, t de Student). O nível de significância adotado foi de p=0,05. Os resultados apontaram para associação da DPOC com prejuízos funcionais nos pacientes com DPOC, com maior necessidade de ajuda para atividades domésticas, menor participação social e lazer; presença de comorbidades clínicas (cardiopatia e baixo IMC) e foram observadas presença de diferenças estatisticamente significativas entre os grupos nos indicadores de depressão, ansiedade e baixa autoestima, com maiores médias no grupo clínico, indicando maior gravidade desses sintomas. No que diz respeito às vivências sexuais / afetivas e utilização de recursos de enfrentamento, os grupos não diferiram significativamente entre si. Não foram verificadas diferenças estatisticamente significativas entre os grupos de maior e menor gravidade dos parâmetros espirométricos e sintomatológicos, com exceção da Dispnéia, que teve maior associação com indicadores de depressão e baixa autoestima. Desse modo, os achados corroboram dados da literatura prévia e analisa de forma conjunta múltiplas variáveis psiquiátricas e psicológicas dos pacientes com DPOC, proporcionando uma compreensão de forma integrada do impacto dessa patologia e favorecendo condutas terapêuticas e preventivas.

Chronic Obstructive Pulmonary Disease (COPD) is a chronic and progressive disease that causes impairment of the respiratory system, with the presence of clinical symptoms, as well as impairments in the individual's quality of life, such as physical limitation, loss of functionality in daily life and the presence of psychological comorbidities. The overall aim of the study is to investigate the associations between clinical disease variables and psychological and psychiatric variables in a sample of COPD patients and to compare them with a control group of subjects without respiratory disease (CG) and between subgroups of severity of the spirometric and symptomatic parameters of COPD (VEF1, CVF, Cough, Dyspnea, O2 Saturation, and Grade). The total sample consisted of 90 participants, 40 from the COPD group and 50 from the CG group, matched for age and gender. For the evaluation of clinical aspects, data from spirometry and dyspnea, cough and expectoration scales were analyzed. For the psychological evaluation, the following instruments were applied: PHQ-9 Patient Health Questionnaire to evaluate depression symptoms; State-Trait Anxiety Inventory (STAI-T); Rosemberg Self-Esteem Scale (EAR); Brief Cope and Scale of Affective and Sexual Experiences of the Elderly (EVASI). Data were analyzed using descriptive statistics and group comparison tests (x2, t de Student). The significance level adopted was p = 0.05. The results pointed to the association of COPD with functional impairment in COPD patients, with greater need for help with domestic activities, lower social participation and leisure time; clinical comorbidities (heart disease and low BMI) and statistically significant differences were observed between the groups in the indicators of depression, anxiety and low self-esteem, with higher means in the clinical group, indicating greater severity of these symptoms. Regarding sexual / affective experiences and use of coping resources, the groups did not differ significantly from each other. No statistically significant differences were found between the groups with higher and lower severity of spirometric and symptomatic parameters, except for Dyspnea, which was more associated with depression indicators and low self-esteem. Thus, the findings corroborate data from the previous literature and jointly analyze multiple psychiatric and psychological variables of COPD patients, providing an integrated understanding of the impact of this pathology and favoring therapeutic and preventive approaches.
