A migrânea é uma cefaleia primária de alta prevalência que pode estar associada a complicações isquêmicas. Danos na substância branca cerebral são verificados com maior frequência no território da circulação vértebro-basilar quando há a presença de aura e cujas repercussões clínicas são desconhecidas. O objetivo deste estudo foi identificar a oscilação do centro de pressão (COP), a agilidade e o impacto da tontura em migranosos com aura (MA), sem aura (M) e indivíduos controle (GC). Os pacientes, todos do gênero feminino, foram triados do ambulatório de cefaleia de um hospital terciário. Os pacientes foram diagnosticados com migrânea com aura ou migrânea sem aura por neurologistas especialistas em cefaleia de acordo com a ICHD-II (2004). O COP foi mensurado através da estabilometria com uma plataforma de força. A agilidade foi avaliada pelo teste Timed Up and Go (TUG) e os sintomas de tontura pelo questionário Dizziness Handicap Inventory (DHI). Foram excluídos os pacientes com obesidade, presença de vestibulopatia, doenças sistêmicas ou outros tipos de cefaleia. Foram avaliadas 92 voluntárias, sendo 31 do grupo M (38±10,0 anos), 31 do grupo MA (37±8,0 anos) e 30 do GC (33±9,0 anos). Os resultados revelaram maior oscilação no grupo MA em relação ao grupo M e GC em apoio bipodal com olhos abertos e fechados (p<0,02). MA apresentaram maior deslocamento do COP em relação ao grupo M nas situações unipodal com olhos abertos (p<0,02). Não houve diferença no tempo de realização do TUG (seg.) entre os grupos MA e M, porém ambos os grupos foram significantemente diferentes do GC (p<0,01). Sintomas de tontura foram verificados em 80% do grupo MA e 65% do grupo M, com impacto significante nas atividades de vida diária do indivíduo. A aura parece influenciar de forma significativa a estabilidade postural de indivíduos com migrânea. A migrânea (com e sem aura) compromete o equilíbrio dinâmico e mobilidade. A tontura é prevalente em indivíduos com migrânea, com impacto em vários aspectos da vida diária.

Migraine is a prevalent primary headache that seems to be a risk factor for cardiovascular disease. Furthermore, evidence suggests that white matter and posterior fossa lesions are more likely to happen in migraineurs with aura than in those without aura and the clinical repercussion of these findings is uncertain. Accordingly, herein we contrasted the center of pressure dislocation (COPD), mobility, and symptoms of dizziness, in individuals with migraine with aura (MA), migraine without aura (MO), and in controls (CG). Participants (women) were selected among patients seen in an outpatient headache clinic. Patients were diagnosed with migraine with aura or migraine without aura by neurologists experts on headache disorders according to ICHD-II (2004). COPD was measured with stabilometry using force plates. Mobility was measured with the Timed Up and Go (TUG) test, and dizziness symptoms with the Dizziness Handicap Inventory (DHI). Exclusion criteria included obesity, history of vestibular problems, systemic disorders and other primary headaches. Of 92 volunteers, 31 had MO (38±10 years), 31 had MA (37±8) and 30 were controls (33±9). Subjects with MA had significantly larger COPD relative to those with MO and GC when standing in the bipedal position with their eyes opened and closed (p<0.02). They were also more likely to have larger COPD relative to MO while in the unipodal position with eyes opened (p<0.02). No differences were seen between MA and MO regarding the TUG, although both groups were significantly different than the CG. Dizziness symptoms happened in 80% of those with MA and 65% with MO, with significant impact on activities of daily life. Aura negatively affects the static balance in individuals with migraine. Migraine (with and without aura), in turn, impairs dynamic balance and mobility. Dizziness is prevalent in individuals with migraine, impacting several aspects of daily life.
