Introdução: Obesidade é uma doença crônica de alta prevalência. Está associada ao desenvolvimento de outras morbidades metabólicas, como a síndrome metabólica. Tem etiologia complexa e múltiplos fatores de risco. Nas mulheres, encontra, provavelmente no antecedente obstétrico alguns fatores desencadeadores. Objetivos: Identificar os fatores de risco sociodemográficos e reprodutivos associados à obesidade e à síndrome metabólica nas mulheres na quarta década de vida. Métodos: Trata-se de um estudo de coorte, conduzido de 1978 a 1979 em Ribeirão Preto, Brasil. Foram coletados dados sociodemográficos, clínicos, obstétricos, antropométricos e de composição corporal através de entrevista e avaliação clínica. Modelos de regressão log binomial univariado e multivariado foram construídos para identificar os preditores de obesidade e síndrome metabólica em mulheres, estimando-se o risco relativo ajustado. Resultados: A coorte incluiu 916 mulheres: 309 (33,7%) eram obesas e 286 (31,2%) apresentavam síndrome metabólica. Obesidade estava associada a baixa escolaridade (RR 1,77, IC95% 1,33-2,35) e primeira gravidez na adolescência (RR 1,46, IC95% 1,10-1,93). Síndrome metabólica estava associada a não possuir trabalho remunerado (RR 1,49, IC95% 1,14-1,95), não possuir companheiro (RR 1,33, IC95% 1,03-1,72), baixa escolaridade (RR 1,72, IC95% 1,23-2,41) e primeira gravidez na adolescência (RR 2,00, IC95% 1,45-2,77). Conclusão: Obesidade e síndrome metabólica são doenças crônicas associadas a fatores sociodemográficos que contribuem para condições de vida desfavoráveis. Não houve associação dessas comorbidades com fatores reprodutivos.

INTRODUCTION: Obesity is a highly prevalent chronic disease. It is associated with the development of other metabolic morbidities, such as metabolic syndrome. Its etiology is complex and associated with multiple risk factors. In women, the obstetric history may be associated with it. OBJECTIVES: To identify the sociodemographic and reproductive factors associated with obesity and metabolic syndrome in women in the fourth decade of life. METHODS: This is a cohort study conducted from 1978 to 1979 in Ribeirão Preto, Brazil. Sociodemographic, clinical, obstetric, anthropometric and body composition data were collected through interviews and clinical evaluation. Univariate and multivariate log binomial regression models were constructed to identify of obesity and metabolic syndrome. The adjusted relative risk was estimated. RESULTS: The cohort included 916 women: 309 (33.7%) were obese and 286 (31.2%) had metabolic syndrome. Low education (RR 1.77;95% CI 1.33-2.35) and age at first pregnancy < 20 years (RR 1.46; 95% CI 1.10-1.93) were associated to obesity. Unemployed (RR 1.49; 95% CI 1.14-1.95), without stable partner (RR 1.33; 95%CI 1.03-1.72), low education (RR 1.72; 95%CI 1.23-2.41) and age at first pregnancy < 20 years (RR 2.00; 95%CI 1.45-2.77) were associated to metabolic syndrome. CONCLUSION: Obesity and metabolic syndrome are chronic diseases associated to sociodemographic factors that contribute to unfavorable living conditions. There was no association of these comorbidities with reproductive factors.
