Introdução: A melhoria das condições sociais aliada ao desenvolvimento tecnológico e científico obteve como resultado a sobrevivência de crianças medicamente frágeis, portadoras de sérias doenças crônicas, com grande repercussão nos custos assistenciais, principalmente hospitalares. No entanto, os dados sobre o tema são escassos no Brasil. Objetivos: Traçar o perfil epidemiológico dos pacientes pediátricos hospitalizados em enfermaria de pediatria de hospital universitário terciário público do interior do Estado de São Paulo e investigar o impacto das condições crônicas complexas (CCC) na utilização de recursos de saúde e nos custos da hospitalização. Casuística e Método: Estudo longitudinal observacional, com coleta retrospectiva dos dados. Foram avaliados três períodos de um ano, separados por intervalos de cinco anos (2006, 2011 e 2016), com coleta de dados demográficos, clínicos e de desfecho. As CCC foram definidas de acordo com a classificação revisada por Feudtner et al (2014) e Cohen et al (2011b). Os custos hospitalares foram avaliados em três amostras sistemáticas de 100 pacientes, constituídas por pacientes não portadores e portadores de CCC (neurológicas - CCC-NE e não neurológicas - CCC-NNE), proporcionalmente à sua participação no ano da amostra. Resultados: Nos três anos de estudo ocorreram 2372 internações de 2172 pacientes. A proporção de pacientes hospitalizados com CCC aumentou de 13,3% em 2006 para 16,9% em 2016, principalmente à custa daqueles portadores de CCC-NE, que no mesmo período passaram de 6,6% para 11,6% do total de pacientes. Observou-se, ainda, aumento progressivo da sua complexidade, com impacto substancial na utilização de recursos de saúde e nos custos assistenciais, com aumento de 11,6% de 2006 para 2011 e 9,4% de 2011 para 2016. Conclusões: No período do estudo, houve aumento das internações de pacientes portadores de CCC-NE, associado a maior utilização de recursos de saúde e importante repercussão nos custos hospitalares.

Introduction: The improvement of social conditions combined with technological and scientific development has resulted in the survival of medically fragile children, with serious chronic diseases which have great impact on health care costs, especially in hospitals. However, data on the subject are scarce in our country. Objectives: To define the epidemiological profile of pediatric patients hospitalized in a pediatric ward of a public tertiary-care university hospital in the countryside of São Paulo and to investigate the impact of complex chronic conditions (CCC) on the use of healthcare resources and on hospitalizations costs. Casuistic and Method: This is a longitudinal observational study with retrospective data collection. Three one-year periods were evaluated, separated by 5-year intervals (2006, 2011 and 2016) with collection of demographic, clinical and outcome data. Complex chronic conditions were defined according to the classification revised by Feudtner et al (2014) and Cohen et al (2011b). Hospital costs were evaluated in three systematic samples of 100 patients each, consisting of patients without and with a CCC (neurological and non-neurological) in proportion to their participation in the sample year. Results: In the 3 years there were 2372 admissions of 2172 patients. The proportion of hospitalized patients with CCC increased from 13.3% in 2006 to 16.9% in 2016, related to the greater participation of neurologically impaired children, which in the same period rose from 6,6% to 11,6% of the total number of patients. There was also a progressive increase in their complexity, with substantial impact on the use of healthcare resources and costs, which increased by 11.6% from 2006 to 2011 and 9.4% from 2011 to 2016. Conclusions: In the study period, there was an increase in hospitalizations of CCC patients with neurological impairment, associated with greater use of healthcare resources and an important impact on hospital costs.
